Comments on Statistics There were no Recordable Injuries during the month of July although a total of twenty-six (26) Industrial Injuries were recorded. This com pared with sixteen (16) Industrial Injuries for June of this year and twelve (12) for the month of July last year. Striking-Against and Lifting-Pushing-Pulling-Type accidents caused over forty (40Z) per cent of the Total Injuries. C. Onits and Groups with Rising Trendlines Rising trendlines for at least the last two months have been recorded in Power 2, Dept. 53, Dept. 51, Dept. 513, Truckers and Laborers, and Carpenters. Areas indicating a rising trendline outside of control for only one month were Dept. 48/56 and Dept. 33. D. Contractor Safety--C.M. McCullough The Recordable Injury Rate for July was the same as June where a reduction in rate had taken place. Injury rates are still lower than recorded over the previous few months. E. CEP Project Safety--B.C. Lancaster They reported on two Recordable Injuries: 1.) Where a Pipefitter collapsed on top of a Dept. 51 tank; and 2.) Where a Laborer received blisters on his feet from an unknown source. McKee Two studies are presently in progress but they have no recommendations to report at the present time. III. REPORT OF THE SAFETY AND HOUSEKEEPING ACTION COMMITTEE (SHAC)--C.M. Cruse SHAC is surveying the Ladder Inspection Program by talking to individuals throughout the plant to determine if the end results of the program are being effective. They are presently evaluating the results of their survey. The major activity of the SHAC committee is to gear-up for the up-coming plant inspection. The inspection dates have been set as September 23, 24 and 25. IV. STATUS OF ACCELERATED VESSEL INSPECTION (AVIP)(SHAC)--C.M. Cruse The Accelerated Vessel Inspection Program is 93% on schedule. This is an improvement from the 91% reported for June. The number of vessels pastdue for inspection has dropped from 118 to 90. It should be noted that while we are 90 vessels behind schedule, a large number of vessels have also been inspected ahead of schedule. V. STATUS OF THE S-T-A-R-T SAFETY PROGRAM--C.M. Cruse SHAC has made a survey of the Expanded Staff personnel to determine the present status of the S-T-A-R-T Program. They received answers that varied from "Yes, they are on schedule", to "No, they have not started", to questions of "What is S-T-A-R-T?" Operations and Maintenance are about 50% complete through the Foreman level. The PMSB suggested that the next "SET" be started in September. VI. LOSS REPORTING--J.E. Fox/R.T. Berg) VIII PLANT MANAGER'S COMMENTS It should be obvious to everyone that the injury rate is going up fairly rapidly and that several of the injuries that have occurred have been of a more serious nature. There should not be a let down in safe practices or safety awareness.
