PLANT MANAGER'S COMMENTS The Plant Manager re-emphasized that the first quarter is histori cally a bad time for injuries. He called for a redoubling of our efforts to prevent accidents and reminded that we have already had a bad trend of incidents as indicated by the list of incidents passed out in the Expanded Staff Meeting in January. 3. OLD BUSINESS a. Plant-Wide System Survey -- Hernandez Previous plant-wide alarm system surveys have revealed problems in several areas. These will be highlighted in the next test of the Plant Emergency Procedure. b. Cyanide Area Alarms -- Ryan The acrylonitrile plant (AN-5) and the HCN Complex are instal ling new HCN alarms at the corners of their battery limits. The purpose of these alarms is to keep additional people out of the operating area until they are definitely needed. Per sons in the area not required to deal with the emergency will be asked to leave the area. In the event the spill or emergency should impact on areas outside of the battery limits, the plant alarm system would be employed. A plant-wide bulletin describing how the alarms work and what is to be expected of people will be publicized widely before the alarms are finally put into use. c. Respiratory Protection Procedure Bulletins -- Ryan/Glass The Respiratory Protection Procedure has been approved by the Plant Manager's Safety Board and is ready for distribution. The training program for use throughout the plant has also been approved and copies of it are ready for distribution as well. These programs will be distributed through the Plant Manager's Expanded Staff. (This has now been done.) d. 1978 S&PP Survey Recommendations--Action Plans The action plans on the 1978 S&PP Survey were scheduled to be turned in by March 1. At the next meeting of the Board, we will distribute the status of these recommendations. e. Control on Number and Use of Bicycles Inside The Plant -- Hernandez The Safety Advisory Committee has instructed the Safety And Housekeeping Action Committee to make a comprehensive survey on the use and control of bicycles inside the plant. A survey questionnaire has been developed and is being reviewed prior to its circulation within the plant. The Office Safety Committee will assist in distribution of these surveys and in a summary of these results. SC 002756 STG 2402076 LAM003963 PMSB MINUTES - 2/21, 1 Page 3 f Duplicate Emergency Radio System -- McCullough The radio system for the Emergency Procedure Room has been approved and the equipment is on order. McCullough will re port the status at the next PMSB Meeting. g. Glove Problems -- Glass The use of non-slip work gloves in handling bags of polyethylene pellets and other materials creates a possible chemical exposure hazard because these gloves offer no protection against exposure to chemicals if they were to be used anywhere else in the plant. However, these non-slip work gloves are useful in certain main tenance operations involving the use of sharp-edged objects such as tower trays, sheet metal, tray clips, and other special items. To reduce the possibility of these gloves creating ex posure problems. Department 35 plans to control the use of their gloves and stamp them "35 Packout" and limit their use to this application. Where these gloves are used in maintenance opera tions, the Maintenance Superintendents are responsible for con trolling their use to the specific applications where this type of hand protection is needed. The Safety Advisory Committee review of the Glove Control Problem stated that they felt that non-slip gloves can be controlled in each line organization, and they offered several policy recommendations: 1) All non-standard gloves should be justified prior to purchase. 2) Gloves approved should be procured for the individual department and not set up as stores inventory. 3) Special gloves should be marked "NOT FOR CHEMICAL PROTECTION". 4) Increased emphasis should be given to the proper use of gloves. 5) Should issue a training alert on the proper utility of gloves. 6) Special training should be included in safety meetings on the use of gloves of all kinds used in the plant. Plant Engineering (McCullough) will be responsible for developing a plant-wide safety meeting about this subject. Outside the Plant Engineering Department, this program will be distributed and implemented through SHAC. 4. REPORT OF THE SAFETY ADVISORY COMMITTEE (SAC)-- METTEN a. Use and Control of Hoses The SHAC Committee provided the SAC a rough draft of a proposed procedure concerning the use and control of hoses. SAC provided three comments to review prior to re-submitting the third draft of this Use and Control of Hoses Bulletin. sTG 2402077 002757 LAM003964 ?MSB MINUTES - 2/21/^ Page 4 b. Bicycle Routes in Operating Areas & Total Plant-wide Bicycle Policy The committee passed on for SHAC committee review several points for consideration in developing a proposal for a Bicycle Procedure Bulletin. c. 1978 S&PP Survey Response Responsibility SAC is currently reviewing eyewash stations, wind breaks, and foot protection programs with progress to be reported in the March PMSB meeting. d. Tech-Facts No. 31, Control of Water Treatment Processes The Utilities Department has assumed the responsibility to re spond to Tech-Facts No. 31. SAC plans no further action on this item. e. Idle Pipe Lines There is an existing Procedure Bulleting (P-0409) covering responsbility for Idle Pipe Lines. The SAC stated that this pro cedure was apparently not adequate to insure the proper job of removing or caring for these lines especially those in the pipe racks out of battery limits. SAC will review the bulletin to see what changes need to be made to improve the performance in this area. 5. REPORT OF SAFETY AND HOUSEKEEPING ACTION COMMITTEE (SHAC)-- HERNANDEZ a. Fire & Entry Permit Procedure Procedure is still being worked on at the present time and ve expect to report progress at the next meeting. b. Procedure for Using Hoses within the Plant The new hose control procedure is being proposed and is being circulated for comments. It is expected to be available for review prior to the next meeting. c. Quarterly Housekeeping Guest Inspections An inspection schedule has been developed for the Spring Quarterly Housekeeping Guest Inspections for the week of March 12. d. Recommendations of 1978 Annual Safety and Housekeeping Inspection Many comments on the recommendations have already been received. We will review the status at the next meeting of the board. Bulletin Board Notice on 12-Volt Lighting Inside Tanks and Use of GFI's This item has been completed and the bulletins have been posted on the bulletin boards. f. Plant-Wide Safety Direction & Philosophy The thrust for the coming year has been prepared and is ready for the Plant Manager's review. Hopefully this will be ready by the next time of the PMSB meeting. 6. CONTRACTOR SAFETY -- MCCULLOUGH Mike reported that there had been a general improvement in Contractor safety performance. He plans to report trends again next month. 8. OTHER SIGNIFICANT ITEMS 1) Absorber Overhead Separator Failure in AN-5 This item has received extensive review and modifications were made in the field while still operating. Extensive plans for revision have been set for the next shutdown. 2) Railroad Track Problem Mike McCullough reviewed the status of the Railroad Repair Survey and promised a five-year program with a copy to each PMSB member within the next month. Mike displayed a survey map on the weights of the rails and the location of substandard switches. It indi cates that the plan is a major one and some areas are so critical that some moves must be made immediately to correct the situation.
