Fra tutti i tumori sperimentali osservati, l'angiosarcoma epadco e certamente quello piu pecuiiare per la sua rarita sia negli animali impiegati, sia nelTuomo, e per essere stato osservato in tutte tre le specie animali studiate, con particolare frequenza nel ratto e nei topo. Esso si presenta in tutti i gradi di differenziazione (figg. 1-6).
Le prime diagnosi di angiosarcomi sperimen tali, in animali trattati con CV per via inaiatoria furono fatte nell'estate del 1972.
Questo risultato fu reso noto, nei mesi che se* guirono, alle maggiori industrie di CV-PCV, europee ed americane, perche fossero intraprese in tal senso indagini.epidemiologiche e controlli medici sulle popolazioni esposte e perche fosse dato
inizio alia messa in atto di misure di prevenzione (Maltoni, 1974 a; Maltoni e Lefemine, 1974 a, b).
Q 19/12/1973, presso una fabbrica della B.F. Goodrich (Louisville, Kentucky, U.S.A.), veniva a morte un operaio con sintomatologia addominale. L'autopsia, eseguita in seguito, m strava trattarsi di angiosarcoma. Questo caso rappresenta la prima osservazione nota di angiosarco ma epadco professionale da CV. II caso fu pubblicato tre mesi dopo, assieme ad altri due verificadsi in periodo precedente, nella stessa fab brica, ma che non erano stad condad prima l'esposizione professionale (Creech e John 1974).

chiaro esempio di predittivita di rischio oncogeno per I'uomo.
Oltre all'angiosarcoma cpatico indagini epidemiologiche hanno portato al -riscontro, tra popolazioni lavoradve esposte, di altri tumori per i quaii la sperimentazione aveva indicato un nesso causale con il CV. Si tratta in pardcolare di tumori enccfalici, tumori polmonari, e probabilmente leucemie e linfomi, epatocarcinomi e tumori mammari
Indagini epidemiologiche retrospetdche e prospetdche ed osservazioni cliniche casuali hanno portato al riscontro, in vari Pacsi (Italia esclusa),
in particolare del Nord-America e dell'Europa Occidentale, di oltre 80 casi di angiosarcoma epadco.
La maggior parte di questi tumori si e verificata dal 1974 in poi. Non pochi di essi tuttavia erano insorti ancfae negli anni precedent^ fin dal 1955, ma in mancanza dell'indicazione sperimentale, non erano stad correttamente diaga^ sticati, data ancbe la difScolta interpretadva tenninata dalla rarita di un tale riscontro, e non correiati all'esposizione al monomero.

