Sirmons Date August 14, 1978 Subject REPORT TO OCCUPATIONAL SAFETY AND HEALTH, OKLAHOMA CITY, OKLAHOMA I received a phone call from Mr. Jim Roberts of the Occupational Safety and Health Office, Oklahoma City, at about 11:30 A.M. today. Mr. Roberts was asking for information regarding the VCM release which occurred at our Plant yesterday. Mr. Roberts said the Tulsa Office had informed the Director of the local office of the report which I filed with that office yesterday (August 13), and that he knew no more than that a VCM release had occurred. I gave Mr. Roberts essentially the same report as was transmitted to the Tulsa office. At his request, I briefly described a double rupture disc, the safety device which failed. Mr. Roberts stated that he was gathering information at the request of the Director from the local office. He expressed thanks for our cooperation and said he would inform the Director. At approximately 12:15 P.M., Mr. Roberts called again and asked the number of equivalent gallons contained in the 50,000 lbs of VCM. I replied that the gallon amount would be roughly 6,944 gallons (50,000 * 7.2 = 6,944). Mr. Roberts indicated he would pass that information to the Director and that was all the information he needed. He did not know when or if we would hear any more from them regarding that occurrence.
