Ka 1 de nbe rg Bubba Bean Paul Siems The Fourth Quarter Safety and Housekeeping Inspection for the Cyanide Complex was conducted by the Inspection Team of Don Kaldenberg, Paul Siems, Bubba Bean and Bob Hammann on 1/24/84. Overall the unit was in good condition. Response to the following recommendations should be made to G. T. Ryan with carbon copy to Steve Moncla by 2/21/84. General Comments The Inspection Team would like to commend the unit for excellent records kept on safety equipment. The safety board in the control room, safety storage room and overall general cleanliness of the control room were excellent. Personal Knowledge of Safety Alarm code was not legible on the phone behind the control panel. Personal Safety Equipment 1. One used fresh air mask in box by laboratory. 2. Five minute escape mask on north side of Dept. 23 ground floor low in pressure. 3. Hearing protection area was marked by CT-2. Since there are only three small areas in Dept. 29/27 that are at or slightly above the Monsanto guide line for ear protectors, plans are to work on engineering controls instead of requiring hearing protection. This needs to be implemented in the very near future or hearing protection should be required. Condition of Safety Equipment 1. Door to deluge valve house needs to be replaced. 2. Cracked plexiglass door on the sample cabinet by 29V103. Unit should be strongly commended for installing sample cabinets throughout the area. Safety Hazards 1. Thor fittings need to be replaced on N2 stations on the second deck of Dept. 23. 2. Pavement needs leveling east of Dept. 23. SC 000616 LAM001814 J. E. Jennings, George Bryan Page 2. 3. Equipment (pipe, valves, hoses, etc.) scattered on the pavement outside maintenance shop. 4. Safety shield necessary for wearing when grinding is in poor condition. 5. Scaffolding on the deck on the northwest corner of the second floor of the ACY unit. Housekeeping 1. Ladder not in rack on 3rd floor ACY structure. 2. Painters' tools in cabinet on southeast corner of second floor ACY structure. 3. Area around the refrigeration units in HCN storage area needs to be cleaned by maintenance. 4. Insulation trash by 23E202 south. 5. Trash in old SO2 building. 6. Ground cable by Dept. 25 PIB separator is imbedded in the ground and needs to be stored properly. 7. Dirty buckets and mop in the motor control center by CT 2. 8. Question if the one ton hoist in Dept. 23 north side of first deck is operable. If so, has it been inspected? / dgb R. T.