angiosarcomi epatici, anche sc probabilmente e il piu importante: in tempi antecedents la scoperta degli effetti oncogeni del CV e in anni successivi, sono stad identificati altri agenti capaci di provocare tumori angioblastici del fegato (Tabella 5).
I primi due casi italiani furono riportati in letteratura alcuni anni fa (Maltoni, 1974b). In que st! ultimi 6 anni si sono verificati altri due casi. j I quattro casi italiani con i dad dell'anamnesi ' lavorativa e clinico-patologica disponibili vengono qui presentad come contributo epidemiologi; co al Registro Intemazionale degli Angiosarcomi epadci da CV.
MATERIAL! E METODI
Le notizie relative all'anamnesi lavorativa provengono dalla consultazione della documentazione degli uffici del personale e/o da informazioni acquisite direttamente nella fabbrica o dal sindacato.
I dad clinici sono stad raccoiti dalle cartelle cliniche degli ospedali ove sono awenuti i ricoveri e dai servizi sanitari di fabbrica.
Per tutti i casi e stato possibile acquisire i dad autopdci disponibili, e riesaminare tutta la documentazione isto-citopatologica esistente.
RISULTATI (CASISTICA)
Caso 1. De L.G. Nato il 15/6/1934. Fino al 1964 aveva lavorato in una lavanderia. Nel 1965 inizid a lavorare presso una ditta, la PANSAC di Mira (Venezia), che
produce sacchi e contenitori van di PCV. Per un anno fu addetto al rcparto di estrusione. ove il PCV viene fuso al calore di 120, e in seguito alffmballaggio di sacchi di plastica. Nel 1968 avrebbe lavorato in Germania (non abbiamo informazioni sul tipo di lavoro) e dalTaprile J969 ri* tomo alia PANSAC.
Fino al febbraio 1970 pare che il paziente abbia sempre goduto di ottima salute; In quell'epoca egli a accorse che improwisamente, durante Q lavoro, non riusdva ad usare correttamente gii strumenti- con la mano destra, per una improwisa diminuzione della forza muscolare .
-- Prima decade gennaio 1971; compaiono coipi di tosse con emissione di espettorato mucoso, striato di sangue rosso vivo. Il paziente lamenta dolore in conispondenza dell'emiareata dentaria superiore sn. che non presenta alcuna alterazione rilevabile ispettivameme.
-- 12/2/1971; una radiografia dd torace non evidenzia
segni di lesioni pleuro-paienchimalL -- 18/2/1971 ; ricovero in una Divisione Pneumologica.
D fegato deborda di un dito dall'areata costaie ed e dolente alia palpazione; la milza e nei limiti. L'esame emocromocitometrico, resame deU'urina, la glicemia. Pazotemia, la VES, le prove sierocolloidali, la protidemia, la bilirubinemia, la transaminasemia e 1'atdvita protrombinica sono nei limid della norma. L'espettorato, sempre piu scarso, si mandene emorragico. Compaiono al matdno, per una-due ore, dolori epigastrici irradiad a sbarra, accompagnad da nausea, n paziente accusa una persistente doienzia epigastrica e ainpocondrio destro.
-- 1/3/1971: febbre intermittente, accentuazione dei dolori addominali, e vomito alimentare tale da impedire spesso del tutto l'alimentazione. Vengono rilevad anemizzazione (Hb 1U g%, G.R. 3.650.000, V.G. 0,95), leucodtosi (G.B. 10.000) e aumento della VES.
-- 2/3/1971; un esame radioiogico delie prime vie dige stive (esofago, stomaco e duodeno) non mette in evidenza aiterazioni degne di nota.
-- 4/3/1971; una radiografia del torace da il seguente esito: Emidiaframma sn. a contorni un po' irregolari e blocco pieuritico parziale del seno costofrenico. Alcune

chiazzette fibrodche al m inferiore del campo polmonare dello stesso lato. Ombra cardiaca assai ingrandita .
-- 6/3/1971: alia colecistografia. la coledsti non appare opacizzata dal mezzo di contrasto.
-- 8/3/1971: un E.C.G. mostra modeste aspecifiche alterazioni della fase di ripolarizzazione .
-- 17/3/1971: in questo periodo si fa intanto visibile una neoformazione a tipo di epulide in conispondenza della regione canino-premoiare defl'emiareata superiore sn. Una radiografia dell'emiarcata superiore sn. non mo stra lesioni osteoperiostee. L'esame istologico di una biopsia di questa neoformazione da come esito: epulide an-
giomatoso . -- Terza decade marzo 1971: compare dispnea da sfor-
zo, con decubito preferibilmente semiortopnoico e turgore delle giugulari. Un E.C.G. di controllo documents un'ischemia subepicardica diffusa. Persiste la leucocitosi (G.B. 9.900).
-- 27/3/1971: accentuazione della dispnea e del turgo re delle giugulari. D paziente viene sottoposto a pericardiocentesi con estrazione di circa 20 cc di liquido di aspctto emadco. Per l'aggravarsi delle condizioni Q paziente e trasferito al reparto di Rianimazione della Clinica Chirurgica delTUniversita di Padova ove viene effettuata una nuova pericardiocentesi con 1'estrazione di 400 cc di sangue scuro . Una radiografia del torace mette in evidenza: < discreta accentuazione del disegno in tutto Tambito pol monare, piu evidente verso le basL Velatura della meta in feriore del campo polmonare di sn^ area di velatura tra m medio e m inferiore del polmone ds. Di piu disegnatL Cuore aumentato in grade marcato, piu verso sn., a profiii piu rettilinei.
-- 28/3/1971: peraistente cianosi, turgore delle giugula ri e ipotensione arteriosa. Vengono eseguite una terza e una quarta pericardiocentesi con estrazione rispetdvamente di 450 cc e di 260 cc di sangue. Una radiografia del to race dimostra: camp: polmonari leggermente meno espansL Un po' ridotta di volume 1'ombra cardiaca. Persi ste la disoroogenea velatura a ds^ che ora sembra piu bassa.
-- n paziente viene trasferito alia Clinica Chirurgica delTUniversita di Padova ove al momento del ricovero
presents febbre (39s) e modesta dispnea da sforzo. Obiettivamente l'area cardiaca e ingrandita in toto, i toni sono ritmici e un po' lontani. presente modesto versamento addominale. La regione epigastrica e dolente alia palpazione.
-- 2/4/1971: toracentesi esplorativa sn. con estrazione di 20 cc di liquido di coiore cmatico con i caratteri di trasudato.
-- 6/4/1971: riduzione del versamento asdtico e pleurico. L'area cardiaca e meno ingrandita e si apprezza qualcbe sfregamento pericardico.
-- 15/4/1971: un vetrino con striscio di liquido pericar dico colorato secondo fi metodo di Papanicolaou viene portato in visione alllstituto di Oncologia di Bologna, per parere diagnostico. L'esame del preparato mise in eviden za cellule di narnra neoplasties, isolate ed a gruppi (figg. 7-8). Circa 1'istotipo fu subito escluso trattarsi di neoplasia epiteliale: siccome te cellule neoplastiche presentavano caratteristicbe di eiementi di rivesdmento fu avanzata I'ipotesi che si potesse trattare di un mesotelioma.
-- 16/4/1971: improwiso dolore in regione precordiale sil, accompagnato da ambascia respiratoria, sudorazione profonda e paliore intenso. Decesso.
L'autopsia mise in evidenza: -- palaio duro: posteriormente 234 sn rilievo a places di
colorito rosso vjnoso, con propaggini interdental! sporgenti suila faccia vesdbolare con rilievi polipoidi angiomatosi; -- polmone- il parenchima e disseminate di formazioni nodulari a dirnensioni variabili da un grano di riso ad una nocdola, di coiore rosso scuro emadco. Quelle piu superficial! tendono a formare placche o rilievi polipoi di per lo piu sessSi, quasi ovunque ricoperd da sierosa indenne; -- cuore: alquanto aumentato di volume soprattutto in senso trasversale, per una dflatazione di entrainbe le cavita ventriedari. 1 due foglietti della sierosa pericardies sono diftusamente adesi, anche se separabili con discreta faciiita. U cuore in toto appare awolto da uno strato di materiale di aspetto emadco, di spessore variabile da 1 mm a 1 cm. A prima vista sembra trattarsi di un coaguio emadco mtrapericardico, parzialmente

