SUMMARY
A wide project of correlated and integrated experiments, performed
for testing the carcinogenic potentialities of vinyl chloride (VC) (a
monomer of high industrial importance), is presented.

The results showed that VC induces Zymbal's gland carcinomas, nlephroblastomas,
and hepatic and extra-hepatic angiosarcomas, in rats, and pulmonary (tumours,
mammary carcinomas and liver angiosarcomas, in mice.
The experimental data have oriented the clinical observations end the
epidemiological investigations, which have recently brought to the 4etectln of the early cases of liver angiosarcomas among exposed workers in the
U.S.A. and in the U.K. The experimental carcinogenicity bio-assays of VC should be considered,
therefore, highly predictive. They represent a convincing example for
potentiating experimental testing for all new industrial compounds, and for
the ones already widespread, but on whose biological effects we are still
ignorant.

In the past 10 years, the number and amount of oncogenic agents produced
and scattered in the working environment have increased and continu^ to
increase in a distressing way. These agents emitted by factories eijiter into
the general environment and thus become an ecological problem.

As a result of this situation, an increase in occupational turners and
of external environmental agents in a broad sense is recorded.
Most oncogenic agents present in the human environment have be^n
identified on Che basis of retrospective epidemiological studies. {

Epidemiological proof should be avoi_ J For all new agents prod uce and used, systematic experimental tests should rather be carried out t predict their possible oncogenic potential.
However, such tests have generally not been carried out in a systematic way, mainly for two reasons. The first one is the absence of standards. The
second one lies in the fact that the design and performance of tests sre often
not adequate, and, therefore, the results of such tests cannot be extrapolated
to man; these tests have generated a certain amount of skepticism.
In our opinion, experimental tests, if correctly formulated and parried
out, have a high predictive value. An example of such tests are those reported here. To our knowledge, this
is the first time that an oncogenic hazard, the importance of this hazard,
and the target bases (foundations) can be predicted with absolute accuracy by
means of an experimental test. The present studies deal with carcinogenicity tests on vinyl chlpride
(VC). This monomer is of great industrial importance, and is used in the
production of polyvinyl chloride (PVC), as a copolymer in the production of
Saran and other plastics, as a solvent and a chemical intermediate product.
For a short time, it has also been used in anesthesiology. The annual VC
production is about 12 million tons.
Workers who risk
exposure to this product are;
1) Workers engaged in production of PVC and other resins containing
PVC, in the production of VC and in the industrial use of VC for
other purposes. 2) Workers who use PVC.
3) Residents of areas adjacent to plants producing VC and PVC. 4) Although to a minor extent, those persons using or coming
into contact with plastics based on VC or with VC-containing
materials.

The project for testing the oncogenic potential of VC follows tlje pre
liminary results of the pioneering work done by Prof. Viola, presented at the
10th International Cancer Congress (Houston, 1970), later published jn
"Cancer Research in 1971, (Viola et. al., 1971).

The results obtained by Prof. Viola showed that exposure to 30,000 ppm VC for 10 months, 4 hours/day and 5 days/week induced tumors in rats. In the interpretation of Prof. Viola, these tumors originated from the $kin, lungs, and bones.

