D., Chairman The interim meeting convened at 9:00 A. M., April 20, 1957, at the Jefferson -ctel, St. Louis, Mo. Committee members present were: Dr. W. 0. Armstrong, Dr. J. crookshank, Dr. Robert Eckardt, Dr. T. M. Frank, Jack Spence, and committee chairman. 15 visitors were also present. After introduction, Dr. Carl Nau, Chief Investigator of the project, re viewed to the committee all work that has been completed to this date, which included a discription of physical facilities, test procedures, and results. A 15 minute color film, which showed the gross appearance and response of the test animals under specified test measures, was presented to the group. The data accumulated reveals that the response of the test rats to the con trol compounds (Benzene and N-Decane) in comparison with the responses of the animalsto C9-C12 fractions exposures is inconsistent in terms of pathological findings. Objectively, the only consistent- findings were the narcotizing and anesthetizing effects of the C9-C12 fractions. The subcommittee concurs with the chief investigator that: 1. None of our data appear to us to be especially significant insofar as the objectives of our study are concerned. 2. Rats do not seem to be desirable animals for our studies. Although the data on mice testing are still incomplete, the results thus far obtained indicate that no significant findings will be developed. The committee recommends that: 1. Studies using mice should be continued until sufficient data for conclusions are available. This should require about k months more time. 2. The subcommittee agreed that this program proceed as originally conceived using a different species of test animal - namely the monkey. 3. The subcommittee is requesting monies in the amount of $20,000 to carry out this work for the fiscal year 1958-1959* API 07296 T EXHIBIT 3 TC MUrJISS CF 3RD MEETING Frcgram Subcommittee B. B. Turcot The chairman announced that Dr. George W. Starbuck, Chairman, Child Accident Prevention Committee, American Academy of Pediatrics, who had been invited to attend this meeting was unable at the last minute to do so and sent his sincere regrets. II. In reference to Appendix E of the minutes of the 22nd MAC meeting on the subject of kerosine poisoning: A. The Medical ar.d Health Committee of the API Board has approved the $5000 requested for 3957-58 for research in the field of treatment of kerosine poisoning. It has not yet acted on our other recom mendations pertaining to the educational programs. Dr. L. E. Curtis, MAC Chairman, has been invited to meet with the Medical and Health Committee on April 25, 1957 in Dallas at which time he will discuss the educational programs outlined in the recom mendations. B. With reference to the treatment of kerosine poisoning the Subcommittee on Economic Poisons will proceed to contact organizations and individ uals interested in this field for their specific recommendations and proposals on research. Our specific recommendations to the MAC will be made after receiving and evaluating all pertinent information. The subcommittee unanimously requests Dr. Curtis to appoint a member to our subcommittee who is an expert in the medical evaluation of this type of research. C. With reference to the Educational Programs, pending the action of the Medical and Health Committee our subcommittee will continue in informal contact with several interested organizations. Such contact has been made to date with representatives of the American Public Health Asso ciation, American Medical Association, and the American Academy of Pediatrics. No contact has been made with other groups. API 07298 -2 - C-2 D. With reference to Poison Control Centers, the subcommittee wishes to announce that in Washington under the U.S. Public Health Service a "National Clearing House for Poison Control centers" has recently been organized. This "Clearing House" is organizing an Advisory Council. Our subcommittee recognizes the desirability of maintaining contact with this development with the idea that the petroleum industry may wish to seek representation thereon. III. New Business A. The subcommittee recognizes tne need to review the possible health impj.i cations in the use of petroleum products which are used by themselves as economic poisons in agriculture. A list of such pro ducts will be requested from the API Committee on Agriculture for our review. IV. The Subcommittee requests that this report be accepted by the MAC. API 07299 r ' EXHIBIT D TO MUJUIES OF 2;RD MAC MEETING Progress Report on Toxicological Reviews (Presented by Philip Drinker in absence of H. W. Gerarde, M. D., Chairman) The literature on the following substances has been searched from the date of the last report until the present. 