Date/Shift Z/4 - 3-C frit CfrG-tJCU) - JP" Comments Po - ^ The drive belt on one of the 30 gallon polys broke, causing the reaction to go out of control and bursting the 500 psi rupture disc. It could have been because of the timing of the sensors, but the second floor fire door (Stream 1) recorded over 100 ppm, while the third floor (Stream 6) showed only 3 ppm. The wind was from the South and the VCM vapors were probably being drawn down the leeward side of the building and back into the building by the slight negative pressure we normally operate under. Area was below 2ppm in one cycle. 3/8 - l-C Stream 4 had I reading of over 100 ppm which was not detected by any other sensor or with the OVA. I was in the building at the time,we could not find a thing. We later found that the VCM filter flange was leaking, which could have been the sour but it didn't last. We now have a new portable analyzer which is very reliable and is greatly helping to find these phantoms. 3/8 - 3/22 Having problems with computer printer which has prevented making logs, but monitoring system is working fine. Comment level and alarm levels were lowered during this period. The overall percentage of readings above 2 ppm was 1.68 percent. On 3/9 the C shift, 10 percent of the readings were above 2 ppm. During this period the wind was from the North and the West. Operators comments showed a number of operations producing a variety of excursions. During this feriod, we did not have the Working Foreman available to follow through when we had a problem. The 1100 gallon operators who would generally enter the comments, were also quite busy. We now have a Working Foreman on each shift except on the weekend, which is much more effective.
