This listing has been submitted to HCI, Inc. to be entered into their Phase 4 Hearing Test Manage ment System. HCI has been testing and processing audiograms for APCI for two years. The contractor's audiogram Interpretation and reporting systems have proven useful for those managing plant hearing conservation programs. Calvert City participation in the program will require the following: You will be contacted by HCI In the near future to arrange to have audiometric test forms sent to you. All future------jgrH oqrains should be recorded on. the MCI_fom./Audiometric data may be deleted from the APCI medical data sheet, although it may be desirable to continue to store the audiometric information along with the other medical findings in the APCI computer. Upon completion of the testing, the HCI test form should be sent to the Virginia, Minnesota office for processing. When you receive the test form back, it should be placed In the employee's medical or personnel file. You will receive an updated management report quarterly. I have enclosed a copy of the instructions for using the Phase 4 Management Program. This information should serve as reference material when you begin receiving audiogram management reports. In addition you will find a copy of the Company Hearing Conservation Program Guide and sample training material. Additional brochures are available for distribution to employees. A 15minute video tape on hearing conservation entitled "Sound of Sound" is also available for your training program.
