Section I - INTRODUCTION
Village of Fort Sumner/Fort Sumner Wastewater Treatment Plant Inspection Date 09/28/2022 Permit No. NM0023477
PURPOSE OF THE INSPECTION
EPA Region 6 inspector David Esparza, PE, arrived at the Village of Fort Sumner Wastewater Treatment Plant (WWTP) (Fort Sumner) at approximately 12:00 PM on September 28, 2022, for an unannounced inspection. I met with Mr. Mark Micelli; Water/Wastewater Supervisor, Mr. Gabe Garza, Water Operator and Mr. Jonathon Geesey, intern. I presented my credentials to Mr. Mark Micelli and informed him that this was an EPA inspection to determine Fort Sumner's Wastewater Treatment Plants (WWTP's) compliance under the Clean Water Act (CWA). This CEI was conducted under the authority of the National Pollutant Discharge Elimination System (NPDES) permit program, in accordance with the CWA. The generation of this report is based on information supplied by Fort Sumner representatives, observations made by the United States Environmental Protection Agency (US EPA) inspector, and records and reports maintained by the permittee (Fort Sumner), and the US EPA. Before leaving the facility, an exit briefing was held with the Mr. Marl Micelli, and the above-mentioned staff, to explain areas of concern noted at the time of the inspection.
FACILITY DESCRIPTION
The WWTP a minor discharger with a design flow of 0.21 million gallons per day (MGD), is located at 179 Sewer Plant Road or approximately 2.0-miles southeast of the Fort Sumner Village Hall, De Baca County, New Mexico (depicted in Aerial #1 below). The WWTP facility serves a population of approximately 900 residents. The WWTP is operated by staff within the Public Works Department and consists of 3 full-time equivalent (FTE) positions, on a Monday through Friday 8-hour per day basis. On-call staff is available on an as needed basis.

Treatment Scheme:
Fort Sumner maintains/operates four (4) lift stations (Identified in Table 1 below), combined with gravity flow that transports wastewater to the WWTP. Raw sewage reaches the entrance works where a comminutor (a.k.a. grinders, macerators) is used to reduce the particle size of the wastewater solids and a backup bypass channel to a mechanical bar screen. Solids and grit caught in the bar screen are collected in a hopper and transported by contracted carrier to the De Baca County Landfill.
The WWTP headworks are comprised of an aerated grit chamber and a six-inch Parshall flume. The influent flow is then lifted by two (2) alternating submersible pumps to Sequencing Batch Reactors (SBR) basins.
Wastewater is processed though alternating cycles of first filling and mixing, then an aerobic, anaerobic, settling and finally a decant phase. Four (4) small blowers provide aeration to these two (2) units. An aerobic sludge digester is located between the two (2) SBR units. Decant water from the SBR basins enter an older model Schreiber flow equalization unit which ensures even flow to the disinfection system.
The WWTP utilizes a single bank of Ultraviolet (UV) lights for disinfection and bacterial control prior to discharging the metered treated effluent to Outfall 001 and into the Pecos River.
Sludge:
Sludge is wasted from the two (2) SBRs and sent to the aerobic digester. Wasted sludge from the digester is then placed in the sludge drying beds for a minimum period of 90-days. Decant waters from the digesters is returned to the SBR system for reprocessing. The sludge drying beds have under drains that return liquids to the headworks. Residual solids are also wasted from the Schreiber system directly to the sludge drying beds. The dried sludge is moved to an adjacent concrete pad, windrowed, and staged onsite.

Fort Sumner does not have any identified Industrial Users (IU) contributing to their WWTP.
Additionally, Fort Sumner has protocols and mechanisms in-place pertaining to Facility Process Equipment Summaries, Support Equipment Operation and Maintenance (O&M) Manuals, inclusive of all manuals for the O&M of the WWTP. However, Fort Sumner does not have a written Fats, Oils, and Grease (FOG) program, though a FOG discussion is described in a circa 1976 Village Ordinance (Appendix 2 Village of Fort Sumner Ordinance No. 199 Discussing Fats, Oils and Grease (FOG)).

I observed the following:
The WWTP has an electrical generator, which is exercised on a weekly basis. It was not apparent which electrical components the generator provides back-up/stand-by power to in the event of a power loss. Additionally, it is not clear if any of the lift stations have a back-up source of power (quick connect or portable) in the event of a power outage.
The bar screen is non-functional. The WWTP operators must manually scrape the grit from the bar screen, place in an adjacent concrete lined basin; thence scooped into a container for transport and disposal at the landfill.
The WWTP Supervisor indicated his belief was the Wier notch blades in the Parshall Flume were installed upside down, bent and currently are too expensive to replace.
The Fort Sumner WWTP Improvements (Project #20210378/ CWSRF # 100/CBDG # 20-C-RS-I-01G-04) 100% Construction Plans were completed in June 2022. This Plan Set together with the "Bid" document (Appendix 3 WWTP Improvements Bid Tab Document) identified multiple facility aspects in need of improvement and/or replacement. Additionally, the Bid document broke down the project to a "Lump Sum" bid or five (5) Additive/Deductive Alternate bids, however it was disclosed the Lump Sum bid exceeded available monies; thus, the Village was in the process of reducing the proposed Scope of Work (SOW) to address the most pressing WWTP issues.
Given the WWTP is in the process of implementing various improvements, it should be noted many of the piping runs or fittings exhibit extensive corrosion, and concrete surfaces exhibit degradation (spalling) (Appendix 4 Proposed Improvements Work Sequence).
Three (3) staged sludge piles were observed and awaiting transport and disposal. The Supervisory Control and Data Acquisition (SCADA) system has no alarm. There is currently no
dial out or audible alarms at the WWTP. Southeast corner perimeter fencing requires repair along with the removal of the extensive
vegetative overgrowth, which negates secure access and potential vermin attraction.
Section III - AREAS OF CONCERN
A review of the ICIS database for the period from January 1, 2020, to September 19, 2022 (Appendix 3) indicates NPDES permit excursions.
A review of the Integrated Compliance Information System (ICIS) indicates several NPDES permit single event violations (February 2022), Schedule Violation (January and August 2022), Biological Oxygen Demand (BOD5) (January, April and May 2020), Total Suspended Solids (TSS) (April, May and July 2020), Whole Effluent Toxicity (WET) (April and October 2020 and October 2021), Discharge Monitoring Report (DMR) Non-Receipt Violation (April, July, and October 2020); and Escherichia coli (E coli) (September 2020 and February 2022).
The following additional areas of concern (AOCs) were identified and/or discussed with the Village of Fort Sumner :
