It will provide consistent and accurate identification of both asbestos and non-asbestos containing insulation materials an all piping and equipment There will no charge for this service. To aid with scheduling, each plant will be responsible for appointing a Building Representative. The Building Representative will be contacted by a Dow Asbestos Identification Administrator to establish a "plan of attack" prior to their buildings sampled. The plant will need to provide the Asbestos Identification Administrator with a 8.5"x11" plot plan of each floor and any tank farm areas associated with their building site. These drawings will be used to section the buildings off for easier identification. The Asbestos Identification Administrator will assign a contractor crew to a plant to take samples and label lines and equipment accordingly. The sampling process will be followed by the Asbestos Identification Administrator. Results of the identification survey will given to the plant for their records. Potential asbestos exposures are to be treated in the same manner as a chemical exposure. Safety Standard S-122 clearly defines the required action to be taken In the event of an exposure. Once the employee or supervisor has initiated the correct response, additional requirements to seal off th<? affected area and perform the required corrective actions are defined in IH Standard 160, Section X. Shower Personnel believed to be exposed to asbestos shall shower to remove asbestos material from the clothing to reduce further exposure and tracking of asbestos material. 2. Call Emergency Services on 1-2-3 Plant personnel are to call Emergency Services on 1-2-3 for transportation of employee to medical and to alert medical of the situation. 3. Contact Plant Supervision Plant supervision is to be Informed of the situation and actions taken. Isolate area Plant personnel wearing protective equipment shall barrier off the area and place appropriate warning signs. Within a facility, disconnect and/or seal off ventilation units. __ 2. Communicate Inform employees within the facility of the situation and to avoid entering area. Contact Industrial Hygiene Services for assistance. 3. Clean-up operations Conduct clean-up operations as per IH-160 (Asbestos Containing Material) ensuring that workers are trained and protected with appropriate respiraforfahd protective clothing. B(basement), 1st, 2nd, etc.) 6. SECTION OF BLDG. A. Identify which section of the building this survey sheet is for.(eg. Section A, B, C, D, E, etc. Refer to the existing building layout drawing or develop one for the insulation survey. Each section is to be shown on a separate survey sheet. Tank farms will also be assigned a section and shown on the layout drawing. B. For work in pipe trusses or highlines, identify the street location (eg. G-15to16, for pipe along G St., between 15th and 16th streets). 7. PROJECT START DATE - Identify when the survey for this section was started. 8. PROJECT FINISH DATE - Identify when the survey for this section was completed 9. SAMPLE A. NO. - Identify each sample container and the material where the sample was taken from with a unique number. The number sequence for each building section should start with 1. B. FRIABLE - Identify whether the material is friable. ('Friable' refers to material that can be crumbled, pulverized, or reduced to powder in the hand and can result in the release of asbestos into the air.) Identify with a "Y" in the 'YES" column or an "N" in the 'NO' column. C. NON ASB. - Identify if the material was determined to not be asbestos by putting a ''NO" in this column, (this can be determined by the analytical lab or expert opinion.) D. LOC. * Identify the section of the building as shown in nurpbpr'6 above and indicate which of the following designations is most appropriate for further defining the sample location within the section. ("N" for north, "NE" for northeast, "E" for east, "SE" for southeast, "S" for south, "SW" for southwest, "W" for west, "NW" for northwest, and "C" for center) An example is "A-NE" for a sample taken from the northeast area of section A in a building. 10. EQUIPMENT AND PIPING A. NO. - Indicate the piping, vessel or equipment number. B. SQ.FT.- Identify how many square feet of material similar to the sample exists on the vessel or equipment item in this section of the building. Do Mt include the quantity of material that will be shown on another survey sheet for another building section. C. SIZE - If the sample is from a pipeline, identify the diameter of pipe in inches (4', 10", 24", etc.) D. LNGTH - If the sample is from a pipeline, identify by pipe number and how much pipe, in feet, exists in this section of the building. SAMPLE SOURCE - Indentify which of the three types of material (jacketing, insulation, or mud) that the sample was taken from and indicate where the sample was taken from, by using one the following abbreviations in the appropriate column: BH - bottom head E - elbow 1 - instrument N - neck P - pipe (not at a fitting) T -tee TH - top head V - valve VB - vessel body 11. RECOMMENDED ACTION - Only one of the three columns should be marked for each sample. A. REMOVE - Identify if the asbestos needs to be removed with a "V" in this column. B. REPAIR - Identify if the asbestos needs to be repaired (encapsulated) with an "R" in this column. C. NONE - Indicate that no action needs to be taken with an "N" in this column. Each building will have all piping, equipment and vessels identified with a number, so that the persons sampling can identify it on the Equipment And Piping Survey Worksheet. These piping, vessel and equipment numbers will be ready reference to identify where asbestos materials are still in place. 3. Identify all sample containers with a unique sample number, with the same number on the material that the sample was taken from, for each building area. 4. Sample all pipelines at least once every 20 feet, providing there are no fittings and the insulation appears and feels to be the same material throughout the line. Sample all fittings. 5. Vessels will be sampled at least once on the top head, the bottom head, and the vessel body. If the jacketing or insulation appears or feels different in various locations, each different material should also be sampled. 6. Sample the insulation, the insulation jacketing, and any insulation mud separately, recording the results as different samples for each item. 7. All samples will be recorded on the Equipment And Piping Survey Worksheet. The worksheet is to be filled out completely, accurately, and legibly. 8. Sample locations shall be visibly identified by attaching a survey identification tag to the equipment or pipe at the sample location. 9. All samples shall be taken from the top side of the sample area to prevent asbestos fibers from falling out and becoming airborne. All sample openings will be plugged and encapsulated. When the results of the samples are determined, the survey person shall mark the sample identification tag appropriately and complete filling out the worksheet.
