W ELCOM E TO LIBERTY MINE This form is in compliance with federal mining regulations and is required to inform you o f the possible hazards you encounter at the mine. Since those potential hazards exist throughout the mine, you need to be aware of them from the moment yon arrive on our property until the time you leave. Take time to review this material thoroughly and if you have any questions please ask a mine official for clarification or additional information. We want your stay on our property to be a safe one. All visitors, delivery personnel* tru ck drivers, etc, must follow these rales while on mine property, 1. Upon ente ring mine property, report to the mine office. 2. Do not drive into the pit area unless you are gi ven permission to do so. 3. Do not. open your door or get out of your vehicle unless you are wearing a hard hat and metatarsal hoots. Up to 24 hours of safety training may be required by MSHA regulations if you get out of your vehicle on mine property and do any directing or work at all. 4. Obey all posted traffic signs and speed limits, 5. Yield to loaded trucks on mine property. 6. Follow a general right-handed traffic pattern and stay on haulage roads. 7. Report any accident and/or hazard to the office immediately, 8. If yon experience mechanical difficulty, raise your hood and a supervisor will assist yon. 9. Do not enter any area where welding is being done. 10. Be alert to the danger of such things as blind corners and mobile equipment backing up. 11. Before approaching any mobile equipment, stop a safe distance away and make sure you are seen by the operator o f the equipment before proceeding. 12. Do not drive into any area marked "Explosives" or "Blasting" , 13. Obey all blast warning signals as post!. 14. No smoking or open flames within 50? o f the shot area. 15. Safety harness and lanyard required when working from elevated areas where tall hazards exist, 16. Respirators and hearing protection required where applicable, 17. Eye protection required where hazards from foreign objects may cause injury to your eyes. 18. Reflection vest or clothes is required by contractors, I acknowledge I have read and understand the rules and agree to abide by these rules while on mine property.
