It is believed that no further cases of angiosarcoma trill be identified from records relating to deceased workers from the Louisville, Avon Lake, or Niagara Falls plants, or from other BFG plants. 2. Proportionate mortality tables suggest that the Louisville plant is unique among the BFG plants, if not In the industry. Although the focus to date has been on angiosarcoma, it appears that there is an excess of other neoplasms. Proportionate mortality figures for Louisville are in excess of the figures for U. 5. males over 15 years of age (1965) for neoplasms of the digestive organs and peritoneum, liver, respiratory system, brain, for lymphosarcoma, and to a marginal extent for the buccal cavity and pharynx. * A comparison of all Louisville workers with all Avon Lake and Niagara Falls workers shows that the excess of neoplasms seen in Louisville Is not so marked in the other two plants. Suspicious proportions of death due to malignant neoplasms of the liver, respiratory system, and genital organs are seen in the Avon Lake and Niagara Falls plants. The numbers of cases are very small. The trends described for Louisville hold true whether the smaller MCAR group, or the larger BFGR report is considered. The Louisville BFG plane has a record of many years of operation during which there were estimated high exposures of chemical helpersv chemical operators, and others. This means that workers now living' may be Identified at death as eases of angiosarcoma. It may also happen that there will continue to be an excess of ocher neoplasms from this plant. It seems unlikely that newer plants will present the same problem. There Is evidence that the frequency of malignant neoplasms In Louisville Is dose responsive, both In terms of length of exposure and '-rree or Intensity of exposure. The subjective nature of the exposure --used, plus job rotation practices, plus evolution of plant hygienic practices makes a firmer conclusion Impossible. Both the degree of exposure, and.the duration of exposure should be considered Important factors. Data for other plants are not available in quantities suf ficient for this analysis. . 4. In the data from the BFG Louisville plant, the proportions of deaths due to neoplasms of the digestive organs and peritoneum, liver, brain, and to lymphosarcoma exceed the proportions for all vinyl chloride workers in the MCAR. Louisville deaths contribute 37.5 percent of the MCAR death data. All BFG plants combined contributed 48 percent of the MCAR deaths. THE MEASUREMENT OF EXPOSURE The job history forms (Attachment C) were, originally designed in the expectation that vinyl chloride exposure on each job could be quantified in parts pet million. This proved generally to be impossible. However, industrial hygiene and safety personnel in each plant were able to identity certain jobs and locations as involving the highest exposures in the plant, and to classify other exposures as medium or low relative to the "high" represented by the jobs with the greatest exposure. Therefore, the attempt to obtain objective exposure data was abandoned, and each exposed job in the worker's history was scored 1, 2 or 3 to indicate low, medium or high estimated exposure. This gross classification has two major failings, as a result of the subjective nature of the estimates. The first is that the scores represent estimated relative exposure within a given plant. It is there fore possible thAtj in objective terms, a "high" score in one plant corresponds to a "medium" or even "low" score in another. The second is that exposures in most plants have tended to decrease over time, so that a worker with long service may have had jobs in the remote past which Involved "low" exposure relative to other jobs at that time, hut which might be "high" in comparison with current exposures In the same job. This subjective classification is therefore of questionable validity in 20076036 characterizing ch exposure of a given worker* For epidemiological purposes* however, chose who have high scores can reasonably be expected* on Che average* to have had the greatest exposure, while those with low scores will have had the least* even though the true exposure in each group may vary considerabley from person to person. The estimated exposure history of each worker was summarized by calculating an Exposure Index (El) This was done by multiplying the number of months on each Job by the exposure score* totalling these overall exposed Jobs* and dividing by the total number of months of exposure. James Leonard Morris was employed for two and one half years as a chemical helper, terminating on September 11, 1952. The death cer tificate indicates that Morris died of glioblastoma multiforme on Kay 7a 1970 at age 43 Glioblastoma mulelforme Is a cerebral or brain tumor composed of a variety of cellular types. This Information would remove one death from the unclassified category, and add one to malignant neoplasms of the brain. Increasing the proportion of mortality from that cause from 2.7 percent to 3.4 percent. (Table 14) James R. Shrewsbury was employed as a chemical technical worker at the Louisville plant for 13 years prior to his termination in 1958. Shrewsbury died January 8, 1967 at age 43. The cause of death was cancer of the lung, metastasized to soft tissues and bone, and with possible cerebral metastasis. This Information removes one death from the unclassified category, and adds one to malignant neoplasms of the respiratory system. Increasing deaths In that category from 4.8 to 5.4 percent. (Table 14) The two deaths Increase the frequency of death due to all neoplasms from Louisville to almost 27 percent (Table 14) , and Improves the follow-back for Louisville*to greater than 97 percent.
