Included in this proposal is an outline of the project information, proposed services to be performed by Entec Services, Inc., and the project cost, which will apply. Scope of Work We understand the sources are to be tested for the following parameters. A final report will be delivered after completion of the project and received by July 28, 2000 Access to Test Location We request the Georgia Gulf provide access to the location of the test ports on the sources to be tested. Compensation Entec Services Inc. will provide the services outlined in this proposal based on the cost outlined on the following attachment. Cost is based on a ten hour work day, if unexpected conditions or process delays are encountered which are not in control of Entec Services, which should cause the testing to exceed the ten hour test period. Cost will be billed at a stand-by rate of $85.00 hr per two man test team. Authorization To authorize us to provide the proposed services, please issue a purchase order to proceed with the work. Any exceptions to this proposal or special requirements not covered in this proposal should be listed on the purchase order.
