Trafton-0-6 The 2nd Quarter Safety and Housekeeping Inspection for the HCN complex was conducted by the Inspection Team of James Platt, Bob Bohmler, Doug Metten, and Joe Stal. Congratulations! You received an outstanding rating. The following specific items need to be corrected: Safety 1. Operator was unsure of assembly point. 2. Chief operator suggested that the assembly point be moved since the doorway has been moved. 3. Four breakers in motor control center had lockout devices, but no c locks. 4. Steam hose in shop had out of date inspection tape. 5. Yellow hose by 25T9 had no inspection tape. 6. Hose by 25V2-1 was blocking escape ladder. 7. Safety shower sign on upper level by 29E204 was broken. 8. Platform ladder 23-10 had no inspection tag. 9. The cover was off of the pump west of 23T401-2. Housekeeping 1. Unit shop was cluttered with trash. 2. Paper all over process area. 3. Offices were cluttered. Boots, boxes, etc. stored on top of cabinets. 4. A/C room needs cleanup. 5. Skills book for safety equipment training an excellent idea, although some operators' training was incomplete. 3. Dept. 51 has an excellent set of safety check lists which would be a good model for all the plant. 4. Some units have wind vanes which are very helpful during emergencies. 5. Operators throughout the plant felt that once per year Scott Air Pak training was inadequate. Please respond to this report in writing to Jack Trafton with a copy to Steven Moncla by July 15. J. A.
