Section I - INTRODUCTION
PURPOSE OF THE INSPECTION
On August 05, 2014, I, Kenneth L. AuBuchon arrived at the Village of Chama at approximately 9:05 am to conduct an announced Compliance Evaluation Inspection (CEI). I met with Mr. Levi Sandoval/WWTP Operator at the Village of Chama City Hall. I presented my credentials to Mr. Sandoval and informed him that this was an EPA inspection to determine compliance with the facility's NPDES Permit and the Clean Water Act. The inspection was conducted under the authority of the National Pollutant Discharge Elimination System (NPDES) permit program, in accordance with the Federal Clean Water Act.
Before exiting the facility, an exit briefing was conducted to explain areas of concerns noted during the inspection that would be detailed in this report. This inspection report is based on information supplied by the Village of Chama representatives (the permittee), observations made by the U.S. EPA inspector, and records and reports maintained by the permittee, State of New Mexico, and U.S. EPA.
FACILITY DESCRIPTION
The Village of Chama has four (4) lift stations that lift influent to a grinder pump. The grinder pump pushes the influent through a inch bar screen. The influent then enters a splitter box were it flows into the first of two aerated, concrete-lined lagoons (See areas of concerns regarding aeration of lagoons). Flow than enters a serpentine chlorine contact chamber followed by de-chlorination with sulfur dioxide. The effluent exits through a 60 degree v-notch weir into an encased 10 inch pipe to the Rio Chamita.
Section II - OBSERVATIONS
The areas evaluated during the inspection will be rated in Section C of Appendix 1 (EPA Form 3560-3 (Rev. 9-94)) and the observations made during the inspection will be detailed in Section III (Areas of Concern).
Section III - AREAS OF CONCERN
Requirement - Under Part III, Standard Conditions for NPDES Permits, Section B. Proper Operation and Maintenance, Item 3.a - The permittee shall at all times properly operate and maintain all facilities and systems of treatment control (and related appurtenances) which are installed or used by the permitee to achieve compliance with the conditions of the permit. Proper operation and maintenance also includes adequate laboratory controls and appropriate quality assurance procedures. This provision requires the operation of back-up or auxiliary facilities or similar systems which are installed by a permittee only when the operation is necessary to achieve compliance with the conditions of the permit.

Concern: The facility's treatment units are not properly controlled and operated to achieve compliance with the conditions of permit NM0027731. Certified DMRs filed by the facility show discharges of pollutants (Total Suspended Solids (TSS), Ammonia, Total Aluminum, Dissolved Oxygen (DO), Biochemical Oxygen Demand (BOD) and E. coli) that exceed the permitted effluent limitations established in Part I.A of permit NM0027731 (See Appendix 2). The Village of Chama is currently under an Administrative Complaint (Docket No. CWA-06-2013-1863). The primary aeration lagoon was not being properly operated as designed or maintained. The original plant designs (August 1984) show heavy aeration laterals in the primary lagoon.
At the time of the inspection, the header that supplies air to the primary laterals was busted. Mr. Sandoval said that the header had been broken since 1992.

The Village has been granted $8.2 million through capital outlay by the State of New Mexico and has started a three (3) year engineering and construction process to construct a new WWTP. Interim operation and maintenance improvements are needed to improve WWTP performance to bring interim discharges into compliance. Representatives from Molzen Corbin, engineering firm for the Village of Chama, performed a site evaluation on October 22, 2013 at the Village of Chama WWTP to determine adjustments that could be made to enhance wastewater treatment see Appendix 3. Section IV - FOLLOW UP I received the following additional information: August 13, 2014 - facility sample results on a CD (not contained in this report) August 18, 2014 - Chama WWTP - Memorandum on Lagoons and Short Term Improvements via e-mail from Molzen Corbin (Appendix 3).
