Section I INTRODUCTION

PURPOSE OF THE INSPECTION
EPA inspectors Fatima Shaikh and Heidi Rausch arrived at the City of Monroe Water Treatment Plant (WTP) at approximately 9:00 am on May 7, 2025, for an announced Safe Drinking Water Act (SDWA) inspection of the City of Monroe Water System, Public Water Supply (PWS) ID No. LA1073031. The inspection was part of the National Enforcement and Compliance Initiative (NECI) Measure 1.4 which aims to evaluate and address 50% of all Community Water Systems (CWS) serving populations greater than 10,000.
We met with Nathan Morris, City of Monroe WTP Superintendent; Sean Benton, Water System Manager; Joctoria Price, City of Monroe Water Distribution Superintendent; Kenneth George, City of Monroe Network Administrator; Jeshirl Brice, City of Monroe IT Director; Tamala Bailey, City of Monroe Compliance Specialist; Ieshea Hollins, Direnzic CISO; Dewayne McKinney, Direnzic Analyst. Barbara Featherson, Tyler Lollis, and Derek Delo from LDH also attended the inspection. Fatima Shaikh and Heidi Rausch presented their credentials to the system representatives and informed them that this was an EPA inspection to evaluate the system under the SDWA. Additionally, the inspectors evaluated the Risk and Resilience Assessment (RRA), Emergency Response Plan (ERP), and cybersecurity practices under the SDWA Section 1433(a) and (b), which require community water systems that serve more than 3,300 people to develop and certify completion of an RRA and ERP.
This report summarizes observations made during the inspection, which consisted of the following
activities:

FACILITY DESCRIPTION

The City of Monroe Public Water System (PWS) is a community water system located in Tallulah, Madison Parish, Louisiana. The City PWS has 19,000 service connections and serves a population of 57,000 customers, according to Louisiana Department of Health Drinking Water Viewer database. The water system is a surface water system with water supply coming from Bayou Desiard and the Ouachita River. The plant consists of conventional treatment including aerators, flocculators, sedimentation, filters, chlorination, ammonia feed, alum feed, pumps, and storage tanks.
There are eight operators at the plant. The plant operates on 12-hours shifts from 6 am to 6 pm, with two operators per shift. The supervisor is also at the plant during normal business hours.
LDH conducted sanitary surveys of the system in 2021 and 2023 and identified a few deficiencies, which were resolved after the surveys.
Section II INSPECTION ACTIVITY SUMMARY
The EPA Inspection Team conducted field activities and interviews to observe the PWS operations and understand the WTP process and condition of the system assets. The LDH sanitary survey reports were provided prior to the inspection to help the inspection team gain a general understanding of the PWS and its compliance. The information in this report is based on document reviews, conversations with the system representatives and LDH, and observations made during the inspection. The documents provided for review by the PWS before and during the inspection included:

Previous Sanitary Surveys
The EPA Inspection Team reviewed the most recent sanitary survey conducted by LDH, which took place on November 14, 2023. This survey identified three significant deficiencies. The deficiencies from 2023 sanitary survey are listed in Table 1 below.

Significant Deficiency
The water system did not provide documentation of a complete list of customers required to install backflow prevention devices and a record of annual testing for each device.
Summary of Current Status
The cross-connection control program with the backflow prevention test letters was observed.

At the Ruffin Drive booster station, a moderate amount of erosion had occurred on the southeast section of the site underneath the fencing, compromising the functionality of the fencing.
The fencing concerns were discussed during the inspection with LDH, as the system representatives plan to address the deficiency.
11.14.2023 At the Thomas Street booster station, one of the pumps had a small leak from the pump packing.
The leak was not observed at the time of the inspection.
FACILITY/SYSTEM WALKTHROUGH
Monroe Water Treatment Plant
The walkthrough of the WTP began at the filters. There were six filters, all of which were operational. Four of the filters were old and had been rehabbed within the last six years. The rehab of the filters included the addition of new media (anthracite and sand) and air scour. Core sampling is done to inspect filter media, and the filters are cleaned every few years. Two filters were new. The system representatives noted that the raw turbidity from the source water was 1.5 NTU, increasing to 3 NTU during heavy rain. The highest turbidity observed was 5 NTU. It was mentioned that the Bayou was the main raw water supply, while the river was only used during drought season when the Bayou levels were low. The filters operated on automatic backwashing, dictated by the run time (every 72 hours) and turbidity. The backwash process was visually monitored by an operator; therefore 80% of the process is automated. The operator hoses down the filters to remove the algae. The backwash water goes to the sewer and out to the river.
Next, the aerators were observed. There were nine aerators, all in operation. The purpose of the aerators was to remove organic gas for taste and odor control. Chloramine, aluminum sulfate, potassium permanganate, and activated carbon are all fed at the aerators. Additionally, bands were added to the aerators to reduce splashing. The system representatives mentioned that there was a golf course next to the WTP which required screens (Photo 1) to be put up to prevent golf balls from impacting the equipment.

