Chairman Manuel Perez opened the meeting by introducing Harry Conrad as guest speaker. Harry introduced Rufus Baca, a member of SHAC, who was attending the meeting as a guest. Harry discussed the rating scheme for the First Quarter Safety and Housekeeping Inspections. He explained that each area was inspected for overall safety attitude and performance by reviewing safety logs, safety procedures, safety meetings, use of safety equipment, etc. Each area was given a score of 1 to 10 in each category and an average was calculated. Each area was also given a score of 1 to 10 in housekeeping. A composit score was calculated by multiplying the safety result by three and adding to the housekeeping score. Ties were broken by giving the highest ranking to the area with the lowest TIR for the past three months. Harry also pointed out that the feedback on the inspections was very positive. Comments from the field indicated that this was one of the most thorough inspections ever conducted. He thanked the members of the Hourly Safety Committee for their participation. Mark Riddle from LP&S commented that we should see improvement and con sistency in safety logs, safety meetings and overall safety practices resulting from this type of inspection. Unfinished Business Cooling Tower Fan Hazard In January, the carpenter representative to the committee stated that a safety hazard existed on all but one of the cooling towers. Even though the fan drive motors are locked-out prior to entering the fan area, the fan turns naturally because of the wind drafts created in the tower. Attempting to stop the heavy fan blades, which sometimes turn at a high speed, is hazardous. SC 001701 M/lfg 29q4 2- - Action Plant Engineering is considering several solutions to this concern and will make a recommendation to the PMSB at the next meeting. Removing Bolts from 51D4 Manway Covers Prior to Decontamination A committee member reported that during the last AN shutdown all but four bolts were removed from each manway cover prior to blinding the column. Action A survey concerning current plant practices is underway. Data from the survey will be furnished to SAC for further action. Extension Ladders An extension ladder was found put up backwards in Zone 1. Barricade Tape A concern was voiced about barricade tape -- tape is being left up after the need no longer exists. Therefore, many times tape is ignored. Actions to Ladder and Tape Concerns A letter restating correct procedures for using extension ladder and barri cade tape was sent to all plant superintendents by the SHAC chairman. Posting HSC Minutes on the Bulletin Boards The SHAC chairman pointed out that the only advantage to posting the minutes was that it made them somewhat more accessable to personnel throughout the plant. However, each committee member receives a copy of the minutes and can make them available to their fellow workers. The main disadvantage is that displaying the minutes on the bulletin board may lead to a situation where the minutes become a means of airing grievances not always related to safety and the committee could lose credibility as a safety organization. For the present, we will continue sending the minutes to all committee mem bers as well as the PMSB and SHAC. sc 001702 LAM002905 -3- New Business Radio Calls Waldo Hughes stated that as more craft leadmen become working leadmen, they are not always able to respond immediately to radio calls from their supervisors. This in itself does not necessarily create a safety problem, but the loud and repeated calls that result when one doesn't answer imme diately become a nuisance and a distraction. Action There is no simple solution to this problem. The matter will be called to the craft supervisors' attention. Plectron Checks Albert solis raised the question as to who checks the Plectrons and how often? Action LP&S is currently working on a plan to test at some regular intervals. Recordable Injuries Manual Perez raised the question, "When is an injury classified as recordable?" Action Mark Riddle stated that many factors are considered and sometimes it becomes a judgement call. Wofford The Tech Center Safety Committee had a special meeting on Monday, March 7, 1983. H. J. Baumgartner sat in for J. R. McGinty. Dave Truitt attended to help present work begun during his tenure on the committee. Rose Mary Fritz described the Competitive Quarterly Safety/Housekeeping Inspection. The question was raised about who would respond to the report issued by the inspection team. G. L. Wofford will look into this. J. H. Cross will accompany the team on the first inspection. The committee decided to continue their bi-annual inspections. The timing of these inspections will be decided at the next meeting. John Garton and Dave Truitt have studied the need for safety guidelines for the pressure regulators used on high pressure gas cylinders. Large numbers of these regulators are now used because the number of gas chromatographs and other analytical instruments has grown in the last decade. Bulletin 6 in the PT Manual has a few scattered guidelines. John described five guidelines and six conclusions. One guideline, color coding of regulators for various gases, was considered unnecessary. H. J. Baumgartner reported that there are no safety guidelines for choosing regulators to buy for the stockroom, which issues most of the regulators used by the Technology groups. There was considerable discussion on how to publish the finished guidelines because Bulletin 6/ which needs a thorough revisio^ may not be widely distributed under the new organizational scheme. This subject will be discussed further at the next meeting. The next meeting will be held on Thursday, March 24, at 0900 hr in the Third Floor Conference Room in Research 1 (CR-8). /bf sc 001704 J.