organizzato, mentre ad un esame attento risulta un tessuto che prolifera in forma di nodi fra pericardio viscerale e grasso subpericardico. L'aspetto a nodi e piu evideue a livello della parete posteriore ddl'atrio ds^ che sembra apparire infiltrato dalla proliferazione neopiastiforme. AITapertura del cuore, oltre ad una intensa dilatazione ddle cavita ventricolari, non si apprezzano altsazioni patologiche di rilievo; -- fegaio: tntta la superficie dell'organo e disseminata di piccole fonnazioni piane, grandi come capocchie di spjQo, di aspetto angiomatoso, situate subito ai di sotto della capsule, e che alia puntura si afflosdano lasciando gemere sangue. Sulla superficie di taglio si osservano rare fonnazioni anaioghe in profondita, oltre a un nodulo grande come un piseilo nel lobo ml, che presen ts gH stessi caratteri dei noduli poimonari. L'esame istologico delle neoformazioni poimonari, pericardiche e epatiche rileva trattani di un angiosarcoma
(fig. 9).
Sulla base di quanto esposto e difficile determinate con sicurezza che la sede primitive sia queila epatica, anche se questa appare la piu probabile sulla base sperimentale ed epidemiologies.
Caso 2. S.E. Nato 3 13/11/1929. Nel 1957 inizio a lavorare in un reparto di CV-PCV ddlo Stabilimento Mon tedison di Porto Marghera. Dal 1957 al 1961 fu addetto aile autoclavi (poiimerizzazioneX Nel 1961 fu nominato capoturao, funzione che egii ricopri fino al 1972.11 periodo trascorso alle autoclavi sarcbbe pertanto di 4 anni. Da ahra fonte invece tale periodo viene indicato in 6 anni (ritomo transitorio alle funzioni di autoclavista?).
L'anamnesi patologica prima del hovembre 1972 regi strar asportazione di polipo nasaie nel 1961; epistassi nel 1971 e broncopoimonite nel marzo 1972.
-- 13/11/1972: dolenzia profonda all'addome. Una radiografia del torace risulta negativa.