Following the aerators, the potassium permanganate room was observed. There were four chemical feed pumps: three were used for the mixing channel, and one was a spare. An eyewash station was observed. The bulk caustic tanks were observed outside in secondary containment.
Next, the alum room was observed. There were three aluminum sulfate fiberglass tanks. The dosage of the alum is dictated by the flow and is approximately 10-11 ppm. There were three chemical pumps pumping to the static mixers and one backup pump. A zinc orthophosphate tank, which is also used for corrosion control, was observed. There are two chemical pumps that pump to the high service pumps at the end of the process for zinc orthophosphate addition.
The ammonia room was observed with five ammoniator machines. They were labeled with the injection sites going to high service pumps and train. An eyewash station was also observed.
Next, the chlorine room was observed. There were 24 one-ton chlorine gas cylinder tanks. Chlorine analyzers and vents were observed. According to the system representatives, deliveries of the chlorine gas are made approximately every 2-3 months. Outside, a bulk activated carbon tank, and two ammonia tanks were observed.
Next, the walking beam flocculators were observed, following the sedimentation basin. For the sludge, there is an underwater rake system. The sludge from the basin goes to the river through a permitted outfall. There was discussion of a possible sludge treatment system with a sludge lagoon in the future. The system representatives stated that there is a preventative maintenance (PM) program. The basins are taken offline every three months for washing and clearing, with one basin cleaned at a time.
Following the sedimentation basin, the lab was observed. There are four lab staff: two work in-house, one is on rotation, and one conducts field sampling. Grab samples are taken from each process every hour including measurements for chlorine, alkalinity, pH, odor, hardness, color, iron, and manganese. All of the equipment is calibrated. The raw turbidity was observed to be 1.49 NTU on the turbidimeter. The system representative mentioned that the target chlorine residual in the distribution system was 2-2.3 ppm in the summer and 1.5-2.3 ppm in the winter. The pH target was 8-8.5. Jar testing was conducted for making changed in coagulation and optimizing the process. The engineer came in to perform changes in injection points, re-rating of the current plant, and conducted a tracer study.
Next, the four high service pumps were observed. Two pumps are used at a time. The pump with the highest rating pumps about 12 MGD. The largest pump is usually utilized during a high usage event such as a fire.
There were two ground storage tanks onsite at the WTP. The 3.1 MG tank was inspected in 2023, and the 3.3 MG tank was inspected in 2024. The tanks are connected and operate in parallel. They are used for contact time and are baffled. The tanks combine to one overflow which is flapped. Chlorine is injected in the 30-inch transfer line.
System representative stated that the valve exercising program started a month prior to the inspection and was in progress. Every valve in the city was mapped and is included in the city's Geological Information System (GIS), including the hydrants.

The assets observed below were all located offsite.
Chauvin Tank Pump Station
The Chauvin Tank has a capacity of one MG and was built in 2018 for expansion. The overflow of the tank was observed to have a flap that flows out to the bayou. The tank was inspected in 2023. The total chlorine reading was observed to be 1.85 mg/L on the chlorine analyzer in the pump station. The station has its own generator.
Ruffin Tank Booster Station
At the Ruffin tank booster station, two pumps were observed. There was boosting of chlorine and ammonia at the station. The reading in the chlorine analyzer was 2.36 ppm. The turbidity reading was 0.223 NTU, and the pH was 7.21. Chlorine alarms, detectors, crash bar door, and vent fan were observed. The pump station has its own generator.
The Chauvin tank has a capacity of 3.1 MG. Vegetation was observed next to the fencing, with a tree above the fencing. The system representative discussed that the tank was inspected in 2022, during which thin areas observed on the tank. About 2-3 inches of sediment was removed at the time of the tank inspection. The tank is scheduled to be rehabilitated.
Thomas Tank Booster Station
The Thomas tank is a 1.5 MG tank that was built in 1950. The overflow was screened and discharged into the storm drain. According to the system representatives, the tank was last inspected three years ago. It was observed that the pH at the station was 7.86, the turbidity was 0.151 NTU, and the chlorine residual was 2.18 ppm. A room containing liquid aluminum sulfate was observed, as well as a chlorine room with a chlorine detector, ventilation, chained cylinder gas tanks, and an entry door with a knob.
Selman Pump Station
Next, the Selman pump station was observed with three pumps. The pH was observed to be 6.99, the turbidity was 0.219 NTU, and the chlorine residual was 1.85 ppm on the chlorine analyzer. According to the system representatives, the pH was lower at this station due to adjustments to the chlorine feed. In the chlorine room, it was observed that cylinder gas tanks were chained, the chlorine detector was visible.
Next, the Selman tank was observed, which is a 4 MG concrete tank. The overflow was flapped and screened.
Powell Elevated Storage Tank
The Powell tank was observed. It was 135 feet tall, with a capacity of 500,000 gallons, and was built in 1953. It was observed that the turbidity was 0.121 NTU, the pH was 8.35, and the chlorine residual was 1.85 ppm.

