Mr. provided the commercial general liability declarations page of the insurance policy that would cover the deductible. The insurance policy expires on 11/15/2023. The certificate and declaration page are provided in Attachment H. Physical Inspection During the site visit, Mr. reviewed documents on site and observed the fill ports, STPs, and one dispenser sump, and the ATG monitoring interface for the two USTs on site. Photograph #9 in Attachment B shows the interface for the Red Jacket Prolink ATG system. Attachment C includes an inventory report for the USTs. Mr. also observed the spill bucket and fill pipe for the two USTs. The spill buckets for both tanks were dry and in good condition at the time of the inspection. Mr. observed a flapper valve in each fill pipe. Mr. also observed the STP for both USTs, which were in direct contact with the ground. Electronic ALLDs were observed on the STPs. The junction boxes with the STPs were in good condition. Access ports for ATG probes were also present for each of the tanks, and the cover for the Tank 2 ATG probe was welded closed. There were two pump dispensers on site and one of these were opened as part of the inspection. Dispenser piping was in direct contact with the ground as shown in Photograph #10 in Attachment B. Mr. did not observe signs of leaks at the time of the inspection. No monitoring wells were observed at the tank field during the inspection. Vent lines are located on the south side of the building and no obstructions or concerns were noted for the vent lines at the time of the inspection. . Harris Oil and Propane CEI Report 3 June 2023 2. SUMMARY Tank 1 has a 10,000-gallon capacity and stores regular unleaded gasoline. Tank 2 has a 10,000- gallon capacity and stores regular unleaded gasoline. Tank 1 and Tank 2 are STI-P3 tanks and were installed in 1989. Only five passing months of records for Tank 1 and four passing months of records for Tank 2 were provided for the previous 12 months. An annual certification for the ATG system was not provided during the inspection. There are two dispensers on site that operate with pressurized steel piping. Passing testing records for the ALLD functionality tests and the annual line tightness tests were provided. STPs for the tanks have electronic ALLDs and are in direct contact with the ground but no visible signs of a leak were observed. The fill pipe for each UST is equipped with a spill bucket and a flapper valve as spill and overfill prevention equipment. No hydrostatic test results for the spill buckets or functionality test results for the flapper valves were provided. A galvanic cathodic protection system evaluation of the UST system was conducted on 04/22/2021. No other galvanic cathodic protection system evaluation was provided. Mr. is a Class A and Class B operator. Mr. is a Class C operator. Monthly walkthrough inspection records were provided for 2023 only. The facility provided a Certificate of Registration that expires on 07/31/2023 showing it is part of the petroleum tank release compensation fund and the commercial general liability declarations page of the insurance policy that would cover the deductible.
