Comments on Statistics--J.A. Glass Safety performance results indicate a continuing rise in the number of total injuries. However, during the month of August there were no recordable injuries. This is the second month this year without recordable injuries. The first was the month of May. Departments with continuing rising trendlines include: Total Injury Rate/Total Recordable Rate-Operators, Maintenance, Dept. 13/14/15/16, Dept. 35, Dept. 44/45/46, Dept. 51, Shops Building, Boilermakers, Carpenters, Machinists, Truckers and Laborers. C. Contractor Safety--Ed Crow for C.M. McCullough The Capital Projects group had only one injury and no recordables during the month of August. Total plant contractor safety perfor mance showed 8 total injury cases and 0 recordables. Details of the statistical report for these two groups is attached to these minutes. George Danner reported that he had just recently made a quarterly housekeeping inspection and saw the best adherence to the use of per sonal protective equipment by the contract workers in the area inspected. He saw no exceptions to the proper use of hard hats, glasses and goggles. David Supak has joined the Capital Projects group as a Safety Engineer for Contractor Safety. D. CEP Project Safety--Bruce Rule for B.C. Lancaster Bruce reported that CED had experienced two recordables during the month of August bringing their year-to-date total to 20. II. REPORT OF THE SAFETY ADVISORY COMMITTEE--G.H. Daues A. Interlocks--A procedure obtained from Chocolate Bayou on by-passing and testing devices was discussed and it was agreed that with some modification would be suitable for Texas City. A copy of the revised bulletin was distributed for review by PMSB members and their staff. SAC requests that consents on this procedure be sent to the SAC chairman by October 5, 1981, so that a final version can be composed for review at the October SAC and PMSB meetings. Alarms for Safety Showers in Remote Areas--Bob Hammann requested that SAC establish a policy on location of alarms on remote safety showers. He suggested that the owning department control house be alarmed instead of, or in addition to, the closest control house. SAC requested that LP&S define the magnitude of the problem before a policy is formulated. SC 003325 LAM004339 PMSB Meeting Minutes - September 16, 1981 Page 3 Plant Telephones There are ten telephones in the Texas City plant which function for outgoing calls when the normal telephone system is ait of order. A rearrangement of certain phones is needed. The recommended rearrangement will be presented at the October PMSB. 1981-82 Plant Safety Program Status of this program emphasizing review of plant safety procedures was requested of SAC at the August PMSB. Current status is: 1. LP&S has issued an updated index of P-bulletins to the expanded staff. 2. C.M. McCullough has issued instructions to the Maintenance Department on how they are to implement this program. 3. Other General Superintendents are issuing similar instructions to their staffs. 4. SAC believes that some of these procedures need updating and is requesting that SHAC review the 35 procedures and determine which need revision and follow through to see that these are revised in a timely fashion. B. Safety P-Bulletins An updated list of Safety P-Bulletins has been issued for review on a plantwide basis. There were several errors noted in this list and there are some revisions being made. The second update is scheduled to be issued on September 18. C. "Tecfacts" An index to "Tecfacts" was distributed by LP&S asking PMSB to indi cate which of these they would like to have copied for their own use. Several persons have sent in their_orders.. These will be combined for a single order to be placed with S&PP on Friday, Septenfcer 18. III. REPORT OF THE PLANT SAFETY AND HOUSEKEEPING ACTION COMMITTEE (SHAC)--J.J. McClellan iL A. 1980 Safety and Housekeeping Inspection A memo is being issued by Arnold Glass giving the most recent update of statements made in response to the 1980 Safety and Housekeeping Inspection to be included in the booklet for the Annual MCI Inspection for 1981. This will be the last chance to cormient on the information before it is published. SC 003326 LAM004340 PMSB Meeting Minutes - Septenfcer 16, 1981 Page 4 B. 1981 Safety and Housekeeping Inspection--October 13, 14, and 15 A request has been made for the Plant Manager's Safety Board to be present for introductions and coffee on the fii^st morning that the group enters the plant for the start of the inspection. Members of the Board that are not directly involved in the presentations will leave after the introductions have been completed. Items of Interest to be Completed Prior to the Inspection 1. Signs in the area to be checked to be sure that they're adequate and readable. 2. The Ladder Inspection Program should be complete and up-to-date (a special memo to the Unit/Craft Safety Promotion Teams will be ' issued by LP&S as a reminder). C. Status of the Accelerated Vessel Inspection Program (SHAC) The status of the inspections were reviewed and exceptions duly noted. D. Status of S-T-A-R-T Safety Program 1. Publicity by LP&S--Bulletin board notices have been posted regarding the subject of the present S-T-A-R-T Program. The monitor reward program is under way and an increased knowledge of the purpose of the S-T-A-R-T Program has been noted. Continued publicity will emphasize the purpose of S-T-A-R-T which is to increase safety awareness and correct unsafe acts and conditions. 2. Current Slide Program--The third-quarter slides have been distri buted for use in plant safety programs. Many of these have already been held; others are planned and under way. Planning forjthe slides to be used in the Safety Awareness S-T-A-R-T Program will focus on correct use of procedures during the fourth quarter. E. Hourly Safety Committee Items Joe pointed out the problem we were having in getting good attendance at the Hourly Safety Committee meetings. Supervision is reminded of the importance of these meetings to have an effective safety program. Joe also noted that members of the Hourly Safety Corranittee had asked for an improvement in the safety program material.