0(a) Benzene ) Cyclohexane (c) Methylcyclobexane (cl) Napthenic acid ( e ) Copper napbthenate (f) Mesitylene (g) Napthalene ( h ) Aromatic solvent naphtha (i) Butadiene A great deal of nev material has accumulated in reference to the toxicity of benzene. The pertinent results are being summarized and will be incorporated into final form along with the information of tne previous reports. Less extensive revision will be required for the remaining agents lir-nc-I above. In the case of toluene, xylene and ksrosine a rather large amount of literature has yet to be searched completely. This is currently being undertaken. The final revision of a number of these substances will be forthcoming in the near future. API 07300 EXHIBIT E m*av MINUTES CF 2;?.D MAC MEETING Report of the Subcommittee on Industrial Hygiene Paul D. Halley, Chai jman , American Standards Association Committees A. ASA-Z37 Committee on Maximum Acceptable Concentrations of Toxic Dusts and Gases: Mr. A. E. Dooley, API Representative to the Z-37 Committee reported on their activities. A standard for formaldehyde has been submitted to the ASA for approval. A standard on carbon tetrachloride has already been approved and is ready for sale. A summary of tentative standards being considered by Com mittee ASA-Z37 with information considered pertinent has been pre pared by Mr. Dooley. Copies of this summary will be made available to the MAC Subcommittee on Toxicological Reviews. B. ASA-ZU Committee on Industrial Sanitation. Mr. Dooley reports that this committee which is concerned with the promulgation of standards for industrial sanitation has not been active the past 6 months. The main business of the Subcommittee was to review a monograph titled "Health and Physiological Safety in the Process Industries" prepared by the M. W. Kellogg Co. This manual had been obtained by Dr. Curtis and was circulated to the members of the Subcommittee in order to obtain their thoughts concerning its value to the API. The Subcommittee recognized numerous technical inaccuracies in the manual and recommends that it should not be distributed in its present form. However, the Subcommittee wishes to compliment the M. W. Kellogg Co. for initiating a project of this magnitude. In considering the merits that a revised text of Kellogg would have to the oil industry, Subcommittee members were of the opinion that such a publication would have some value, but that at the present time, the Subcommittee is not in a position to undertake a project of such volume and scope. Accordingly, the Subcommittee voted to defer for one year, a decision on the preparation of such a manual. API 07301 EXHIBIT F `T ' r * S-' MINUTES CF '3RD MAC MEETING Report of the Subcommittee or. Dermatitis Leo J. Vaie, M. D., Chairman Inasmuch as I shall not be present in St. Louis, I an asking Doctor Newquist to review for the Medical Advisory Committee the present status of the attempt to provide some industry-wide advice to consumers of cutting fluids and other petroleum products. We have been told by Mr. Rumoshosky that the Joint Subcommittee which sponsored the project at the Industrial Hygiene Founda tion is considered disbanded. The Hygiene Fanel of the Lubrication Committee has been in communication with Dr. Braun, of the Industrial Hygiene Foundation, concerning the preparation of an A.P.I. booklet on the safe handling of cutting fluids. Dr. Braun and his associates were unwilling to commit themselves to any further course of action without having an opportunity to review the materials now available in the industry for customer education. To that end, the Hygiene Fanel recommended the following: " 1. Have the Panel Secretary bring up to date his collection of current bulletins issuea by oil marketers on preventive hygiene and request the marketers to furnish supplementary information that their Medical Departments can contribute. An A.P.I. issue should then be prepared as a composite by a recognized organ ization such as Industrial Hygiene Foundation, who would be credited for the release, indicating that it was from an unbiased source. . "2. Arrange for publication of an article in a suitable trade journal, placing the incidence of dermatitis due to petroleum product exposure in proper perspective. This article would be based on the Dermatitis Survey information as soon as it is released for such use by Industrial Hygiene Foundation." It is quite apparent that the assistance of our committee is not wanted at the present time. Unless members of the Medical Advisory Committee feel otherwise, it is our opinion that we might well bide our time until a collection of information by Mr. Erickson has been completed. D., Chairman The Subcommittee on Carcinogenicity has held several meetings since the last meeting of the Medical Advisory Committee. Basically it has been concerned with *wo problems during this interval. The first of these problems concerns the potential carcinogenicity of waxes used on milk cartons, and was precipitated by a meeting of the Cancer Prevention Comnittee of the International Union Against Cancer, held in Rome in August 1956. At that time, it was proposed in newspaper releases of the Cancer Prevention Committee that milk carton wax was suspect as a carcinogen. Although it is the opinion of the Subcommittee that milk carton wax presents no hazard to the consuming public, the same cannot be said about other waxes which may come in contact with food. Nonethe less, it is believed that the newspaper reports of the meeting in Rome cannot be ignored and that the petroleum industry shujid prepare itself to show conclusively that milk carton and other waxes do net present a hazard to the consuming public. It is the belief of the Subcommittee, as a result of the review of the literature, that the petroleum industry today is not in a position to make an adequate defense of this position. In view of this, therefore,'the Subcommittee recommends, as already recommended to the MAC at its November meeting, the following courses of action: 1. A survey of wax uses which will bring wax into contact with foods or into use as food packages should be reviewed. 