-- 19/11/1972: ricovero alTOspedale di S. Dona di Piave per melena e anemia acuta.
-- 24/11/1972: viene trasferito alTIstituto di Patologia Chirargica ddlTJuiversita di Padova per gravissima eraatemesi e melena.
-- Una esofagogastroscopia mette in evidenza: voluminose varici esofagee sotto notevole tensions. Su alcune di esse si osservano delle soffusioni emorragiche .
-- 29/11/1972: e sottoposto ad intervento sull'addome. Viene riscontrata una mama delle dimensioni di un me* lone, a superficie estremamente iiTegolare, reladvamente moUe (sareomaiosa), situata fra fegato, parete addominale latsrale e colon. Tale massa sembra solo appoggiare al co lon e al duodeno, mentre Tunica zona di sicura adesione e Pangolo infero-laterale del lobo epadco ds. . Viene eseguita asportazione della neoplasia, di un grosso frammento del lobo epadco ds., e di un linfonodo a ridosso della porta.
L'esame biopdco intraoperatorio da come esito neo plasia maligna ad dementi scarsamente differenziati di probabile natura epitdiaie (epatocellulare).
L'esito dell'esame istoiogico dei pezzi operatori risulta: epatocardnoma con metastasi linfonodale .
-- 15/12/1972: il paziente viene dimesso. - 27/12/1972: decesso. II riesame da noi eseguito delle sezioni istologiche alia luce dei dad sperimentali e delle prime risuhanze epidemiologiche, mette in evidenza un angiosarcoma epatico scarsamente dfflerenziato (figg. 16-12). L'aspetto del tumore in molte zone e tale da potere in realta simulare una neoplasia parenchimale.
Caso 3. V.G. Nato il 14/3/1920. Nel 1945 inizio a lavorare presso h stabilimento Societi Chimica dell'Aniene della Solvay , ove dal 1945 al 1947 ha operato nd reparto chimico e dal 1947 al 1953 nd reparto acido

doridrico . Nel 1953 viene trasferito alia Solvic di Ferrara. Dal 1953 al 1961 e stato addetto alia puiizia delle autoclavi, dal 1962 al 1974 ha operato come conduttore di autoclavi. Dal 1974 per ragioni di salute e stato trasferito al magazzino con I'incarico di numerate i sacchi vuoti, incarico che ha ricoperto fino al ricovero ospedaliero. al termine del quale e awenuto Q decesso.

Anna di Ferrare dove in seguito e stato periodicamente controllato. -- 3/4/1975: malore, per cui viene ricoverato all'Arcispedale S. Anna di Ferrara per un solo giomo.

-- 5/6/1975: radiografia dd torace presso un Radiolo go private. Secondo informazioni pare che non fossero state riscontrate lesioni pleuro-parenchimali.
-- 9/6/1975: in seguito ad emoftoe e a grave deperimento organico viene inviato al Dispensario Antitubercolare di Ferrara, per accertamend. Ivi vengono eseguiti un esame radioscopico ed two radiograiico dd torace che mettono in evidenza una decina di noduli in sede mfe
-basale Q referto, dato verbalmente sarebbe stato di spette metastasi poimonari .
-- 12/6/1975: ricovero (ultimo) presso la Divisione di Malattie Infettive dell'Arcispedale S. Anna di Ferrara (cartdla n. 18255) per ematemesi (all'anamnesi i parenti fanno rdevare che analoghi episodi si erano verificati, pur con intensita minore altre volte negli anni passati. Non viene pero riferito per quante volte e da quanto tempo). AITesame obiettivo si riscontra epatomegalia cospicue con fegato debordante tre dita dall'arcata costaie ma non indurito (tale reperto confermava i rilievi precedend dd Medico curante che gia da tempo, controilava il Paziente per epatopade senza pero corrdaria ail'anamnesi professionale), e stato anermco grave e segni di collasso cardiodrcolatorio.
Venivano effettuati i seguend accertamend: due radiograde dd torace con diagnosi di: fibrosderosi polmonare (tale giudizio diagnosdeo non coinddeva con le conciusioni degli accertamend radioiogid effettuati precedentemente al Dispensario Andtubercolare di Ferrara e su dtati, se condo cui si sarebbe trattato di sospette metastasi poi monari ); radiografia complete dd tubo digerentc con esito negative, e eolecistografia pure risultata negative
Altri accertamend, fra cui esofagoscopie sdndgrafia epadca ed eventualmente laparoscopie non sono stad ef fettuati per le precarie condizioni dd paziente Veniva praticata una intensa terapia trasfusionale.
-- 10/7/1975: decesso per emoperitoneo e collasso circolatorio periferico. La diagnosi clinica di morte parla di grave anemia con ematemesi e melena per sospetta neo plasia ddl'apparato digerente, in pregresso infarto miocardio*.