Immediately after the Houston meeting, we contacted Prof. Viola, who graciously made available to us the histological sections of his tumors and his extensive report, which was still in the publishing stage at that
time. On the basis of this material, we reached the following conclusions: 1) Under the experimental conditions used by Prof. Viola, VC clearly showed a carcinogenic effect in rats. 2) The amount of 30,000 ppm VC is an extremely high dose. 3) The tumors described as cutaneous tumors originated from Zyrabal glands, which are sensitive to a wide range of carcinogens, such as 2-acetylaninofluorene, benzidine, polycyclic hydrocarbons, aminostilbene, urethane, etc. Tumors described as pulmonary tumors were actually metastases of
Zymbal gland carcinomas; locations of pulmonary neoplasms were all nated in animals carrying Zymbal gland tumors and were entirely similar to thjEese tumors.
However, there is an urgent need for systematic research to furltther clarify the type and degree of the carcinogenic effect of VC, partialularly in regard to experimental factors correlated with exposure, such as the administration route, concentration, duration, etc., which can provi|ide more direct indications on the hazards of conditions found in an occupati)onal environment.
In this respect, the Institute of Oncology and the Bologna Turner Center are particularly specialized in studies for the prevention and control of tumors, and the Montedison Co., the major Italian producer of VC, htis been directed to strengthen our program of tests on the biological effects of VC. The Montedison was later joined by ICI (Great Britain), Solvay (Belgium), and Rhone-Progil (France) (Cooperative European Group for the Study of the Biological Effects of VC).
Prior to initiating the operational phase of this research, it was necessary to prepare suitable conditions for exposure by the inhalai :ion route.
The study and the construction of an apparatus for the programmed and controlled exposure has required several months. Control is perfo:arjcd by means of gas chromatography. Such an apparatus allows exposure to concentrations
ranging from 30,000 to 50 ppm, and the simultaneous treatment of 1200 test
rodents. The apparatus consists mainly of stainless steel and crystal.

VC was provided by Montedison, j'ach VC batch was analyzed prior to
use to determine any eventual inpurities present; the VC used by us contained
negligible amounts of impurities.
Tests were carried out mainly on Sprague-Dawley rats, but Wistar rats,
Swiss mice, and hamsters were also used. All animals, except hamsters, were
bred for years at our Institute, and whatever their use, they were subjected
to a complete autopsy, and for this reason their current spontaneous pathology
is known to us.
A consecutive series of 14 tests was initiated. Some of these tests
were programmed from the beginning, while others were retained (held back) as
necessary later, on the basis of the first experimental results.
The design of the tests at the present date is shown in Table I.
Essentially, we wanted to verify (check) the oncogenic action of VC
in relation to various factors, such as the administration route, dosage,
duration and rhythm (measure) of treatments, animal species, strain, and age.
In addition, we wanted to test the eventual possibility of a transplacental
action of the compound.

All animals were observed until their natural death, and a complete
autopsy was performed on each dead animal. As a rule, the following organs
and parts were examined histologically: Zymbal glands, interscapular fat,
salivary glands, tongue, lungs, liver, kidneys, spleen, stomach, various
segments of the intestines, bladder, encephalum (brain), leg bones, and
every other tissue with pathological lesions. All animals exposed tcj higher
doses (30,000 and 10,000 ppm), with or without tumors, were examined i
radiologically during treatment and/or at the tine of death, to chec^ the
appearance of eventual acrosteolytic lesions.
Animals exposed to lower doses, when carrying tumors, were also
examined radiologically.
Tables II-VI show preliminary data relating to test nos. BT1, BlJS,
BT6, BT5, and BT4. In regard to the other experiments, we must still) wait
to obtain any results.
From the data given in the tables and histological studies, the following
points can be derived:

1) VC has an oncogenic effect under the experimental condition^
tested by us. It induces Zyrr.bal gland carcinomas (Fig. 1),
nephroblastomas (Fig. 2), and angiosarcomas of the liver
(Figs. 3, 4) and of other tissues and organs (Figs. 5, 6)
in the rat; in the mouse, it induces pulmonary adenomas and
adenocarcinomas, mammary carcinomas, and hepatic angiosarcomas.
2) There is a relationship between dose and response. 3) Nephroblastomas and Zymbal gland tumors can be bilateral.

4) The hepatic angiosarcomas are polycentric.

5) In the animals treated, with or without angiosarcomas, diffuse
vascular alterations in various tissues and organs are often
present, consisting of a simple, distypical (untypical ?) and
frankly atypical endothelial proliferation. The oncogenic
action on the vessels is, therefore, considered as a systemic
effect.

6) The observation of three ossifying angiosarcomas suggests new hints (cues) for a different pathogenetic interpretation of the acrooteolysis found in workers exposed to the action of VC.
7) Up to the present time, we have not observed any acrosteolytic lesions in rats treated by us.
Carcinomas of Zymbal glands, hepatic and extrahepatic angiosarcomas and nephroblastomas have never been observed in untreated rats of our Sijrague-
Dawley line. The only three cases of Zymbal gland carcinomas noted [in
untreated rats were found by Dr. Tannenbaum et. al., in 1962; thesejanimals
were Sprague-Dawley rats.

