The reccnmendations are listed on the attachnsnts as follows: Attachment A - Priority I Items Attachment B - Priority II Itens Attachment C - Priority III Items The items vrere given the following suggested priorities: I. Items involving serious hazards which should be given iirmediate attention. II. Potential hazards which under certain conditions could cause an accident. III. C 101 Install vertical bars in opening between ladder cage and railing. .2 Light ends column Add screens between ladder cage and handrail on 3rd and 5th levels 3. Cooling Tower Rungs should be replaced on all wooden ladders (seme are broken and others in bad shape). 4. HCl Tower Screens should be installed on already existing metal protection from ladder cages to handrails. 5. Heavy ends column Install safety screens in opening between ladder cage and railing. 0000165^ CCR ATTACHMENT B PRIORITY II ITEMS VCM Plant - Lake Charles 1. H 202 A Portable handrail should be installed on both the north and south ends. 2. T 503 Safety bars needs to be installed on 1st platform. Steam pipe running vertically at top section of ladder obstructing clearance of rungs. Safety bar needs to be installed on top platform. 3. Demag Building rHwo ladders need safety bars, installed instead of chains. O^ / 4. T 204 Ladder on north side needs a safety bar installed. 5. Liquid Chlorine Storage Bullet Safety bar needs to be installed on the wast end. 6. S 401 Grating loose on top - should be J-bolted down or welded. 7. Tank Farm Install handrails on crossover dikes. 8. Cooling Tower Install safety bars on all vertical ladders. 9. T 431 Install safety bar on ladder 10. S 208 A & B Install safety bars on ladders on the south side. .11 T 204 Install safety bar on ladder on the north side. Light ends column Housekeeping had on platforms on southside. 3. C 201 Handrail needs repairing on top platform. 4. H 210 A & B Install toeboards on 1st platform on the westside. 5. HC1 Column Replace grating on 6th platform (grating is corroding in spots) 6. H 408 Fin Fan Replace expanded metal for grating and add supports. Install mid-rails on upper deck in places.