L'autopsia eseguita presso llstituto di Anatomia Patologica deU'Arrispedale S. Anna di Ferrara mise in evidenza: -- caritd eraidea: nulla ai tessuti molli pericranici. Ence-
falo normocohfonnato, presenza di modesti fenomeni arteriosclerotici a carico dei vasi del poligono; al tagiio edema cerebrate discreto; -- emitd torodea: i caw pleurici liberi da aderenze e da
versamenn contengono pochi ml di liquido di aspetto trasudadzio. I polmoni hanno un volume normale, superficie pleurica lisda, levigata e colorito pallido; al ta giio si apprezza diffuso enfisema, e presenza di piccoli
focolai di aspetto emorragico, a margini indistinti, di circa 1 cm di diametro: preiievi di minuti frammenti effettuati in corrispondenza di tali focolai, e sottoposd a docimasia, affondano; -- caro pcricardica'. contiene pochi ml di liquido giallo-citrino. II cuore ha grasso sull'epicardio nomiorappresentato; al tagiio non si apprezzano lesioni agli apparati valvolari ed Q miocardio presenta un'area circosciitta di tessuto biancastro cicatriziale in sede antero-settale; le eomnarie hanno imbocco beante, decorso tortuoso e a carico della loro superfide intimale si apprezzano piccole chiazzette lipidiche; -- cavixd addottdnaic: il arm peritoneaie, libero da ade renze, contiene circa 3 1 di materiale emadco. Dfegato (gr. 2/300) presenta superfide di colorito giailastro, con aspetto micronodulare; in corrispondenza del lobo destro la glissoniana appare lacerata e fuoriesce materiale necrotico-emorragico coagulato; al tagiio si apprezza presenza di una zona necrotico-emorragica di vasta dimensione (dd diametro di circa 10 cm) capsulata e ben riritmitMTa dal rimanente parenchima, che ha colorito
giailastro con rinforzo dello stroma. La milza (gr. 300) a cspsula normale, al tagiio appare fluente. I rent (gr. 130I'uno) si scapsuiano agevolmente, hanno superfide regolare ed al tagiio mostrano modid segni di patosi. I surreni presentano corticate ridotta e midollare malarica. Nessun reperto patoiogico a carico dello stomaco, del dmdeno, e dd restante tratto intestinale. Lo stoma co contiene residui di materiale ahmentare di colorito bruno verdastro;
-- aorta: mostra presenza di chiazze lipidiche ealcifiche in tutto il suo decorso;
-- tertebre: assenza di metastasL -- Diagnosi anatomica: arteriosclerosi lipojalinocalcifica
aortica, cerebrate, coronarica (miocardiosderosi). Voluminosa neoplasia intraepatica emorragica con perforazione della glissoniana ed emoperitoneo imponente. Piccoli focolai emorragiri polmonari muldpli bilaterali. -- Causa della matte: emoperitoneo e neoplasia epatica. I preiievi per l'esame istologico, sono stati eseguiti solo $ul fegato. L'esame istologico incite in evidenza un angiosarcoma epatico nella quasi totalita indiffermziaio o scarsamente dffferenziato e solo in un'area limitata ancora ben differenziato (ftgg. 13-16) (e presumibile che i focolai emorra giri polmonari fossero localizzazioni metastatiche). Nel contesto dd tessuto angiomatoso, si osservano aree di displasia delle cellule parenchimali epatiche (fig. 17).
Caso 4. F.B., nato fl 20/7/1937. Ha lavorato dall'aprile 1959 al dicembre 1980 presso 1'ANIC di Ravenna. I primi 8 mesi e state addetto alia produzione dd monomero. Successivamente ha sempre operate nel reparto di pro duzione di PVC in sospensione, ricoprendo diverse mansioni per period! non esattamente prerisabili: autoclavista, quadrista (i quadri si trovano nello stesso locale dd reattori), addetto alio scarico delle autoclavi, addetto alia filtrazione dd torbido, addetto aU'essicamento. Dal 1963 e stato capo squadra e dal 1975 e stato capoturno sempre operando nello stesso reparto.
-- Meta dicembre 1980; compare dolore all'ipocondrio e fianco sinistri con nausea e vomito ahmentare che nd gi ro di.pochi giomi si diffonde anche all'ipocondrio destro, e non risponde a terapia antalgica ed antispastica.
-- 29/12/1980: viene ricoverato ah'Ospedale Civile di Faenza ove viene pradcata una laparoscopia ed una biopsia epatica. All'esame istologico della biopsia risulta trattarsi di un angiosarcoma epatico moderatamente differenziato (fig. 18).
-- 2/1981: viene trnsferito presso la Clinica dd Lavoro ddFUniversita di Milana Una TAC evidenzia una massa interessame diffusamente il lobo sinistro dd fegato.

