EPA met with MDEQ official, Bailey Long and facility official, Mr. J Thomas Robinson and held an opening conference explaining the reason for the inspection. The inspectors conducted a physical site inspection; reviewed documents; discussed Discharge Monitoring Reports (DMR) submittal and the future operation of the facility. The following is a summary of the RI findings and observations. I. Records/Reports A review of EPA's Enforcement and Compliance History Online (ECHO) database shows "Failure to report" on the DMRs. Mr. Robinson informed the inspectors that Larry Cook, Sr., was the previous operator and no prior logs or documents were made available to him from Mr. Cook. Mr. Robinson became the operator of the facility in October 2020. There were two quarters with discharge since Mr. Robinson's start at the facility. As noted in MDEQ's 2019 inspection report, MDEQ informed Mr. Cook that all records should be kept for three years and that DMRs must be submitted on time. EPA's ECHO database identified the eight quarters prior to Mr. Robinson's arrival for Failure to Report DMR - Non-Receipt. II. Facility Site Review The Farrell POTW consists of a two-cell lagoon system in series with a depth of 6 to 8 feet. The POTW treats the septic overflow of approximately 200 citizens via one pump station. It was not known when exactly the facility began operation. The POTW is designated as a Minor Municipal NPDES facility with a design flow of 0.3MGD. The lagoon system is located on fenced 1-2 acres. The lagoon system utilizes a chlorination and dechlorination station with chlorine and sodium sulfate tabs (Figure 1). The discharge point has a V-notch weir to stairway aeration to a drainage ditch that eventually leads to the Yazoo River (Figure 2). There was some vegetation growth noted with tree trunks of 1 to 3 inches around the lagoon banks that needed to be addressed (Figures 3 & 4). No floating solids were observed during the inspection. There was no discharge at the time of inspection. Mr. Robinson informed the inspectors that no discharge had been recorded in the previous year. There was a turtle carcass that had become wedged in the lagoon's line to the chlorination station. The last quarters of discharge were recorded in October 2020 to March 2021, according to Mr. Robinson. A foamy liquid was observed prior to the discharge point at the weir. When the area floods, the ditch backs up to the chlorination station. The pump station was inspected and there is still washout underneath the pad as noted in the 2019 MDEQ inspection (Figure 5). No evidence of any spills was noted, and the pump(s) were operable. There were only two sampling documents available for review. The chain of custody was not completed by the person taking the sample and relinquishing it. The Permit was not available for review. The DMRs are signed by Charles Fitz, Secretary of Coahoma County Utility District #2. The operating logs did not indicate the person conducting the inspections. Page 3 of 8 The EPA recommends that the facility submit any outstanding DMRs, if possible. The vegetation around the lagoon needs to be maintained. The pump station base should be maintained to prevent any damage to the structure. The chain of custody for sampling events should be properly documented. Operating logs should identify the operators performing routine inspections.
