Well-nourished, adult colored male. Appears quite happy and cheerful. No evidence of pain or any dis ability. Walks with ease and a normal gait.
Weight; ljgJ Lbs. Temperature: 93. Pulse? 32. Blood Pressure: 130/7^*
States he is taking pills for his head that were prescribed by the doctor at the Charity Hospital of New Orleans, and that he feels much better. Physical examination of heart and lungs, essentially negative. Has no pain or tenderness or distention of the abdomen. No muscular atrophy. Pupils are round and equal and react to light and accommodation. Knee Jerks slightly exaggerated. Hhomberg: Negative. No tremor of the extended fingers. Tympanic membrane is clear and intact. Appetite, good. Patient states he sleeps well.

Well-nourished, adult colored male. Walked into the hospital without anything unusual "bout his gait, balance or oosture. Complains of weakness in back, pain over the chest and head bothering him sometimes.
Weight: 17k Lbs Temperature; 93-2/5. Pulse: 7^* Blood Pressure: 123/76.
Examination of the heart fails to reveal any enlarge ment or murmurs. BreathJww- sounds are normal, no rales Abdomen, negative. No evidence of muscular atrophy. Patellar re*! exes are normal. Has full motion of the soinal column. No limitation of motion. Rhomb erg: Negative. No tremor to extended fingers. Pupils round
and eoual and react to light, du^gish reaction to accommodation.. Tympanic membrane clear and intact. Patient states he sleeps well and has a good aopettte. BtpoA and urine samples are being forwarded for lead determination.