II fegato appare diffusamente interessato, nel lobo sinistro, da una massa emorragica. Sull'omento vengono riscontrati due piccoli noduli emorragici. L'esame istologico confenna la diagnosi bioptica di an giosarcoma epaiico moderatamente dffferenziato. Le mansioni, il periodo di latenza e il periodo intercorso tra i primi sintomi ed il decesso dei 4 casi descritti sono riassunti nella Tabeila 6.
CONCLUSIONI
14 casi di angiosarcoma epatico da CV, italiani vengono ad aggiungersi agli 86 descritti negii
altri Paesi: pertanto il numero attuale a livello internazionale assomma a 90.
H periodo di latenza dei singoli casi (con la so la eccezione del Caso n. 1) e medio, il decorso clinico e il quadro autoptico degli angiosarcomi riportati, sono anaioghi a quelli dei casi descritti negii altri Paesi (Tabeila 6).
Dal punto di vista microscopico gli angiosar comi riferiti appaiono diffusi e con aspetti multiccntrici, e presentano un diverso grading (fino alle forme piu scarsamente differenziate: angioblastomi): tale dato ha un preciso riscontro non so lo negii angiosarcomi professional! da CV non italiani, ma anche negii angiosarcomi umani da agenti noti, o a causa ignota ( spontanei ), e

L'associazione tra angiosarcoma e displasia del tessuto epatico riscontrata nel Caso n. 3 e in linea con l'osservazione sperimentaie di angio sarcomi epatici ed epatocarcinomi associate nei rarti trattati con CV in eta neonataie e con un caso di associazione angiosarcoma-epatocarcinoma, in un lavoratore canadese di CV-PCV, riferito da Delorme (1978). La possibility di una tale associazione si comprende dalla storia naturale deU'angiosarcoma, ricostruita suila base di osservazioni condotte su materialc sperimentaie e clinico (Tabella 7).
I nostri dad confermano indue la rapida evolutivita della maiattia, la cui unica possibility di controllo si rawisa sempre di piu solo nella mes-
sa in atto di opportune misure di protezione ambientaie.
RIASSUNTO
Dopo una breve rassegna dd dad fondamentali suila cancerogenesi da cloniro di vinile (CV) vengono riferid i primi 4 casi di angiosarcoma epadco osservad in Ttii in operai esposd ai monomero.
I primi due casi erano gia stad pubblicad in passato (Maltoni, 1974).
In tre di quesd casi (Casi n. 2,3,4) si tratta di operai di repaid di polimerizzazione; l'altro caso (Caso n. 1) si riferisce a un operaio addetto all'estrusione di PCV.
Eccetto questo ultimo caso, il periodo di latenza degli angiosarcomi descritd e analogo a qudlo do casi non italiani riportad neila letterarura internazionaie.

SUMMARY
After a brief review on the basic data on Vinyl Chloride (VC) carcinogenesis, the first four cases of liver angiosar comas observed in Italy among workers exposed to the monomer are extensively reported. The cases 1 and 2 were already published (Maltoni, 1974).
Three of the cases (Cases 2. 3 and 4) are dealing with | workers of polymeradon units; the other one (Case 1) deal ( with a PVC bog extruder.
A part of tiusTast case, the latency time of the described . angiosarcomas is similar to that of the non Italian ones reA-'i 'j ^ ported in thfr-.intcmational Literature.

INTRODUCTION
1971 marked the start of a large-scale programme of Integrated experimental research Into the carcinogenic properties of vinyl chloride monomer (VCM) at the Institute of Oncology and Tumour Centre, Bologna.
The experiments were designed to investigate the effect of VCM administered in various ways (by inhalation, ingestion and intraperitoneal and subcutaneous injection) at various concentrations and over various periods, continuously and intermittently, to animals of different Bpecies (rats, mice, hamsters), strains (Sprague-Dawley and Wistar rats), sexes and ages.
The pr ject lasted for 10 years, involved some 7000 animals with a total of ^3,000,000 days' administration and entailed the examination of about 200,000 histological preparations. It was the most extensive experimental project ever carried out on a single material by a single organisation.

