I can help you get the summary of the incident or any other info you need from the Tech center database or you can get it yourself if you access to the database. Contact Rita Lacina at the Tech Center if you need access. I also have access to the Midland reactive chem database if you need info from it. I have already put together a fairly extensive listing of the incidents and Reactive chemicals testing done from having searches done on the the reactive chemicals database.