Grammont Tower
Next, the Grammont tower was observed. Its capacity is 500,000 gallons, and its height is 115 feet. The overflow was flapped and screened. The chlorine residual was observed to be 1.87 ppm, the pH was 8.51, and the turbidity was 0.107 NTU.
Bayou Intake
Next, we observed the bayou intake, which had three pumps. Pump 1 had a capacity of 7 MGD, Pump 2 had a capacity of 12 MGD, and Pump 3 had a capacity of 7 MGD. System representatives mentioned that raw water testing was completed everyday, including measurements for turbidity, alkalinity, and pH. It was also discussed that there were bar screens at the intake. If leaves get stuck on the screen, the pumps are shut down to remove the debris. The indicator for debris being stuck on the screen is that the pumps usually struggle to work. The intake was at a depth of 65 feet, and there were cameras installed.
RRA and ERP Document Review
The EPA Inspection Team reviewed the System's Section 1433 RRA and ERP ,as well as the system's cybersecurity practices.
Section III OBSERVATIONS/AREAS OF CONCERN
I observed the following areas or concern and/or the following information was provided, stated on May 7, 2025.
AWWA Manual: M27 External Corrosion Control for Infrastructure Sustainability, Chapter 1 states,
''The health of water consumers may be threatened whenever extensive corrosion breaches the sanitary
integrity of the water system. The ever-present danger of backflow of contaminated liquid into the drinking water system is further increased when water pressure is interrupted to facilitate repairs on
corroded wells, pumps, treatment equipment, pipes, valves, and services."
Observation 1. There was visible corrosion on the aerators. (Appendix 2, Photo No. 3).
Observation 2. There was debris visible on plate settlers. (Appendix 2, Photo No. 19).
Observation 3. Pump 3 was leaking at the Bayou intake (Appendix 2, Photo No. 51).
The following observations pertain to the SDWA Section 1433(a), Risk and Resilience Assessment:
SDWA Section 1433(a), 42 U.S.C. 300i-2, Community Water System Risk and Resilience requires a CWS serving more than 3,300 persons to conduct an RRA of its system, including an assessment of:
a. the risk to the system from malevolent acts and natural hazards; b. the resilience of the pipes and constructed conveyances, physical barriers, source water, water collection and intake, pretreatment, treatment, storage and distribution facilities, electronic, computer, or other automated systems (including the security of such systems) which are utilized by the system;

c. the monitoring practices of the system; d. the financial infrastructure of the system; e. the use, storage, or handling of various chemicals by the system; and f. the operation and maintenance of the system.
Observation 4. On May 6, 2025, the EPA inspection team completed an inspection of Respondent's System under SDWA Section 1445, 42 U.S.C. 300j-4. The EPA inspection team requested
to review Respondent's RRA and observed the following:
a. Document was available at the time of the inspection. b. The RRA was dated to show that it was completed before the date of the certification of completion. c. The RRA assessed all required elements of Section 1433(a). d. The RRA included a complete assessment of the required elements.
The following observations pertain to the SDWA Section 1433(b), Emergency Response Plan:
SDWA Section 1433(b), 42 U.S.C. 300i-2, Community Water System Risk and Resilience requires a CWS serving more than 3,300 persons to conduct an ERP of its system, including an assessment of:
a. strategies and resources to improve the resilience of the system, including the physical security and cybersecurity of the system; b. plans and procedures that can be implemented, and identification of equipment that can be utilized, in the event of a malevolent act or natural hazard that threatens the ability of the CWS to deliver safe drinking water; c. actions, procedures, and equipment which can obviate or significantly lessen the impact of a malevolent act or natural hazard on the public health and the safety and supply of drinking water provided to communities and individuals, including the development of alternative source water options, relocation of water intakes, and construction of flood protection barriers; and d. strategies that can be used to aid in the detection of malevolent acts or natural hazards that threaten the security or resilience of the system.
Observation 5. During the inspection, EPA inspectors also reviewed Respondent's ERP and observed the following:
a. Document was available at the time of the inspection. b. The ERP was dated to show that it was completed before the date of the certification of completion. c. The ERP completed all required elements of Section 1433(b). d. The RRA did not include a complete assessment of the required elements.
The cybersecurity portion of the inspection was discussed with the system representatives where the list of cybersecurity questions for community water systems was provided to the water system prior to the inspection.

Section IV CLOSING CONFERENCE On May 7, 2025, the EPA inspection team conducted a closing conference at the Monroe WTP at approximately 5:00 PM. During the conference, the inspection team reviewed observations and areas of concern with the system representatives.
