Reports j The Monthly Safety and Housekeeping Audits and Injury Summary for March were reviewed. Clean and dirty red rags were stored in a cabinet in stead of approved containers. 1 Audit committees continue to find untreated asbestos material around the plant. Everyone is encouraged to review their area for this material; The use of the term "solvent" rather than the actual chemical name on safety cans can lead to; problems when employees are not familiar with a particular ;area wmch may use this system of labeling. Lack of glass tube rotometer protective covers con tinues to be an item picked up by audit committees. Pressure gage SOC-operating guide marking problems continue to be noted by audit commi-ttees. A mechanic was spared serious injury when he was struck on his hard hat by a large piece of coke. Note-worthy in this incident was that the employee was using breath ing air and had adjusted his hard hat so that it could be worn with the air mask. An employee suffered a mild ankle sprain when he stepped from a ladder to wet grating. Another employee bumped his side when he backed off of a 12" high platform. While these were not serious injuries, they point out the need for all of us to be 100% alert at all times concerning where and how we put our, feet. Congratulations to Bldg. 3 labs, 311 Area Maint. Shop, HMD Synthesis T/F, AOP Unit, Bldg;. 516, 517 T/F and Bldg. 518, 519 and 509 T/F as Good Housekeeping Award winners. II. Incident Reports Reports 79-8 through 79-13 were reviewed.; The following addi tional comments were offered; 79-16 points out need to continue rpview of R.V. installa tions for potential plugging. ; As a result of-this incident and several similar R.V. plugging problems noted in the last;couple of months, Dan Logan requested that this situation be reviewed by the relief valve subcommittee for potential common cause, design problems, etc. The cbmmittee chaired by John Chambard was asked to review and report back to Central Safety. : DUP 0801453 DU 007920 Victoria Plant : Central Safety Committee : Minutes of Meeting Held Tuesday, April 10, 19"9 I ______ Page 5 III. C. F. O'Brien presented a summary of k new on-plant safety meeting program on Hearing Conversation. This material will be available from the Safety Office starting May 1st. Rhythm - J. E. Mitchell A Rhythm training program was proposed for all plant personnel having any responsibility for preparing hazardous materials for shipment. The program will- be conducted with training aids (film strips and tapes) prepared byjthe Hazardous Materials Section of the Transportation and Distribution Department with assistance from the Wilmington Safety 5 Fire Protection Division. The target start-up date is July 1979 with a completion date of December 1980 and will involve approximately 700 people. S F has delayed the next Safe Distribution Survey until 1980 based on the excellent survey report! at Victoria in 1977. V. Industrial Hygiene Audit Summary - J. M, Ka'yser Jim Kayser presented a summary of the Industrial Hygiene Audit program which was started by the plant S ! F Division during 1978. These audits follow a pattern simijlar in scope to the Wilmington S F type audits covering program review and field inspection. An outline summary of this program is attached. VI. Industrial Hygiene Procedures - A. H. Nickolaus Industrial Hygiene Procedures i #2 - Communication #3 - Chemical Inventory #11 - Risk Appraisal #12 - Exposure Control ! were distributed at the meeting. Committee members were asked to review and forward comments to S 8 F before the next meeting. These procedures will be placed on the next Central Safety Agenda for approval. VII. Driving over Work Hoses - G. R. Ehrman : As a result of an accident that occurred in town involving a vehicle driving over a fire hose, G. R. Ehrman asked that the plant review its posture in this area. Si H. Markett and a Construction representative to be designated by S. V. Hinson will review this situation and report back to the committee. VIII. Crane Right-of-Way - R. M. Robertson All areas were asked to review paragraphs!13 14 of S F 67 concerning mobile crane and trackmobile right-of-way on plant streets. Factory Mutual Plant Survey j A survey of the Victoria Plant was conducted by the Factory Mutual System inspection group for insurance purposes. Attached is a letter from Mr. R. R. Ingham, DuPont Insurance Section Manager, expressing his appreciation for our coopera tion during this survey. X. Kev Safety Data The Key Safety Data indicator, frequency rate, of 6.42 for high potential incidents is up over January and February and this upward trend doesn't look good.
