The injury mas the result of using a "cheater bar" in securing a chain binder around cells for shipment. Upon reuieming our records me haue discouered that this is the second such incident in our block. UJe, at Dom, consider this unacceptable performance. In our inuestigation me feel that the root cause of this incident is the use of the regular chain binder rather than a screm type binder. UJe mould like to request that in future shipments you consider the use of the screm type. Please do not take this to infer that your performance has been poor or me consider you to be operating unsafe in any may. Mr. Wilcox has recovered and all is OK. I have talked with him about the accident and he states that he feels the accident could have been prevented if he had taken more time and had been paying more attention. The drivers chain and bind every day and some just-let up on their safety percautions. Safety is very important to Inway, ALL drivers have been instructed on safety and since the accident on 2-14-91 they have been reminded once again. If the drivers will work with caution and in full attention of what they are doing, I believe this type of accident can be prevented in the future. Our working relationship with Dow Chemical for the past 12 years has been great and it is very important that this continues.
