This incident was reported to Dick, who was located at Leonard Eefineries at Alma observing the un loading of a-new''bar# ' fin# ::1iayim**d,:s abs#### from the office because of sickness the call was referred to the writer. The Michigan Tank & Steam Cleaning Co. of Detroit cleaned 8 - 15,000 gallon slop tanks on September 19 at the Cities Service Pipeline Terminal, Toledo, Competition apparently was notified of this intended entry since one of their representatives was present, inspected and approved the equipment, and then left. one of the two employees of the contractor handling t h e ^ I e S B ^ ^ ^ admitted being in the tanks about eight hours without any airline protective equipment. lie complained of headaches and a general ill feeling and contacted his family doctor, who is Dr. Leslie Joseph, on this ill feeling. The writer called to further determine information as to the family doctorTs "name and various telephone numbers and addresses which are as follows: The Michigan Tank & Steam Cleaning Co. 5718 G1inwood Detroit;, Michigan Telephone: Tyler 6-8177 Street Detroit, Michigan Telephone : Taylor 5-7435 Dr. Leslie Joseph (family doctor) 4337 Wetfort Street Detroit,Michigan Telephone : Taylor 6-4770 In the course of the conversation withfflB^H^Hfe, he mentioned that actually he had been feeling sick a b o u ^ ^ ^ i k before he went to Toledo to handle the work, and that the symptoms of which he complained (headaches, etc.) began to bother him about a week after the tank cleaning incident. This information was reported to Mr. Fleck of our Kew York Safety Department who referred the matter to Dr. Kehoe at Cincinnati. Just for the record, Dick Husta has the A & N with the contractor, Michigan Tank & Steam Cleaning Co., with the last contact made there in January 1955. The A & If contact on the Cities Service Pipeline Terminal at Toledo is the responsibility of L. M. Slack and was made last on October S, 1954, according to Central Regional office records. H. B.
