Dear Fellow Inspector: Beginning In February, we are Initiating an Inspection program described in detail in the attached procedure. This program Is designed to In corporate several advantages and stimulus for our safety program.
By sampling, we'will obtain a rolling index of safety performance In the Plant, both by area and total Plant. By inspection, we will see how the other areas are doing their safety Job, sharpen our own eyes to be more observant, and gain the critical observation of others so that we can inqjrove our own areas. By participation, we will involve almost all of the manufacturing supervision in an active and useful safety program. By training, we will learn more about safety and improve our performance.
A con?)lete cycle for this program will take two years, but we will be continually evaluating the value of the program. The time required of each supervisor is nominal, amounting to about one hour a month, and the results can be so rewarding.

This program is designed to measure group and plant safety performance augmenting our Injury statistical program so that corrective measures can be taken to prevent injuries. It Is a safety sampling Bystem focused on personal Injury prevention and is not a measure of process adequacy or hazard.
The Plant Is divided Into twenty-eight areas, listed on the attachment. Each area will be Inspected every other week by a different team of three supervisors. These Inspections will be limited to a maximum of fifteen minutes and the Inspectors will look for safety rule Infrac tions, unsafe actS'Or practices, and unsafe conditions or hazards (housekeeping Items only If serious enough to cause Injury). The In spection team will give no advance' notice but will "check In" with supervision of the area at the time of Inspection.
The Inspection teams---fifty-six teams of three supervisors each--are listed on the attachment. The twenty-eight teams In Group A will make their Inspection during the second week of the month. The twentyeight teams In Group B will make their Inspection during the fourth week of the month. A roaster schedule will be prepared for each of the team captains.
The team captain Is responsible for:
1. Setting the time of Inspection during the scheduled week.
2. Selecting the section or sections of the area to be Inspected.
3. Collecting the team member's reports and preparing a summary Immediately after the Inspection. The summary should be made as an original to be sent to the General Supervisor of the Inspected area and a copy to be sent to the safety department for calcu lation of the Index for the Inspected area and compiled with other reports to obtain a Plant Index.
A sample Inspection sheet is attached. The summary can be In pencil and should include the total In fractions found In each category and a listing of a comment on each of the Infractions found so that the supervisor will know what was found In his area.
On the first Thursday of the month, members of the Group A teams will meet In the Research Conference Room at 1300 for a Safety Training Session. Similarly, on the third Thursday of the month at 1300 In the Research Conference Room, members of the Group B teams will meet for Safety Training. These training sessions will be conducted by the safety director. Initially on how the Inspections will be conducted but later will include other safety subjects. Since two sessions are given each month, each Inspector Is expected to attend preferably the assigned session but If a conflict occurs, then the other session that month.
The first meeting of Group A will be on February 7 and for Group B on February 21, 1963.

The boys on die bicycles are rid ing two abreast insteed of single file, making it difficult for can to pest. Also, they are on the left side of die street when they should ride on the right Cyclists are required to obey die same traffic laws as motorists.
The man using power mower is bore footed, an abominable practice, for one slip and the blade could chop off his entire foot Persons mowing should wear leather shoes, preferab ly safety shoes with a steel tip. Chil dren are playing near foe mower as they should never be allowed to do. And the yard is littered with debris which die mower blade could pick up and burl with deadly farce.
The follow refueling die power mower commits two sins with gaso line: carrying it to a glass fug and smoking while pouring it Vapors could ignite, blowing up to his face.
The man on foe ladder bas made a number of errors. He bas placed tbe ladder at such a shallow angle that ft could slip. Nor is it firmly sup ported at die top; it should extend
about three feet above foe house roof. It has a broken rang. The man is not holding on to the ladder, is standing too for up (fifth rang foam top should be maximum) and is car rying a sharp tool to his pocket which could injure himself or some one else.
'Indoor* Living room: Curled up edge of rug is a tripping hazard. There's 'no screen on foe fireplace; coals could pop out, igniting nearby combusti bles.
A do-it-yourselfer is wntktog on a television that's plugged in; he shouldn't be tinkering with die set at aO, but doing it while it's live" is asking for electrocution. The smoker is using a dangerously de signed ashtray, a fiat one with no grooves or high edges to hold ciga rettes in.
Baaement: Min is using a saw with no guard; thus his fingers could slip under die blade. He's not wearing eye protection to keep wood chips from flying to his eyes.
The tangled "octopus" of cords shows that wiring is overloaded and could cause fire. Woman is cleaning a dress with benzine which is so flammable it should never be used to homes. Tbe basement stairs bas no railing.
Kitchen: Man is taking pills while child watches--a dangerous practice, for children are imitative Mid may take die medicine when you're not looking.
Fot handles are turned outward so
the child could pull bot liquid ou her. Woman is standing on top step of ladder and could dip. A poison bot tle has been thrown in foe waste bas ket where children could retrieve it; even if bottle is empty, foe residue might poison a child.
Bedroom: Baby, left alone, could
roll off bed, or be suffocated by foe

Dudley Hawthorne of Union Carbide at Texas City called to reject the tankcar NATX 14948 because it did not meet the "clear, colorless , no suspended matter specifications". The YCM was the "color of coffee". He had this confirmed by two other lab personnel and alBo sampled the car three times. The car was released back to Monsanto Texas City.

- Carbide has been a good source to dispose of VCM which could not be Bent to Springfield in the past. Generally, Carbide can take almost any material, but VCM with a brown color is certainly not acceptable to them.
SPECIFIC ACTION REQUESTED
By customer - meet the clear and colorless specs in the future.
By marketing -
Production - determine if and where our sample monitor failed to pick up this off-color VCM. Also if we can,determine whether this particular tankcar could have contributed to coloring this VCM. Control Lab - analyse sample of the car after it returns to our plant. Try to identify the source of the color in this shipment.
