It was announced that the previous record of days without a lost workday injury was broken yesterday. Safety Performance at the Texas City plant is excellent so far this year. A copy of the SHAC Agenda/Report is attached. Bob Hammann presented the SHAC bulletin that was issued regarding nitrogen backflow preventers (copy attached). Plans for the Quarterly Inspection were finalized, and the Group Categories were presented. OLD BUSINESS None. Quarterly Safety and Housekeeping Inspection - Reminder, Scheduled for last two weeks in June - Service areas and laboratories eligible for awards - New laboratory check list developed by the Tech Center Safety Committee Sample Bottle Disposal - Surveying units for methods of disposing of laboratory sample bottles Drug and Alcohol Abuse Program - Bill Latimer and Gene Wofford met with Shell personnel. - Can develop an educational program with outside help but need a means to assist personnel in need after educational program is implemented Hose Tape - Hopefully the problem of fading hose tape will be resolved with receipt of new tape, which is now in the plant. Holeman Duties - Procedure written. Need PMSB approval. SC 001171 LAM002371 Nitrogen Backflow Prevention - LP&S (Bob Hammann) reported that inspections were not being conducted according to P-Bulletin 0126. SHAC will issue an alert. Bulletin requires weekly inspection by owning depart ment and quarterly inspection by instrument department. Unit Laboratory Inspection - TCSC will inspect any lab on request to insure compliance with lab standards. /jP SC 001172 LAM002372 RESPONSIBILITIES OF ATTENDANT (HOLEMAN) FOR PERSONNEL ENTERING CONFINED SPACE One person shall be stationed outside of the confined space when internal work is being performed. The attendant (holeman) will: Make certain proper permits have been issued. 0 Make certain specified safety equipment is available and used. Maintain continual communication with the individual(s) in the enclosure. (In some cases, visual contact may not be possible. In such cases audio contact must be frequent.) Maintain appropriate communication system to summon emergency assistance. 0 Know how to use rescue equipment specified for the job. Be alert for changing conditions. 0 Take emergency actions short of entering the confined space until emergency assistance arrives. There have been installations found in the past few weeks where nitrogen backflow preventers do not comply with the plant standard (found in P-0126), a copy of which is attached. Please check your units, especially the peripheral areas (i.e., tank farms) to assure that your nitrogen backflow preventer stations comply with this plant standard. The owner is responsible for making a weekly inspection to ensure that the bypass valve is in the closed position with seals or locks intact.
