Kellv -Mgr.Safety INSPECTION REPORT and SAFETY RECOMMENDATIONS This report confirms our safety visit and inspection on the above date. We met with Mr. Carl E. Schank and Mr. Vincent Rizzo. On the tour we were escorted by Mr. Rizzo, and resulting recommendations were reviewed with him at the conclusion of our tour. A safety meeting was held under the direction of Mr. Rizzo. It was decided to again start formalizing safety meetings at this plant. We feel that the committee has a good start and would like to encourage the following items brought out during the meeting. The monthly safety meeting should contain: 1) A review of the prior month's accidents. 2) A report of a plant housekeeping and safety inspection committee. 3) Reports of departmental orforemen safety meetings. 4) Submission of a check list report on condition of fire fighting equipment to be read at each meeting by the fire equipment inspection committee. 5) Safety instruction to up-date and improve the knowledge of the committee. This inspection reveals many violations in the plant, dealing with Federal Regulations, under section 1910.219, which is entitled "Mechanical Power Transmission Apparatus". It is suggested that this paragraph be copied and distributed to all persons in the Maintenance Department, supervisors and others working with guards. In Volume 202, the paragraph begins on page 22289 . and continues through page 22293, which not only contains information as to the requirement of guards but paragraphs fM' and 'O' give very specific and detailed information as to the construction and allowable materials for guards. Many of the recommendations on this report are a result of section (f), paragraphs (3) & (4), which deal with sprockets and chains, and openings for oiling. Section M(2)(i) deals with installation of guards. It is of particular importance that the Maintenance Department be aware of this section, as many of the guards throughout/the plant are improperly fastened. February 6, 1973 Page -2- The recommendations contained in this report represent substandard conditions which we observed during this visit. There might well be others which we did not observe, which could be the basis of a non-compliance citation at the time of an OSHA inspection. It is not intended that these recommendations cover all possible substandard conditions existing at this location at this particular time. SAFETY RECOMMENDATIONS STILL PENDING The conduit pipe and plug at the concrete block wall, near the shingle loading dock should be repaired, as it has pulled away from the wall. Color codes throughout the plant should be adhered to. Specifically noted were hand rails which were painted red. At the end of the wet line, in the Asbestos Department there is a red square of aluminum. This is directly in line with the chain drive of the vaccuum stacker. The red color in position gives a false impression that it is a stop bar. Actually this is only an end guard for the chain. spills near the battery charging areas for fork lift trucks. Previous OSHA inspections have given indication that a water hose will satisfy this regulation, provided it is in close proximity of the battery charging area. Spring loaded safety clips should be installed on all hoist hooks. c The walks over the conveyors to the shingle line should have the installa tion, of mid-rails and toe boards, as these walks are in excess of 4' high and do present a hazard of material_falling ontp personnel, and additionally onto the finished product. -continued- WCH08704 ASSOCIATED SAFETY &. This recommendation refers to the Machine Shop,, pnd all other locations where oxygen and other gases are stored. The Edlund drill press located in the Machine Shop, should have the ^ installation of belt and pulley guards. The entire plant should use only safety signs of an approved design. These are covered in the Federal P.egister under 1910.145. A permanent ladder should be installed on the Stirdovent to replace the portable ladder. The open side of the equipment, located on the 2nd Floor of the large roofing line, should have the installation of the supplied enclosures. This presents a serious hazard of falls into the machinery on the 1st Floor. \ 73^24 The panic door latch, located on the emergency door of the 2nd Floor, northeast side, should be repaired as it is presently inoperable. The fire extinguishers located in the shutter storage area, 2nd Floor, known as the T.N.A. Section, should be remounted. These extinguishers are too high, are blocked, and need a red panel or other approved means of identification. In the T.N.A. Section the door leading to the office area should be unlocked, as it is required that there be two exits from this area. Additionally, "Exit" signs should be placed over the doors. On the 1st Floor, small roofing line, the floor opening to the basement should have the reinstallation of top rail and middle rails, at the front opening. Stronger supervisory control should be exercised over the use of - respirators in the Asbestos Areas. We understand that it is company policy that these respirators be worn here, and it was noted that only approximately 25Z of the employees in this area were actually wearing them. A standard hand rail should be installed on the automatic palletizer side of the conveyor crossover located near same. The inside chain and sprocket drive to the conveyor system, located next to the turn leading into the automatic palletizer, has a guard which is presently attached with baling wire. The present system has a gate which allows the machine oiler to enter the area and close the gate behind him. Particular attention should be paid to section 1910.219f4 of the Federal Register-v^hich gives specific instructions on openings for oiling. (w _/] ;/)^ fhMj. The acceas plates should be replaced on the roll winder. *171 A guard should be installed over the chain and sprocket drive for the hangar conveyor on the Si & #2 machines roofing mill. Guards are needed at both start and finish sprockets. This unit comes out of the electric box on the wall next to the Si machine and with controls labeled "elevator", the wire goes up to the ceiling, travels approximately 15' and drops down near the relays control panel, for the Si machine. All electrical equipment in the Cafeteria should be reviewed for proper electrical grounding. Specific notations were made of the three refrigerators, located in this section. Fire extinguishers in the plant should be reviewed for proper installation. References are made to OSHA 1910.157 and 1910.144, which indicate color codes. Extinguishers pointed out on this trip included those outside the conference room, Cafeteria, outside the personnel office, the stock room of the Maintenance Department, and the extinguisher located near the trim line, Asbestos Department. Complete chain & Sprocket drive guards should be installed on the drive of the bulk weigh units of the Asbestos Department. These drives.presently have one side guarded. However, the exposure is sufficient to warrant further guarding as well. -C The chain conveyor located on the trim line should have chain 4 sprocket guards installed at both the start and finish end. Construction should be in accordance with OSHA 1910.219. A chain or other removable guard rail should be installed across the openings to the autoclaves. When the bridge plates are removed these units have a considerable drop. Persons falling off the edge could sustain seriou personal injury. The pedestal fan located between trim lines of the Asbestos Department should have the ground wire attached. The fan has, at present, the cover removed from the junction box and the ground wire is laying loose inside.
