Section I - INTRODUCTION
4D Oil and Gas, LLC / Unknown Inspection Date: 02/20/2020
PURPOSE OF THE INSPECTION EPA Region 6 inspector K.W. Sanborn visited the 4D Oil and Gas, LLC oil and gas facility located near Barnsdall, Oklahoma on 02/20/2020 for an unannounced inspection. There were no representatives of 4D Oil and Gas, LLC present for the inspection. The inspection was conducted by authority of the Clean Water Act and the Safe Drinking Water Act to determine compliance with the Environmental Protection Agency (EPA) regulations.
FACILITY DESCRIPTION This tank battery was located about 2-3 miles north of Barnsdall, Oklahoma and west of Highway 123.
Section II - OBSERVATIONS The storage facility for produced water (SWF) had a large pit at the facility that had been pumped down prior to the inspection and was lined. I observed the injection well (EPA ID: OS3614) was injecting at 115 psi.
