Personnel dosimeter monitoring testing conducted again on December 10, 1979 indicate you were exposed to a vinyl chloride concentration of 0.005 ppm measured over an 8.0-hour period. In accordance with OSHA Standard for VCM Exposure, Title 29, Chapter XVII, Part 1910.93(q) an exposure above 0.5 2Pm of VCM requires that I offer to you an opportunity for medical surveillance at the company's expense if you desire such action. Medical surveillance will consist of a blood testing procedure and a hands on physical examination by a doctor assigned by the company. Results of these examinations will be made available to you or your designee upon your written request and signature of release on the prescribed form.
