EPA inspector met with Randall Jones (pumper), Mike Perrier (BIA), and Chris LaViolette (Osage Nation DNR). This was an EPA inspection in response to a spill (NRC#1400427) which was reported by Chris LaViolette. The inspection was conducted under the authority of the Clean Water Act (CWA) to ensure compliance with Environmental Protection Agency (EPA) regulations. FACILITY DESCRIPTION EPA conducted an inspection at the Iron Hawk Energy Group,LLC, McClain Lease. The facility consisted of the McClain lease tank battery and the connected flowlines/wells. Section II - OBSERVATIONS I observed the McClain lease facility signage. See photos No. 1 and 2 of Appendix 1 (photo log). For additional GPS location information and photo metadata, see the attached photo log. I observed the release point of the spill, which was a hole in the steel pipe, located below an oil/water knockout at the McClain lease tank battery. The knockout and piping were located outside of the secondary containment. See photo No. 3. I observed the flowpath of the spill as it exited the facility and migrated NW toward US Highway 60, which was located along the north border of the facility. See photo No. 4. In the ditch of US highway 60, upstream from the Point of Entry (POE), I observed a background TDS field reading of 190.6 mg/L. This should be used when determining background TDS levels for the area. See photo No. 5. The ditch of US HWY 60 was an unnamed tributary of Clear Creek, which was a tributary of Bird Creek. I observed the point of entry (POE) of the spill into the ditch along US HWY 60. At the same location, I observed an elevated TDS field reading of 46,553 mg/L which was the highest TDS reading observed during the inspection. See photos No. 6 and 7 and 8. Continuing east, I observed an elevated TDS field reading of 22,481 mg/L in the impacted ditch. See photo No. 9 and 10. I observed the end of spill impacts along the impacted ditch. At this location I observed a TDS field reading of 241.5 mg/L. See photos No. 11 and 12. 2 Iron Hawk Energy Group, LLC / McClain Lease (OKU002208) Inspection Date 05/30/2024 As I was leaving the facility, I observed a vacuum truck as it began to pick up the impacted water from the impacted tributary. Section IV - FOLLOW UP Inspector will conduct a follow-up inspection to ensure impacted waters were removed from the impacted tributary.
