The audit was fairly lengthy and contained a wide range of items - both positive comments and constructive criticism. In oraer to structure the reply, I have sorted the audit items into categories based on their status. POSITIVE COMMENTS - NO ACTION REQUIRED DU-1 2. Signs identifying asbestos, hearing protection, goggle areas throughout unit - widely used - great'. 4. Scaffolding erected per procedures - inspection tags in place. 5. Fire extinguisher inspections current. 6. Process isolation tags and use in compliance per orocedures. 7. Housekeeping good. 9. Eauipment identification - excellent. 10. Protective orange valve stem caps used throughout the unit. i 15. Housekeeping on elevated structures is remarkable. 25. Permits and electrical lock-outs up to date. 27. Steam lance disconnected from header near E-7414. 26. General maintenance book -- Refinery needs updating. 29. Suggest relocating trash can: hinders access to wall mounted Scot Air Pak. 32. Debris on platform by tank S--333. DU-2 1. South flash heater has old insulation (asbestos?) exposed on small steam line - southeast corner at grade. Also at oil gun line -- south side at grade - saw other places with exposed insulation. Suggest unit wide inspection/repair. >> Distilling has not conducted a unit wide inspection of exposed insulation. We have encouraged all employees to write maintenance work tickets to repair exposed insulation as they find it and are currently addressing several areas. 3. Old scaffold boards positioned 8' above grade under south flasher heater near Burner #8 - north side. Should be removed. 4. Secure safety shower green/orange placard that is hanging loose - by Aloha Chem tank. 5. Tub filled full of water and soap under Alpha Chem tank. What is purpose? Should be labeled. >> This has been removed. DPMC-09554 LAM 003453 6. Hot'water dripping out of valve packing near BFUl pump P-5159. 8. Lots of steam and hot condensate blowing out of sewer hub at south crude heater snuffing steam valve station. 10. Smoking pad by heater south at control room without butt bucket. 11. In lab, you have an eye wash station and then a bottle of "Buffered Eye-Lert" on the other side of the lab. Is there a reason to use one or the other? >> Either the eye wash or the "Buffered Eye-Lert" may be used. 13. The safety shower on south end of structure is being used to secure a chain for a chain valve, 15. The fire extinguisher by the Turbo 68 oil storage unit below V-5118 nas not been inspected and the station for this extinguisher has no number (i.e., may want to see if this unit is on the month end check list). 16. A Complex issue, as in DU-2 and many other units, is the use of tap to leak check flanges. The issue is that the tap does not always get removed and in cold service this can result in excelerated corrosion under the tap. On DU-2 the accumulator deck (crude overhead) offers plenty of examples. >> DU--2 is not in cold service and corrosion is not perceived to be a problem in these areas. 16. Steam coming from sewers on south end of DU-2 has hydrocarbon smell. >> Several of the steam traps which trap into the sewer have been replaced to minimize nuisance odors. There is currently no condensate return system at DU-2 -- a capital project is being developed. 17. The concrete on the west side of the exchanger train is in need of some touchup (i.e., hole could be tripping hazards). 19. There is a trash can at that is labelled "oaDer hazardous waste can due it correctly labeled. the west end of the desalters only". We used goggies. >> Goggles are required when handling liquid samples; otherwise, safety glasses are required. Question concerning the priority of these statements. This causes phenol to fall to the ground. LOADING ARM PLATE HAS BEEN TEFLON LINED, WHICH SIGNIFICANTLY REDUCED THE FALLING OFF. WAS ALSO SUGGESTED THAT A DROP COVER BE INSTALLED ON THE EAST SIDE OF THE LOADING RACK. WILL PURSUE FURTHER IF ISSUE IS SEEN TO JUSTIFY. 2.
