Such an Index can be developed for this plant. However, It can be significant only if a sufficient sample Is used-- such as, a unit or group supervised by a general operating supervisor or equivalent. The data will be acceptable and reportable only as long as the unit is compared with itself (not in competition with others) and the basis is under stood. The "units" recommended a as follows: Material Handling Utilities Ethylene Styrene VCM AN Oxyge n-Ace ty le ne Methanol Polyethylene 4l Polyethylene 42 Shops Stores Pilot Plant & RPU The Index having a total divided: of 100 points would be A. Injury - based on minor and serious injury data already available. The total value of this category will be worth 40$ of total score. B. Inspection - based on multiple spot Inspections In the "units" covering safety and housekeeping Items (inspection form attached). These multiple five minute inspections will be made by safety Inspectors on random basis. This category will be worth 20$ of total score. LAM015390 PMSB Page 2 C. Incidents -based on fire and spills requiring an alarm and near-misses. The nearmiss is defined as an equipment failure or human error which did or could logically cause: 1. Significant hazard and/or injury to personnel. 2. Emergency shutdown of unit or portion of the unit. 3. Significant equipment damage. 4. Fire or spill handled without outside assistance. The general operating supervisor will determine whether the incident is a near-miss and report by simplified form, (sample attached). Thi3 cate gory will be 40# of total score. Since maintenance field supervision is responsible for the safety of craftsmen in the areas, the scene of the injury or incident will be used to assign the points. There were other possible Index Items considered but rejected for various reasons. Commendations, Reprimands, Awards for Man-hours Safety Meetings Fire Equipment Inspections Relief Valve and Overspeed Testing Hose Inspection Membership in Wise Owl, Turtle, and Golden Shoe Club. The subcommittee feels that such needed experience can be gained In three or four months; therefore, we could be ready to begin actual rating of the units on January 1, 1962. This means a period of "dry runs" to collect base data. It was our suggestion that this be sent to you with the agenda of the August PMSB meeting, and thus provide you the opportunity to study and comment on this prior to that meeting.
