All functions (except Head Office) show improvements vs. 1988 year-end rates. Seven of 10 Manufacturing locations have no "days away" cases in 1989. Total re cordable incident rates are not as encouraging. Manufacturing is trending up overall; improvements in chemical are outweighed by deterioration within refining. Overall Products shows a 10% improvement with excellent progress in most functions. ACHIEVEMENTS Belpre Chemical Plant During the month of March, Belpre's UE&C Catalytic Maintenance contractor employees reached a milestone of one-half million hours without a "days away from work" case representing a period of time in excess of two and a half years. Shell Pipe Line Corporation On April 6, 1989, the employees of Shell Pipe Line Corporation completed 1,500,000 safe hours worked since July 12, 1988. This is the best performance for Pipe Line since mid-1986. The reinstatement of Cal-OSHA was a result of Proposition 97 which was passed last fall. Federal OSHA will continue concurrent enforcement in Califor nia until September 30 when the state's revived program is expected to be in full force. INSPECTIONS Deer Park Mfg. Complex - Non-Serious Asbestos Standard Citation A non-serious recordkeeping citation under the Asbestos Con struction Standard has been received for not having summarized in a single document the basis for exempting "incidental" removal of asbestos insula tion from the standard's monitoring requirements that call for an evalua tion of each job. Deer Park had performed the required air sampling to meet the exemption and shared the results with employees, but had not formally de signated those jobs which would be considered exempt. Products' locations should review their compliance position with regard to paragraph (N) of the Asbestos Construction Standard. Additional guidance will be distri buted to field locations. SELECTED NEAR MISSES Handling Pyrophoric Materials A laboratory technician was using a "dry box". When she removed her samples, she also brought out a paper towel that had been in the box and that was apparently contaminated with pyrophoric material. On expo sure to air, the towel burst into flame. This incident highlights the importance of handling pyrophoric materials with care and cleaning up af ter work. Overhead Lighting Fixtures Recently, a plexiglass and metal cover of a fluorescent light fixture in a manufacturing location control room fell 10 feet from the ceiling to the floor. This commonly used light fixture was located di rectly over the unit Emergency Board and in a high traffic area. Fortu nately no one was injured. An investigation revealed that cover locking devices were not properly secured and eventually opened due to vibration. As a corrective measure, the location repaired the cover prior to rein stallation and checked other fixtures to assure that covers were locked.
