It is a most interesting program; the only problem will be fitting it into our accustomed time slot. In fact, we may need to continue after lunch on Friday. Please keep this in mind when making your travel plans. Hie hotel notes that the cutoff date for our block of reservations is September 10, 1984. After that date, not only will you not get our $60 rate, you run the risk of not getting a room at all. Five risk management questionnaires are attached to this letter; most members have indicated that such surveys are very valuable so I make no apology for the number. One of them is a follow-up on last year's runaway reaction control survey. John Vaughan also proposes to circulate directly a questionnaire on another aspect of the topic, which has aroused so much interest that a short review of last year's data and conclusions is in the program. n cA 3 iv r $ -O n O' iVPRIAL T 3 cTL020l9g 2- - The oxygen/VCM survey addresses a topic of concern to all of us. The gasholder and recovery system corrosion questionnaires arise because of actual plant incidents which will be discussed by the presenters. The initiator survey is appropriate at the present moment because closed reactor charging has increased the problems in this area, and because a number of alternative forms of initiator are now being offered in the interest of safety. This questionnaire seeks opinions on specific hazards and precautions rather than process details, firstly because this part of the operation is felt to be in a state of flux, and secondly, to reduce concerns about the proprietary nature of some process details. The seriouness of the topic will be highlighted at the meeting by reviews of fatalities caused by different types of initiator system failure. At the last meeting, the general handling of these questionnaires was discussed with the following conclusions: 1. A review of the responses will be presented by the author of the questionnaire at the meeting designated; contributing companies will not be named. 2. Copies of the review and any other data the author thinks appropriate will be sent only to those companies which contributed. 3. Anonymous responses will be accepted but will, of course, not obtain the review. 4. If the author wishes at a later date to make a publication in the open literature including some data from the review he is at liberty to do so, provided the individual data sources are not named. Please indicate if you would not wish your data to be included in such a publication. Ebr maximum benefit, as many responses as possible, with maximum candour are desired. Sometimes actions taken may be considered to be proprietary. In this case, the statement "Proprietary" is preferred to a blank entry, which may be taken as indicating no action at all. Mr. Gottesman's contribution at 4 p.m. on Thursday is outside the official hours of the meeting because the by-laws do not permit him to attend the meeting. This is, however, a very important topic.
