FUGITIVE EMISSION TAG - THIS IS A 1-1/2'' SQUARE STAINLESS STEEL TAG THAT IS ATTACHED TO ALL VALVES IN THE OPERATING AREAS. IT IS A MEANS OF MONITORING AND TRACKING VALVE CONDITION. IT SHOULD BE REMOVED FROM OLD EQUIPMENT AND ATTACHED TO REPLACEMENT EQUIPMENT SO CONTINUITY OF DATA CAN OCCUR. 11. INSTRUMENT ROUTING TAG - THIS TAG IS USED TO IDENTIFY ALL INSTRUMENT EQUIPMENT TO BE SENT FROM THE UNIT FOR REPAIR. THE FOREMAN SHOULD FILL OUT THE NECESSARY INFORMATION, SEAL WITH THE PEEL-BACK PLASTIC COVERING AND ENSURE IT IS ATTACHED TO THE CORRECT EQUIPMENT BEFORE REMOVAL FROM JOBSITE. 12. MULTI-COLORED BARRICADE FLAGGING - USED TO BARRICADE RESTRICTED ACCESS AREAS OR PIPING AND EQUIPMENT THAT COULD CAUSE A TRIPPING OR INJURY HAZARD. A PALE BLUE BARRICADE TAG WILL BE ATTACHED TO THE FLAGGING IDENTIFYING PURPOSE AND USER. 13. YELLOW "CHEMICAL HAZARD" TAPE - THIS TAPE SHOULD BE PUT UP WHENEVER AN UNKNOWN OR KNOWN CHEMICAL SPILL OCCURS. ANYONE IDENTIFYING OR SUSPECTING A CHEMICAL SPILL SHOULD BARRICADE OFF THE AREA WITH THIS TAPE UNTIL THE AREA IS SECURED. ONLY THOSE PERSONNEL SPECIFICALLY PERMITTED TO CROSS THIS BARRICADE MAY DO SO. AFTER THE SPILL HAS BEEN CLEANED UP, OPERATIONS SHOULD REMOVE THIS BARRIER. 14. YELLOW NYLON W/MAGENTA CENTER STRIPE - USED ONLY FOR BARRICADING AN X-RAY EXPOSURE AREA. THIS MUST BE INSTALLED BY RADIOGRAPHY TECHNICIANS BEFORE X-RAYING BEGINS. IT IS TO BE USED IN CONJUNCTION WITH X-RAY WARNING SIGNS AND AMBER FLASHING WARNING LIGHTS. TECHNICIANS MUST USE THIS BARRICADE TO BLOCK ANY AND ALL ENTRANCES INTO AN EXPOSURE AREA, (of particular importance is ladders, stairways, elevators) 15. YELLOW AND BLACK "ASBESTOS" HAZARD - USED IN CONJUNCTION WITH MULTI-COLORED FLAGGING TO PREVENT ENTRANCE INTO AN AREA WHERE AN ASBESTOS HAZARD EXISTS. THIS BARRICADE SHOULD BE LARGE ENOUGH TO COMPENSATE FOR WIND, ELEVATION ETC. IT WILL BE INSTALLED BY INSULATING PERSONNEL, AND ONLY REMOVED AFTER ASBESTOS WORK IS COMPLETED AND CLEANED UP. WHEN THIS SIGN IS USED, AN ADDITIONAL BARRICADE TAG IS UNNECESSARY. 18. RECTANGULAR STAINLESS STEEL TAG - USED TO IDENTIFY CONTROL AND RELIEF VALVES, EXCHANGER AND VESSEL TRAY PARTS AND THEIR RESPECTIVE LOCATIONS. THESE SHOULD BE INSTALLED PRE-T/A WHENEVER POSSIBLE. REQUIREMENTS FOR INITIAL ENTRY (Vessels, exchangers, pipe,) PPE requirements will be followed on all initial entries until otherwise established by Operations and/or Health and Safety. If operations establishes that piece of equipment is decontaminated, by taking a grab sample from an open bleeder or flange, the respirator requirements can be established according to the grab sample results. So, initial equipment opening will not necessarily requires breathing air^ 2. ASBESTOS REMOVAL All asbestos removal occurring during the turnaround will comply with DPMC Safety order S-125. 3. RESPIRATORY PROTECTION All personnel, subject to the use of respiratory equipment on the "A" DEPARTMENT turnaround, will be required to be fit tested and trained in the proper use of, all forms of respiratory protection used on this turnaround. All training will be as per DPMC Safety Order S-108 Guidelines, documented and subject to audit by Health and Safety at any time during the shutdown. Respiratory protection requirements set forth in this text are subject to change by Shell Safety and/or Shell Industrial Hygiene Department as conditions change, (i.e., improved ventilation) during or before the actual task. A. Welding - Burning and welding on exotic metals (i.e., stainless steel) in confined spaces will require the use of air supply respirators by the welder and welders' helper. Industrial Hygiene will monitor some of these tasks for future references. B. Air-Arc Gouging- Welders performing arc gouging are required to wear an air-supplied respirator. Half face air supplied respirators designed to fit under a welder's hood are available at the Respiratory Center. Welder helpers are also required to wear air supplied respirators.
