"Purple-peeper" flame out dectector replacement was passed on to the Instrument Department for determination of cost and timing. B. Plant Standard for Pump Shaft Guards - This item is being reviewed by SAC with particular attention focused on smooth rotating shafts having no coupling. SAC is making a survey of the extent of the exposure prior to reaching a decision. C. Emergency Communications System Study - A MA analysis of five alternative methods of providing back-up radio communications within the plant were presented to PMSB by the study team. The Plant Manager will review with SAC and make a decision. D. A P-bulletin on Superhazardous Liquids is being prepared and will be presented at the next meeting of PMSB. III. REPORT OF SAFETY AND HOUSEKEEPING ACTION COMMITTEE (SHAC) -- Stuart A. Stuart reported on plans of SHAC to go to Chocolate Bayou to report on results of multi-plant survey of safety programs the committee compiled as a preparation for the NEW PLANT WIDE SAFETY PROGRAM. Chocolate Bayou was one of the plants visited during the survey and asked for this feedback. B. A proposal for tagging ladders previously presented by Will Lane for PT has resulted in plans to issue new stick-on labels which display a ladder inspection check list and a place for displaying the name of the person making the inspection and the date of inspection. Revisions will be made by Riddick. D. A proposal was made for communicating the changes in timing and emphasis in the NEW PLANTWIDE SAFETY PROGRAM. The proposal was reviewed and plans for announcing these changes were to be developed and presented by SHAC to the Plant Manager. IV. The following incidents were reviewed by the Plant Manager's Safety Board: A. 11/15/77 B. 11/22/77 C. 12/24/77 D. 12/25/77 Catastrophic Failure of Turbine 972P2DT-6 -- Fuller Switch Condenser Fire - 53E6-13 -- Trafton Safety Incident - Dept. 15 (pipefitter was sprayed with ethylbenzene) -- McCullough Incident - Lacerated Middle Finger, Left Hand -- McCullough V. The status of the following items was reviewed by the PMSB: - Safety and Property Protection (S&PP) Survey Recommendations - Safety Check on Tanks in Atmospheric Service - Checking Controls on High Speed Rotating Equipment VI.
