COMMENTS ON STATISTICS The February safety performance resulted in a total of eighteen (18) injuries compared to twenty-six (26) in February of 1978. Our year-to-date injuries totaling thirty-nine (39) reflects an improvement over the same period in 1978 with fifty-three (53) injuries. In February we experience four (4) recordable injuries, one of which resulted in a lost workday case. The four (4) incidences were: one (1) chemical con tact, one (1) temperature extreme, and two (2) striking against. While our total number of injuries are down, the serious type (recordable injuries) reflect no improvement over the same period for the past two years. 3. OLD BUSINESS a. Plant-Wide Alarm System Survey -- Hernandez The concern about alarms is that they cannot be heard in certain areas in the plant. A plant-wide survey will be made in conjunction with the next Plant Emergency Procedure Test. b. Cyanide Area Alarms -- Ryan The cyanide area alarms are being established for the purpose of notifying persons in the operating areas that an emergency exists within the unit. Persons in the unit should take appropriate protective and corrective action. Persons outside of the operating unit should be warned by the alarms and the lights not to enter the operating area until these alarms are cut off. Testing of the new HCN alarms is in progress; however, a plant-wide publicity program about the lights, alarms, and appropriate actions need to be carried out prior to their being placed into service. It is estimated that it will be sometime in April before these alarms will be ready. c. 1978 S&PP Survey Recommendations -- Action Plan The Action Plans for the 1978 recommendations are essentially complete with four out of twenty-two remaining unreported. As soon as all reports are in, a status report will be issued. d. Control on Number and Use of Bicycles Inside The Plant -- Hernandez A survey on bicycle use is being conducted by the Office Safety Committee. The survey questionnaires have gone out to people throughout the plant. They will summarize the results and make a report to SHAC/SAC. e. Duplicate Emergency Radio System -- McCullough The radio system to be used .as back-up for the existing radios in the Emergency Procedure Room has been set back to the plant for additional justification. Glove Problems -- McCullough A survey and recommendations for actions on the glove problem is in progress. A program is being developed. Target time for completion is second quarter in 1979. Plant Engineering (Leonard Schwab) is responsbile for developing a safety meeting on this subject to be presented to crafts and contractors. Out side the Plant Engineering Department, the program will be communicated and implemented through SHAC. 4. REPORT OF THE SAFETY ADVISORY COMMITTEE (SAC) -- METTEN a. Idle Pipe Line in Ex-battery Limits SAC has reviewed P-0409 "Idle Pipelines" and feel that while most of the responsibilities are identified in the proper function, the procedure is not."action oriented" and assumes this job area is being done on a routine basis (not the case). SAC recommends that SHAC revise existing P-0409 bulletins with the following emphasis: A) Thrust should be stronger toward the "Owning Department" as to making the procedure work. It is not strong enough as now written. B) Procedure should deal with "making it happen". System should insure the mechanics for: 1) Periodic status reviews (plantwide) 2) Cost of maintaining versus potential use review. 3) AR preparation 4) Budgeting 5) Above items in the proper sequence C) Should deal with the idle plant concerns when a product has been dismantled and the idle pipe left in the racks (insure communications, require timeframe for ultimate use, etc.) While SHAC reviews the bulletin. Plant Engineering has taken the leader ship role to collect a current base point of the problem. Each zone is developing (with assistance from operating personnel) a list of all lines requiring removal along with the removal/repair costs. This data is to be held for AR development by Plant Engineering Services and for 1980 budgeting. Target date for this preliminary survey is April 1st. b. Ham Radio Station AR for purchase of equipment has been circulated and approved. Equip ment will be put on order the week of March 19th with delivery in early April. Doug Metten is writing an article for the Plant News to generate interest in the existing radio club with a planned recognition article scheduled by June. Policy on Obtaining Medical Information As recommended in the Investigation of the Owens-Corning Insulator acci dent in P.A., SAC reviewed the procedure in existence to collect medical information. SAC recommended changes to P-0113 (Contractor Accident or Incident Reproting, Investigation and Followup Procedure) to LP&S, re visions have been made, and SAC is currently reviewing the revised pro cedure prior to reveiw by PMSB. 5. REPORT OF SAFETY AND HOUSEKEEPING ACTION COMMITTEE (SHAC) -- HERNANDEZ a. Fire & Entry Permit Procedure The SHAC Committee has reviewed with the authors of the Fire & Entry Permit Procedure the original intent regarding the requirement that such items as`blinds .and lockouts must be visually checked daily before issuing fire and entry permits on continuing shutdown jobs. This discussion de veloped a number of areas of concern and it will remain an agenda item for next meeting. b. Procedure for Using Hoses within the Plant SAC is interested in reviewing a summary of the field comments prior to final issuance. Since SAC has a fundamental concern of "over proceduralizing", at Texas City, we would like the opportunity to make an impact assessment of the field comments. c. Quarterly Housekeeping Guest Inspection These inspections are in progress and are expected to be completed by the middle of next week. d. Recommendations of 1978 Annual Safety & Housekeeping Inspection The status is given in the table attached. e. Plant-Wide Safety Direction & Philosophy The SHAC Committee has prepared a recommendation to be reviewed with the new Plant Manager at his earliest convenience so that the new plant-wide Safety and Direction Philosophy can be implemented through out the plant. A special session of the PMSB may be called to review these plans before they are put into effect. 6.
