We are involved in a cohort mortality study of a manufacturing facility in New York State and thus need to obtain death certificates to complete our work.
Our study seeks to determine whether there are any unusual frequencies for causes of death which are associated with the work place and specific chemicals in the work environment. The study is of a defined cohort using mortality follow-up and comparison with vital statistics rates to see if the death rates in the observed cohort are different from those expected. The attached protocol provides more information concerning this study.
The results will be used to advise workers of any increased risk, advise management of changes In the work place that would be beneficial to the health of the employees, and for publications in the scientific literature so that others will know of these results and be able to act accordingly.
In our Division of Epidemiology and Biostatistics, we routinely keep all information concerning individuals in locked file cabinets in our offices which are locked when not in use. Only those persons actually working on the project have access to these records and each such person has signed an agreement concerning confidentiality of all such records. At the end of our study, the material that you provide for
ft us will be shredded and disposed of. The anticipated date of completion of the analysis of this cohort is December 31, 1982.

which is currently owned
by Hooker Chemical and Plastics Company and was previously owned by Rubber Corporation
of America. Hooker Chemical is the funding source of our study.
The attached letter shows the approval of this project by the University Review Board for the Protection of Human Subjects. We shall not follow back or contact any patients, relatives, physicians, etc.
We assume your review of this material will result in approval of this project; at that time we shall send you a list of about 150 names of persons for whom we need death certificate information and $2.00 per uncertified copy. The employment history of these people will show previous employment at this facility or other places of employment.
