No facility representatives were present at the time of the inspection. The inspection was conducted by the authority of the Clean Water Act (CWA) to determine compliance with Environmental Protection Agency (EPA) regulations. There were no other federal, state, or local agency representatives present at the time of the inspection. FACILITY DESCRIPTION EPA conducted an inspection on the Cornerstone Energy, LLC - Careyville Landing Tank Battery, which consists of three (3) separators, four (4) crude storage tanks, and two (2) saltwater tanks. Section II - OBSERVATIONS I observed the facility sign, secondary containment, pipes, valves, and storage tanks located at the facility; see Appendix 1 (photo log), photo No. 1 and No. 2. For additional GPS location and photo metadata, see Appendix 1. I observed a cut into the secondary containment berm allowing crude oil and produced water to release outside of containment, see photo No. 3. I observed the pool of water impacted by the crude oil and produced water outside of containment, see photo No. 4. I observed loose oil outside of secondary containment, see photo No. 5. I observed an unnamed tributary north of the facility. I measured a Total Dissolved Solids (TDS) value of 23 mg/L for the unnamed tributary, see photo No. 6 through photo No. 8. I observed a pool of water also north of the facility. I measured a TDS value of 55,895 mg/L for the pool of water, see photo No. 9 through photo No. 11. I observed fresh sand covering the loose crude oil inside secondary containment, see photo No. 12. I observed a leaking tank valve near the cut in the secondary containment berm, see photo No. 13. The TDS field readings were taken with a properly calibrated, EPA issued, YSI Pro 30 conductivity meter. 2 Cornerstone Energy, LLC - Careyville Landing Tank Battery / ARU002292 (CWA) Inspection Date 1/28/2025 Section III - AREAS OF CONCERN No closing conference was held due to no Cornerstone Energy, LLC staff being present during the inspection. Areas of concern at the time of the inspection include the cut in the secondary containment berm, loose crude oil outside of secondary containment, and produced water outside of secondary containment. Section IV - FOLLOW UP Inspector will conduct follow-up phone calls with the company representatives over the coming weeks and months to ensure areas of concern are properly addressed.