To the best of our knowledge, spontaneous nephroblastomas and hjeoatlc
angiosarcomas haye never been reported in rats. Nephroblastomas havle never
been induced experimentally.
Hepatic angiosarcomas have never been induced experimentally in rats
with diethylnitrosamine (Reuber and Lee, 19GS; Hadjiolov, 1972).
In mice, hepatic and extrahepatic angiosarcomas have been induced with
O-aminoazotoluene (Andervont et. al., 1942; Andervont, 1950) and with
p-dimethylaninobenzene-l-azo-2-naphthalene (Mullay and Saxon, 1953), and also
with other agents, including urethane, diethylnitrosamine, dimethyln:.trosamine7
and dimethylhydrazine.
In the rabbit, angiosarcomas have been induced with Thorotrast j
(Zeithofer and Speiser, 1954). In man, angiosarcomas are rare.

the status of angiosarcomas described in the literature, collected 11|7 such cases, of which only 21 were hepatic angiosarcomas; there seems to bej an
increase in the number of such tumors in recent years.

In man, the literature has recorded one case of hepatic angiosarjeoma
induced by radium (Ross, 1932) and numerous cases of hepatic angiosarjeomas induced by Thorotrast ('lac Mahon et. al., 1947; Ludin, 1953; Da Silva! Morta
et. al., 1965) and arsenic (Roth, 1957).
As soon as it became clear, about 11/2 years ago, that VC provides an
explanation of the oncogenic effect described here, wc have immediate;.y
communicated this fact to not only
the Society of the European Cooperative
[
Group, but also to other major producing companies in the world. Our data
have provided a point of orientation (direction) for clinical obsorvadions and
epidemiological studies.

At the end of December, 1973, a worker in an American VC plant di^d
from a liver disease, an autopsy disclosed the presence of an hepatic !
angiosarcoma.

Retrospective studies performed during the past few weeks have diLclosed
3 similar cases (7_ in the USA and 1 in Great Britain) of hepatic angiosarcomas,
which in the past either have not been correctly diagnosed or have not been
correlated with occupational exposure (Figs. 7, S). Such a number of cases
will probably increase in the future.
In the USA and in Germany, some workers exposed to VC and suffering
from a suspected liver disease, are now kept under constant observation*
la conclusion, we hope that experimental carcinogenicity tests will
become a rule prior to the approved release into the ambient atmosphere of
new agents, and that more work is done in that direction and less timej is
spent on scholarly discussions (which is often done at sessions of committees
and congresses) concerning- the validity and extrapolation to man of experimental
results, for which basic requirements are often lacking.

SUMMARY. -- A wide project of correlated and integrated experiments, performed for testing the carcinogenic potentialities of vinyl chloride (VC) (a monomer of hi^h industrial
importance), is presented. The results showed that VC induces Zymbal's gland carcinomas, nephroblastomas, and
hepatic and extra-hepatic angiosarcomas, in rats, and pulmonary tumour^ mammary
carcinomas and liver angiosarcomas, in mice.
The experimental data have oriented the clinical observations and the epjdcmiological
investigations, which have recently brought to the detection of the early ejases of liver
angiosarcomas among exposed workers in the U.S.A. and in the L'.K.

