Dear Dr. Kehoe, I am extremely sorry to have to send you the enclosed copy of a letter, dated the 7th March, 1945, from the Anglo-Iranian Oil Co.Ltd., giving the advice of the death of three native labourers at Abadan, as the result of tank cleaning. I am also enclosing copy of Ray*s acknowledgement. There is little I can say about this, since the matter is factually reported in Harrison* s letter. I must say that the Instance strikes me rather bitterly, especially in view of the very thorough discussions which I had on this matter of tank cleaning whilst out in Abadan last year. At the instigation of the General Hfinery!Manager a meeting was held in his office, at which all the senior officials who were in any way concerned attended, making a gathering of some ten to a dozen people We spent about two hours discussing all the aspects of tank cleaning and also repair work, and the potential hazards of the operation were undoubtedly fully appreciated. It is sad to think that, even after this, it takes a fatal occurrence redly to drive home the absolute necessity of ensur ing constant supervision over the actual operation itself.