These results formed the object of a number of reports Issued by the Institute of Oncology (Maltoni, 1974a, 1977; Maltoni and Lefemine, 1974 a, , b, 1975; Maltoni et al, 1974, 1980).
Hepatic angiosarcoma Is certainly the most noteworthy of the experimental 1 tumours we observed because of its rarity in both experimental animals and
man and because It was found In all three species of animal under investigation, being particularly frequent in the rat and mouse. It presents in all degrees of differentiation (Figs 1-6).
j Experimental angiosarcomata were first observed in the summer of 1972 in animals to which VCM had been administered by inhalation.
During the next few months these findings were disclosed to the leading European and American VC and FVC producers so that epidemiological investigations and medical checks could be carried out on groups of exposed persons and a start could be made with the implementation of preventive measures (Maltoni, 1974a; Maltoni and Lefemine, 1974a, b).
On 19 December 1973 an operative at a B F Goodrich plant in Louisville, Kentucky, USA died after exhibiting abdominal symptoms. The resulting autopsy revealed the cause to be angiosarcoma. This is the first recorded case of occupational hepatic angiosarcoma attributable to VCM. The case history was published 3 months later together with two others that had occurred earlier at the same plant but had not been thought previously to have any connection with occupational exposure (Creech and Johnson, 1974).

The results of experimental carcinogenesis by VCM with particular reference.to hepatic angiosarcomata are currently the most obvious illustration of a predictable oncogenic hazard for man.
Epidemiological investigations on groups of workers exposed to VCM have revealed the existence of tumours other than hepatic angiosarcomata that

would appear, from experimental findings, to be attributable to it. In particular brain tumours, pulmonary tumours and probably leukaemias and lymphomas, hepatocarcinomas and mammary tumours have been found.
Retrospective and prospective epidemiological investigations and chance observations have revealed more than 80 cases of hepatic angiosarcoma in a number of countries (not Including Italy), particularly in North America and Western Europe.
The majority of these tumours were diagnosed from 1974 onwards. A fair proportion, however, had arisen before this, as far back as 1955, but had not been correctly diagnosed for lack of experimental data since it was difficult to interpret the findings because of the rarity of the condition, and no correlation had been established between the incidence of the tumours and exposure to VCM.
Practically all the cases were observed in operatives oaployed on polymerisation and only a few involved workers exposed under other circums tances.

The number of cases of angiosarcoma attributable to 7CM observed to date (excluding the figures for Italy) and their breakdown in terms of geographical distribution and the type of work carried out by the operatives concerned are presented in Tables 2-4, together with data on annual incidence and the latency period.

VCM is not the only agent that can induce hepatic angiosarcomata although it is probably the most important. Other agents capable of inducing angioblastic liver tumours were Identified both before and after the discovery of the oncogenic effect of VCM (Table 5).

The first two cases in Italy were reported in the literature a few years ago (Maltonl, 1974b). Two more cases have occurred during the last 6 years.

The four Italian cases, with such data as are available on the patients' occupational and dlnico-pathologlcal case history, are presented here as an epidemiological contribution to the International Register of Hepatic Angiosarcomata Attributable to VCM.
MATERIALS AMD METHODS
The notes on the occupational case histories are based on personnel department records and/or Information obtained directly from the plant or trade union concerned.
The clinical data were obtained from the files of the hospitals to which the patients were referred and from the works medical departments.
In all cases we were able to consult such autopsy findings as were available and to re-examine all the existing hlstopathologlcal and cytopathologlcal records.
CASE HISTORIES
Case 1
De L G. Bom 15 June 1934. Worked In a laundry up to 1964. In 1965 started work at PANSAC, a company that manufactured a variety of PVC bags and containers at Mira (Venice). Employed for one year In the extrusion shop, where PVC was melted at 120C, and subsequently In the bag packaging section. Worked In Germany during 1968 (we have no Information on the type of work) and returned to PANSAC in April 1969.

The patient appears to have enjoyed excellent health up to February 1970, at which point he noticed that at unpredictable intervals during his work he was unable to use tools correctly with his right hand because of sudden "muscular weakness"
- First 10 days of January 1971: fits of coughing, expectorating mucus streaked with bright red blood. The patient complained of pain in the left superior dental arch which exhibited no visible signs of deterioration.

