Fuller) OSHA Related Subjects The State of Texas is out of the OSHA Inspection business and will now serve industry only in a consulting capacity. They will retain a small group cf inspectors who are available on call by Texas industries. As a result of this change we can expect to have more Federal OSHA inspectors active in Texas. The OSHA report on their inspection of Crescent equipment being used in the plant had not been received at the time of this meeting. However, two copies were received this week and were sent to McCullough and Schwab. The report cited 17 non-serious violations for which Crescent was fined $545. It also provides us with a reminder to regularly inspect our own pole, line, and hoisting equipment for compliance with Monsanto standards (and OSHA regulations). The status of the On-Going OSHA Self-Inspection Program within the plant will be reviewed by the SHAC Committee before the next meeting of the Board. Members of the Board should be reminded that OSHA Inspection Survey Report forms have been previously distributed throughout the plant and that these forms provide a feasible basis for an On-Going Inspection Program. If anyone needs additional copies of this inspection report form, please contact the Loss Pre vention & Safety Department. Vacuum Truck Operator Affected by AN The Board discussed the events surrounding the exposure of a vacuum truck operator to AN. The cause of the problem appeared to be faulty equipment and failure to follow directions of Monsanto supervisors. The procedure for handling vacuum trucks and directions to be given to operators about the hazards of handling waste materials will be reviewed by Plant Engineering (McCullough). The procedure for handling injured contractor employees will be reviewed by Loss Prevention & Safety. Glass will report at the next meeting of the Board. Contractors Safety Meetings Safety meetings have been arranged for all contract personnel working on Plant Engineering projects and maintenance on Thursday, July 24. STG 2431100 LAM016044 ) Plant Manager's Safety Board Minutes 2- - July 18, 1975 SHAC Committee SHAC Committee activities are being described in minutes of their minutes. At the request of the Board, the distribution of these minutes will be extended to superintendents, managers of P-T, P-T managers, and P-T group leaders to get better distribution of the activities and assignments of the SHAC committee. Review of Recent Incidents The Board reviewed the near-miss involving a bicycle/truck accident. Details of this will be issued in a report by N. J. Broussard. The chlorine leak was found to be caused by corrosion at the point of contact with the support in the pipe rack. It has been reported that there are a number of instances where AN lines are in direct contact with pipe supports in the AN-5 project. T. G. Ryan will check on the status of the effort to provide "shoes" on these lines as a part of the project. Ryan will report at the next meeting of the Board. Trafton is responsible for a review of the procedure for the close inspection (by "crawling-the-line" in the pipe rack) on a regular basis for lines in super-hazardous service such as chlorine. The Plant Manager asked the members of the Board to personally review each near-miss incident report and to use them as discussion items in their monthly safety meetings. Review of Statistics The Plant Manager reviewed the downward trend in the safety statistics and pointed out the importance in keeping the pressure on to maintain this direction. He also expressed the need for increased involvement by people in the program and his desire for increased visibility for the personnel aspects of the program. He expressed a desire to see an increased emphasis in monthly safety meetings on job-related safety programs such as Job Hazard Analysis (JHA), Job Audits (JA), Safety Training Observation Programs (STOP). The primary emphasis of these programs is for a person to examine the hazards connected with a job and to determine appropriate action necessary to insure that the job is done in a safe manner. The Safety Department will supply information on the STOP program at the next meeting. STG 2431101 Plant Automobiles The reports of plant autos being in poor or unsafe condition was dis cussed at some length.
