Wynn, safety consultants, on April 20, 1971 Ed Painter, safety coordinator Miss Clara Deptula, R.H. REPORT SUMMARY "We reviewed the accident record with Mr. Ed Painter and Miss Clara Deptula, R.E. An accident to Mr. D. Cunningham on February 10, 1971 > resulted in a foot fracture. This resulted when the injured employee was wearing safety shoes. The piece fell .on Ms safety shoe and bounced to Ms instep resulting in injury. Another accident occurred when the injured ^employee was not wearing safety shoes.and a piece struck his toes also resulting in toe fractures. The supervisor will be required to exercise closer supervision to further prevent this type of injury.-4 "We discussed the asbestos exposure and found that fresh air masks will be provided at the asbestos bins and where respirators are used the operator will sign a log as to the changing of filters. "We found a number of compressed gas cylinders unsecured. All cylinders should be secured to prevent their falling or being knocked over." HO 68 cc: Mr. F. I. Maurer Mr. G. E.