The experimental carcinogenicity bio-assays of VC should be constdeijed therefore
highly predictive. They represent a convincing example for potentiating experimental testing for all new industrial compounds, and for the ones already widespread, but on whose
biological effects we are still ignorant. .
Jn queste ultime decadi il numero e la quantita di agenti oncogeni prodotti
e diffusi nell'ambiente di lavoro sono aumentati e vanno aumentando in ma-
niera preoccupantc. Dalle fabbriche gli agenti oncogeni vengono immessi
nell'ambiente generale diventando in tal modo un problema ecologico.
Quale conscgucnza di questa situazione vicne registrato
aumento
dei tumori professional! e da agenti esogeni ambientali in senso |ato.
La maggior parte degli agenti oncogeni present! nell'ambientd dell'uomo
sono stati idcntificati sulla base di studi epidemiologici rctrospdttici.
La prova epidcmiologica dovrebbe essere evitata. Andrebbero \ iuvecc ese-
guiti per tutli gli agenti di nuova produzionc ed uso sisteniatici saggi sperimen
tali per predire le loro eventuali potenzialita oncogene.
Tali saggi non vengono tuttavia eseguiti in genere in maniera sistematica,
essen2ialmente per due ragioni. La prima consiste nella mancanza di una
normativa. La seconda nel fatto che, poiche spesso il disegno e la jeonduzione
degli esperimenti non sono adeguati e pertanto lc loro risultanze rjon estrapo-
labili airuomo, attorno ad essi si e creato un ccrto scetticismo.
A nostro avviso invece il saggio sperimentalc, se correttamentB impostato
e attuato, ha un alto valore predittivo.
Ne e un esempio quello che qui vi riferiamo. A nostro concjscenza e la
prima volta che mediante un saggio sperimentale vicne previsto don assoluta
precisione il rischio oncogeno, 1'entita del rischio e i substrati bersaglio.(*)

Le presenti indagini si riferiscono a saggi di cancerogenicita del cloruro di vinile (CV).
Questo monomero e di grande importanza industriale. Esso e usato nella produzionc di cloruro di polivinile (CPV), come copolimcro nella produzione di saran e di altre plastiche, come solvente e come intermedio chimico. Per un breve periodo e stato impicgato anche in anestescologia. II CV e prodotto nella quantita di circa 12.000.000 di tonncllate all'anno.
Vanno incontro a rischio di essere esposti a questo prodotto:
1) gli operai addctti alia produzione di CPV e di altre resine di cui e costituente, alia produzione di CV e alia utilizzazione industriale di CV per altri scopi;
2) gli operai che utilizzano il CPV; 3) gli abitanti di zone circostanti le fabbriche in cui viene prodotto CV e CPV; e 4) sc pure in misura minore, coloro che utilizzano o vengono a contatto con plastiche a base di CV o con materiali in esse contenuti.
II progetto di saggiare il potenzialc oncogeno del CV fece seguito ai risultati preliminari del lavoro pionieristico del prof. Viola, presentato al X Congresso Internazionale sul Cancro (Houston, 1970) e che lu in seguito pubblicato su Cancer Research nel 1971 (Viola e coll., 1971).
I risultati del prof. Viola indicavano che I'esposizione a 30.000 ppm di CV, 4 ore al giorno, 5 giorni la settimana, per 10 mesi, produceva tumori nel ratto. Neirinterpretazione del prof. Viola questi tumori originavano dalla cute, dal polmone e dalle ossa.
Immediatamente dope il resoconto di Houston entrammo in contatto con il prof. Viola, che gentilmente mise a nostra disposizione le sezioni istologiche dei suoi tumori e il suo resoconto in extenso, ancora in pubblicazione.
Sulla base di questo materiale noi raggiungemmo le seguenti conclusioni:
1) nelle condizioni sperimentali del prof. Viola il CV mostrava chiaramente un effetto cancerogeno sul ratto;
2) 30.000 ppm di CV erano una dose cstremamente alta; 3) i tumori clcscritti come cutanei originavano dalle ghiandole di Zymbal, che sono sensibili ad un largo spettro di agenti cancerogcni (2-acetilaminofiuorene, benzidina, idrocarburi policiclici, aminostilbene, uretano, ecc.); 4) i tumori descritti come polmonari erano in realta metastasi da carcinomi delle ghiandole di Zymbal; le localizzazioni neoplastiche polmonari erano osservate tutte in animali portatori di tumori delle ghiandole di Zym bal ed erano in tutto e per tutto sirr ili a questi tumori.
Si impose pertanto 1'urgenza d: ricerche sistematiche per chiarire u!teriormente il tipo e il grado di effetto cancerogeno del CV con particolare riguardo a quci fattori sperimentali, correlati con 1'esposizione, quali la via di somministrazione, la concentrazione, la durata, ecc., che possono dare piu dirztte indicazioni circa il rischio delle condizioni dell'ambicnte professional.