2. Biological tests of selected waxes from the above uses should be undertaken in accordance with the outline of carcinogenic testing submitted to the Subcommittee by one of the three following laboratories: a. Dr. Philippe Shubik's laboratories at the Chicago Medical School. b. Dr. Lloyd Hazleton's laboratories under the . direction of Dr. Ivor Cornman. c. Dr. Norton Nelson's laboratories at the Institute of Industrial Medicine. The specific proposals of these three laboratories are available in the files of the Subcommittee and can be made available to any group which has a valid interest in them. The Subcommittee recommends the above three laboratories in the order indicated, although it suggests that any of the three labora tories would be in a position to conduct competent studies on this problem. API 07303 - 2- G-2 3- The Subcommittee, and any of its members, is willing to cooperate with and/or participate in any Joint interdivisional committee which cay be appointed to consider the problem of wax carcinogenicity. It is suggested that some representation from the MAC would he advisable on any such Joint interdivisional Committee. . U. It is estimated that these studies will cost in the vicinity of $60,000 to $100,000 per year and should extend for an interval of at least three to five years, or possibly longer. The second problem considered by the Subcommittee was to follow the carcinogenic work of the Kettering Laboratory and in this respect the Subcommittee reports as follows: 1. It accepts the report of Dr. Phair on the epidemiological work. It urges that every effort be made by the Ket tering Laboratory to prepare a final report of these epidemiological studies by July 1, 1957 2. It accepts the Interim Report of Dr. Horton, dated March 20, 1957. 3. It recommends that $35,2kO be appropriated by the API MAC to continue to work of the Kettering Laboratory until July 1, 1959 It recommends that the work at Kettering Laboratory be terminated at that time. It points out that Dr. Kehoe has suggested that the petroleum industry might find it advisable to con tinue to support the Kettering Laboratory at some modest level in its carcinogenic investigations for an indefinite additional period. The Subcommittee is not sure what benefits would arise from this con tinued support. Dr. Kehoe suggests that the benefits might lie in the fact that the technology of the petroleum industry may change in the coming years and that this would make it possible for the petro leum industry to keep abreast of changes in the carcinogenicity of petroleum products with such contemplated future changes in the technology of the petroleum industry. The Subcommittee suggested that if a specific proposal for a specific amount and type of support of this work can be obtained from the Kettering Laboratory, then consideration be given at that time for 6uch continued support. Staples, Chairman The Medical and Health Committee met Thursday, April 25 and acted on recommendations of the Medical Advisory Committee as follows: 1. It approved the MAC budget for expenditures totaling $77,500 for the year ending June 30, 1958 included in the API budget approved November, 1958. 2. It authorized the Medical Advisory Committee to proceed with the research project on clinical and physiological effects of air pollution using the special fund cf $20,000 as authorized by the Executive C t ihtee pursue.?..: to the endorsement by the General Committee Bivisim cf F.vfinirg, November lb, 1958, which also was included in API budget approved November, 1958. 3. It authorized the chairman of the Medical and Health Committee to appoint a committee composed of 1 member of the Refining Division, 1 member of the Marketing Division and 1 member of the Medical Advisory Committee to study and recommend the need of and ways and means of investigating the toxicology of petro leum waxes as heretofore recommended by the MAC. b. It authorized the chairman of the Medical and Health Committee to refer to the vice president of the Marketing Division the recommendation of the MAC that a sum of $5000 be provided for financial assistance to organizations outside of the oil in dustry which sponsor programs for the protection of children against kerosine poisoning. Economic Poisons API 07307 J. W. Osborn, M. D., presented the Subcommittee on Economic Poisons report which is given as Appendix F. There was considerable discussion as to whether the requested work with Dr. Nau should be started now or should be postponed until after the April reeling. Likewise, considerable discussion resulted frcm the recommends tion that, further wcrk should be done cn animals rather than clinical. D., Chairman of this Subcommittee, had written Chairman Curtis that there was no report from this Subcommittee. H. Physical Agents Lamson Blaney, M. Halley, Chairman of the Industrial Hygiene Subcommittee, presented his report which is given as