(conoco) Interoffice Communication To Jim Barton From Alton Reeves Date September 18, 1978 Subject VAPOR RELEASE On the 18th of September at 1945 p.m., Harold Sanders was venting the inerts from 741 Reactor over into the recovery unit when the gasket blew out on the bullseye on the recovery line. Operator Walter West noticed the M.S.A. unit went on and saw the vapor from the bullseye and recovery area. Walter then notified panel operator Harold Sanders of the blow out, Harold then sounded the vapor alarm. Sanders then notified me of the blow out. I then went to the walkway north of the unit and put on a Scott air pack and went to the area and closed off and isolated the K/0 tanks from the blown out bullseye gasket. I then waited a few minutes and closed off the deluge water. I then checked the area with the HNU for any VCM vapor; did not pick up any. Sounded the all clear. I called in maintenance for repairs.