A questo proposito poiche l'lstituto di Oncologia ed il Centro Tumori di Bologna sono particolarmcnte spccializzati in studi di prevenzione q controllo dei tumori, la Montedison, la maggiore p-oduttrice italiana diCV, si torientata a potcnziare il nostro programma di saggi sugli efTctti biologici dell CV. Alla Montedison si sono in seguito unite la ICT (Gran Brctagna), la Solvaiy (Belgio) e la Rhone-Progil (Francia) (GruppoCooperative Europco per lo Stiudio degli Effetti Biologici del CV).
Prima di iniziare la fase operativa di questa ricerca e stato Inccessario preparare condizioni idonce per l'esposizionc per via inalatoria. r
Lo studio e la costruzione di un apparato per esposizione projgrammata e controllata ha richiesto alcuni mesi. Il controllo viene cseguito mediante gascromatografia. Un tale apparato rende possibile l'csposizione a concentrazioni variabili da 30.000 ppm a 50 ppm, e il trattamento simultanfco di 1200
roditori da esperimonto. Essenzialmcnte 1'apparato e costituito jdi acciaio
inossidabile c cristallo. Il CV e stato fornito dalla Montedison. Ciascuna fornitura e stBta analiz-
zata prima dcll'uso per determinare le eventuali impurita: il CV da noi usato conteneva impurita di ordine trascurabile.
Gli esperimenti sono stati condotti principalmente su ratti | del ceppo Sprague-Dawley; sono pure stati usati ratti Wistar, topi Swiss e crjiceti. Tutti gli animali eccetto i criceti sono stati allevati nel nostro Istituto iper anni e qualunque sia stato il loro uso essi sono stati sottoposti a complete autopsia, per cui noi conosciamo la loro patologia spontanea correntc.
Sono stati iniziati in sequenza 14 esperimenti. Alcuni di essi! sono stati
programmati fin dall'inizio, altri invece sono stati ritenuti necessari .in seguito,
sulla base dei primi risultati sperimentali.

Essenzialmente abbiamo voluto verificare 1'azione oncogen^ del CV,
in relazione a vari fattori: vie di somministrazione, dosi, durata h ritmo dei
trattamenti, specie a ccppo animalc, ed eta degli animali,
Inoltre abbiamo voluto saggiare 1'eventuale possibility d'azione traspla-
centare del composto.
Tutti gli animali sono tenuti sotto osservazione fino a morte Ispontanea.
Alla morte su ciascun animale viene eseguita una autopsia combleta. Sono
esaminati istologicamcnte di regola: le ghiandole di Zymbal, il g:*asso inter-
scapolare, le ghiandole salivari, la lingua, i pojmoni, il fegato, i rerli, la milza,
lo stomaco, diversi segmenti dell'intestino, la vescica, I'encefalo, Id ossa delle
zampe e ogni altro tessuto con lesioni patologiche. Tutti gli animali esposti
alle dosi piu alte (30.000 e 10.000 ppm) con o senza tumori vengonp esaminati
radiologicamente durante il trattamento e/o alia morte, per verificare Finsor-
genza di eventuali lesioni acrosteoiitiche.

Sono stati esaminati radiologicamente anche gli animali esposti a dosi piu
basse, quando portatori di tumori.

Nelle Tabelle 11--VI vengono esposti i dati preliminari relativi agli esperi-
menti BT r, BT 3, BT6, BT 5, BT4. Per quanto concerne gli altri esperimenti dobbiamo attendere ancora per avere risultati.
Da quanto presentato nelle Tabelle e dallo studio istologico emergono i seguenti punti:
1) il CV e oncogeno nelle condizioni sperimentali da noi saggiate: esso provoca carcinomi delle ghiandole di Zymbal (fig. 1), nefroblastomi (fig. 2) e angiosarcomi cpatici (figg. 3, 4) e di altri tessuti ed organi (figg. 5, 6) nel ratto, e adenomi-adenocarcinomi polmonari, carcinomi mammarii e angiosar' comi epatici nel topo;