Appendix I. ACTION: It was regularly moved, seconded, and unanimously passed that this report be accepted. There was some discussion relative to the status of the industrial hygien ist in the petroleum industry. The following action was taken: ACTION: It was regularly moved, seconded, and unanimously passed that the Medical Advisory Committee recognize the importance of industrial hygiene as an essential function of occupational health in the petroleum industry and that the industrial hygienist should be accorded professional status. III. REPORTS BY LIAISON MEMBERS A. Dermatitis Survey Leo J. Wade, M. D., reported that there was no longer a liaison with the Lubrication Committee. He reported experience with skin cancers in his company. B . Asphalt Institute R. E. Eckardt, M. D., stated there was nothing to report as there had been no liaison activity. API 07308 i -k - C. Smoke and Fumes Coirmi ttee J. C. Ruddock, M. D., reported that the liaison with the Division of Refining's Smoke and Fumes Committee was excellent. They were extremely cooperative in supplying copies of their committee's actions and keeping him informed otherwise. S. American Standards Association Chairman Curtis reported that the American Standards Association activities on the Z - k , Z-37, and Z62 Committees were of direct interest to the Industrial Hygiene Subcommittee. Reports were made directly to the Industrial Hygiene Sub committee . E. American Medical Association J. W. Osborn, M. D., reported that he had been in recent conversation with Dr. Torald Sollmann relative to the AMA Committee on Toxicology's Proposed Conference of Governmental, Industry, and Medicine for the purpose of drafting a model law on labeling of hazardous chemicals. Dr. Sollmann told Dr. Osborn that his committee's original concept of a simple example of a model law when presented to the AMA legal department was rejected as being non-specific. The AMA legal department is now closely studying the proposed laws that are now before Congress. Dr. Sollmann anticipated that his Committee on Toxicology will probably present its recommendations to the AMA Council at its meeting, November 1st and 2nd. F. Labeling M. N. Newquist, M. D., reported that the Interdivisional Committee on Labeling has completed an API Labeling Bulletin which will include several recommended labels in addition to the general philosophy of labeling. This Bulletin will be available about the first of the year. G. First Aid Training T. M. Frank, M. D., reviewed the activity of the Centred Committee on Accident Prevention and its proposed publication of an Administration Guide to help instruct the instructors of first aid training. He indicated that the API First Aid Training Guide was now translated into the Arabic language. There were l601 qualified first aid instructors from 73 companies. IV. EXECUTIVE SESSION A. 1958-1959 Budget Chairman Curtis explained that the Operating Subcommittee had considered the 1958-1959 Medical Advisory Committee Budget and had recommended it to the Medical and Health Committee in order that it could be presented to the Board of Directors during their November meeting. V. NEW BUSINESS A. By letter dated October 29, 1957, Dr. Philip Drinker transmitted to the xembers of the Medical Advisory Committee copies of the Harben Lectures which he delivered in England on May 13, I k , and 15, 1957- ACTION: It was regularly moved, seconded, and unanimously passed that Dr. Philip Drinker be extended a vote of thanks for trans mitting copies of his lectures to the Medical Advisory Committee. B. George M. Saunders, M. D., complimented the chairman of the MAC for the very competent way he had discharged his duties as chairman for the past two years. All present voiced a similar sentiment. C. The desirability of having a round table conference at the April 1958 meeting was discussed. The desirability of suggesting nominees to the Medical and Health Committee to receive a Certificate of Appreciation was discussed. Staples, Chairman of the API Medical and Health Committee presided. Dr. L. E. Curtis made introductory remarks In regard to the activities of the Medical Advisory Committee and presented a watch to Mr. David Stroop (Dave to our group) in token of the great esteem held by the API MAC for Dave, who retires from the API. The principal speakers for this meeting were as follows: Jack R. Ewalt, M.D., Clinical Professor of Psychiatry, Harvard Medical School, Cambridge, Massachusetts. "Emotional Problems in Industry". Louis N. Katz, M.D., Michael Reese Hospital, Chicago, Illinois. "Your Heart -- Your Job". These men were excellent and it is too bad we did not have a larger group to hear them. Either one would be competent to speak before any API general meeting. Some thought might be given to having such men at a general meeting of the API where more members would be able to hear their speeches. In the afternoon, the Round Table Dlscussional meeting was held. The subjects were: Poisoning from Petroleum Distillates. Dr. J. W. Osborne Noise Control Techniques in Refineries. Mr. Franklin W. Church Current Hygienic Limits and Concentrations for Petroleum Products. Dr. C. H. Hlne There were 3^ members in attendance for this meeting, which Included both doctors and hygienists.
