The first guest speaker was George Danner, General Superintendent, Plant Engineering. George reviewed the procedure bulletin P-0113, "Injury, Illness, or Near Miss Investigation Reporting," and the pumper gauger injury resulting from HC1 release from a tank truck.
C. M. Cruse, EB Plant Manufacturing Superintendent, presented an overview on a special tank entry and cleaning procedure which will be used in the near future on a trial basis.
c OLD BUSINESS East Plant Steam Vent Noise
o The noise level in the area east of the 16T23 condensate tank reduced considerably over that reported last month.
o Process changes were made which reduced the amount of steam vented from the 16T23 (unfortunately it won't save steam). A noise level survey indicated the need for ear protection signs near the 16T23 tank--signs will be posted. Several plant projects now under consideration should result in venting and noise reduction in the near future.
Yellow Caution and Red Fire/Spill Tape
o Concern was expressed in the last meeting over non-Monsanto personnel knowledge of the hazard involved when yellow/black or red tape is used. Some Monsanto employees are also not up-to-date on the meaning.
o In order to communicate this plantwide, we will re-issue.the bulletin board announcement covering the intended use and difference between the tapes. However, this will not be issued until LP&S completes a search for a suitable red tape with the label "ENTRY REQUIRES SPECIAL PERMISSION."
NEW BUSINESS
o During the HCN alarm of Tuesday, April 20, there was some confusion over what maintenance personnel were to do. Some employees entered or continued work in the area because fellow workers indicated it was a false alarm.

o The HCN alarm in the Alf-5 plant on April 20 was a false alarm. Mark Devore, AN Operating Supervisor, indicated that the procedure has not changed. During an HCN alarm, all personnel within the unit should report to their base area (maintaining an upwind position from the spill). Personnel outside the unit should stay out.
Minor Injury Forms
o In a recent incident, a boilermaker was cleaning exchanger tubes and felt short of breath. He continued working, but at the suggestion of the fore man went to first aid for oxygen. He was forced to fill out an injury form. Can't oxygen be given in a case of non-chemical exposure without filing an injury form?
o Tony Ozerchuk of the LP&S Department indicates that in those cases where treatment at the dispensary is not due to a work-related injury or an alleged work injury, an injury form is not required. Instead, treatment and diagnosis is recorded in the dispensary log.
However, in the incident described above, the injured stated that he in haled a chemical from the exchanger he was working on. In this case, it must be classified as an injury in order to meet State law and Monsanto local and corporate requirements. The injury report form requests sufficient information to classify the injury so as to prevent future incidents of a similar nature.
It is imperative that minor injuries be reported immediately so that initial treatment can prevent the injury from developing into a more serious mecical condition.
In addition, immediate injury reporting protects the employee by allowing medical coverage as an industrial injury (workman's condensation).
1982 Recordable Injury Frequency
1982 recordable injuries total seven. The year-to-date safety record continues to be no better than 1981. Discussion was generated around the reasons for safety performance worse than we are capable of doing.
Members of the Hourly Safety Committee were requested to come to next month's meet ing prepared to address suggested means of improving our safety performance.

1982 SHAC ACTIVITIES PMSB REPORT--APRIL 20, 1982
First Quarter Safety/Housekeeping Inspections
All inspections will be complete this week (last one scheduled today). Inspection reports will be issued and responses will be requested on all items.
HSC March Meeting
Attendence was 4 of 9 members (one absent triple heart bypass, three out of town, one training--HDPE transfer). Carlos Navarette discussed 1982 SAC objectives. Recent P-Bulletin issues "Fire Protection and
Equipment" and "Bypassing and Testing Protective Devices" were reviewed. Safety suggestion log books were discussed.

C Status of Investigation Report Recommendations
In keeping with a request from G. L. Tromblee, the status of investi gation report recommendations was reviewed. Recommendations of reports issued after October 1, 1981, were checked by contacting the persons responsible for completion. The results were overwhelmingly positive. Although not always completed on time, all recommendations were at or near completion.
Vessel Inspection Program Status
The total plant VIP status is 97.8% complete (significantly better than last quarter). Nine deferment requests were approved.
SHAC Portable Building Action Request
A SHAC Action Request is in preparation for issue by month end. Enough copies for all superintendents reporting to PMSB members will be provided.

deletions.
Will need to evaluate reason for having buildings and develop action plan for removal or develop justification for continued use.