2) esiste una relazione tra dose e risposta; 3) i nefroblastomi e i tumori delle ghiandole di Zymbal possono essere bilaterali; 4) gli angiosarcomi epatici sono policentrici; 5) in animali trattati, sia portatori di angiosarcomi o no, sono frequentemente presenti alterazioni vascolari diffuse a vari tessuti ed organi, consistent! in proliferazione endoteliale semplice, distipica e francamente atipica. L'azione oncogena sui vasi va ritenuta pertanto un effetto sistemico; 6) 1'osservazione di tre angiosarcomi ossificanti suggerisce nuovi spunti per una diversa interpretazione patogenetica deiracrosteolisi in operai esposti all'azione del CV; 7) fino ad oggi nei ratti da noi trattati non abbiamo osservato lesioni acrosteolitiche.
Carcinomi delle ghiandole di Zymbal, angiosarcomi epatici ed extraepatici, e nefroblastomi non sono mai stati osservati in animali non trattati della nostra linea di ratti Sprague -Dawley. I soli tre casi noti di carcinomi delle ghiandole di Zymbal, in ratti non trattati, furono riscontrati dal dott. Tannenbaum e coll, nel 1962: si trattava di ratti Sprague-Dawley.

A nostra conosccnza sul ratto non sono mai stati riportati neftoblastomi
e angiosarcomi epatici spontanei.

I nefroblastomi non sono mai stati provocati sperimentalmente.
Angiosarcomi epatici sono mai stati indotti sperimentalmente nel ratto con dietilnitrosamina (Reuber e Lee, 1968; Hadjiolov, 1972).
Nel topo angiosarcomi epatici ed extraepatici sono stati indotti con
tf-aminoazotoluene (Andervont e coll., 1942; Andervont, 1950) e cod ^-dimen-
tilaminobenzene-i--azo--2-naftalene (Mulay e Saxen, 1953), e con altri agenti
tra cui urctano, dietilnitrosamina, dimetilnitrosamina e dimetilidrazina. Nel coniglio angiosarcomi sono stati provocati con thorotrast (Zeitlhofer
e Speiser, 1954). Nell'uomo gli angiosarcomi sono rari. Landells nel 1958, facentfio il punto
sulla situazione degli angiosarcomi descritti in letteratura, ne racccglieva 117
casi, di cui solo 21 epatici. Essi pero sembrano andare aumentand(o in questi
ultimi tempi.

In letteratura sono stati riferiti nell'uomo, 1 caso di angiosarcoma epatio da infissione di radium (Ross, 1932) e numerosi casi di angiosarconhi epatic da thorotrast (MacMahon e coll., 1947; Liidin, 1953; Da Silva Horia e coll. 1965) e da arsenico (Roth, 1957).
Non appena ci fu chiaro, circa 1 anno e mezzo fa, che il CV jesplicav? I'azionc oncogena su riferita, ne demmo immediata comunicazione Inon. sole alle Societa del Gruppo Cooperative Europeo, ma anche alle altre maggior Societa produttrici del mondo.
I nostri dati hanno orientato l'osservazione clinica e l'indagine dpidemiologica.
Alla fine di dicembre del 1973 un operaio di una fabbrica ameficana d CPV veniva a morte per epatopatia.
L'autopsia rivelava trattarsi di angiosarcoma epatico. L'indagine retrospettica condotta in queste ultimo settimane ha( portatc al riscontro di 8 casi analoghi (7 degli USA e \ in Gran Bretagna) di Angiosarcomi epatici, che in passato o non erano stati correttamente diagnpsticati 0 comitnque non correlati con l'esposizione lavorativa (figg. 7, 8). Tale; numero e dcstinato ad aumentare. Negli USA e in Germania alcuni operai esposti al CV, con epatopatia sospetta, sono ora sottoposti a costante osservazione.
A conclusione vogliamo auspicare che saggi sperimentali di jcancerogenicita diventino una rcgola prima che venga approvata rimmissijme nel* l'ambiente di nuovi agenti, e che si lavori di piu in tal senso e meno njella scolastica discussione (spesso occasione di Comitati e Congressi) sulla validity ed estrapolabilita all'uomo di risultanze sperimentali a cui mancanq troppo spesso requisiti di base.
