Further downstream, removal of ten concrete box culverts and concrete rubble used to construct a single - lane stream crossing approximately 16 feet wide and 35 feet long. Restoration of stream banks to pre - impact conditions; revegetation with seed mix specified in approved restoration plan. Construction of low water crossing per specifications in approved restoration plan. Restoration / Mitigation Start Date:Restoration / Mitigation Completion% Complete: 100% November 2015Date: Spring 2017 (remedial measures to address bank erosion) Review of As - Built Report: As - built survey was completed 01/29/16 and submitted to EPA 02/04/16 with photos of restoration site. Visual Observations Overall, the site was robustly vegetated, with significantly greater cover than was observed by EPA inspectors in 2018, when the site was characterized as well - vegetated. Previously eroded banks that were regraded and seeded / planted in the fall of 2016 and the spring of 2017 were observed to remain in good condition and have further stabilized with increased vegetation that has grown since 2018. The low - water crossing remains in good condition following removal of boulders prior to 2018 that were improperly placed along the eastern bank. Upstream, the restored area where the earthen dike had been constructed within and across Plum Creek to divert water remains in good condition. Owner / Operator Statement: In subsequent communication following inspection, current owner indicated green fence posts marking restoration area would be removed, as well as any remaining weathered " CAUTION " tape. All other restoration activity requirements were met prior to 2018 compliance inspection including removal of the earthen diversion dike and associated concrete barriers; removal of concrete box culverts and associated concrete rubble, and replacement with low - water crossing and small amount of riprap; regrading of stream banks and reseeding / planting. There were no environmental conditions present at the time of the inspection that might have an impact on the findings or interpretation of those findings. Air temperature was approximately 80 F; sky was sunny to partly cloudy; calm. Camera did not task satellites for lat / long readings. Camera did record field - of- view directions. Douglas County inspected the site 09/19/18 and found that " All previously disturbed areas are completely stabilized and / or meet the re - vegetation requirements. The temporary sediment controls have been removed. The GESC Permit is closed. " GESC refers to grading, erosion, and sediment control.