- 12 February 1971: A chest X-ray showed no signs of pleuro-parenchymal lesions.
- 18 February 1971: Admitted to a chest unit. The liver was found to project
beyond the costal arch by the width of a finger and was painful on palpation.
The spleen remained within bounds. Haematocrit level tests, urine
blood
sugar tests, test for asotaemia, VES, tests for serocolloids, proteinaemia,
bilirubinaemia, transaminases and prothrombin activity were within the normal
limits. The sputum, which became progressively more scanty, still contained
blood. Outwardly radiating epigastric pains occurred for one to two hours in
the mornings, accompanied by nausea. The patient complained of a persistent
ache in the epigastric region and right hypoehondrium.
- 1 March 1971: Intermittent fever, accentuation of abdominal pains and vomiting of such severity that it was often impossible to take food at all. Signs of Increasing anaemia were observed (Hb 11.2 gZ, red cells 3,650,000, Cl (colour index)- 0.95), also leucocytosis (GE 10,000) and Increase in the VES

- 2 March 1971: Radiological examination of the oesophagus, stomach and duodenum revealed no deterioration worth noting.
- 4 March 1971: An X-ray of the thorax produced the following findings: "outline of left hemidiaphragm somewhat irregular with partial pleuritic block, of costophrenic angle. Some flbrotlc spots in lower third of pulmonary field on the same side. Cardiac shadow greatly enlarged".
- 6 March 1971: On colicystography the gall bladder was not rendered opaque by the contrast medium.
- 8 March 1971: An ECG revealed "moderate non-specific deterioration of the repolarisation phase".
- 17 March 1981: A neoplasm of the epulis type had meanwhile appeared in the canine/premolar region of the left superior dental arch. An X-ray of the left superior dental arch revealed no periosteal lesions. Biopsy showed that the neoplasm was an "angiomatous epulis".
CO - Last 10 days of March 1971: The patient exhibited dyspnoea on exertion and swelling of the jugular veins and breathing was easiest in the semi-recumbent position. An ECG check revealed widespread cardiac ischaemia. Leucocytosis was still present (GB 9,900).

- 27 March 1971: Accentuation of dyspnoea and swelling of the jugular veins. Pericardiocentesis was performed and some 20 cc of a fluid of haematic appearance were extracted. As his condition had worsened, the patient was transferred to the intensive care unit of the Surgical Department of the University of Padua, where pericardiocentesis was again carried out, resulting in the extraction of 400 cc of "dark coloured blood". An X-ray of the thorax revealed "slight accentuation of the outline throughout the pulmonary region, more noticeable towards the base of the lungs. Clouding of the left lower half of the pulmonary area, clouding between the middle and lower third of the right pulmonary area. Hilum more clearly defined. Marked enlargement of the heart, more towards the left side, more rectilinear in outline".
- 28 March 1971: Persistent cyanosis, swelling of jugular veins and arterial hypotension. A third and fourth pericardiocentesis were performed, with the extraction of 450 cc and 260 cc of blood respectively. An X-ray of the thorax revealed "pulmonary areas slightly less distended. Volume of cardiac shadow slightly reduced. Non-homogeneouB clouding on the right still present but now appears to be lower down".
- The patient was transferred to the Surgical Department of the University of Padua and was found on admission to have a high temperature and moderate dyspnoea on exertion. On examination the whole of the cardiac area was found to be enlarged, the heart beats were rhythmical and somewhat faint. The epigastric region was painful on palpation.

- 2 April 1971: Explorative thoracocentesis on the left side, resulting in the extraction of 20 cc of a fluid the colour of blood with the characteristic features of a transudate.
- 6 April 1971: Reduction of ascites and pleural effusion. The cardiac area was less enlarged and a certain amount of pericardial friction was observed.
- 15 April 1971: A glass slide bearing a smear of pericardial fluid coloured by Papanicolaou's method was examined at the Institute of Oncology, Bologna for diagnostic purposes. Examination of the preparation revealed cells of a neoplastic nature, both isolated and in groups (Figs 7-8). It was immediately concluded from the histological findings that there was no question of epithelial neoplasia. As the neoplastic cells exhibited the characteristics f lining elements, the hypothesis was advanced that this could be a ease of mesothelioma.
- 16 April 1971: Sudden pain in the left precordial region accompanied by breathlessness, heavy sweating and intense pallor. Death.
Autopsy Findings
- Hard Palate: Behind upper 234 teeth left, flat wine-coloured excrescence with interdental offshoots projecting over the vestibular aspect with angiomatous polypoid excrescences.
- Lungs: The parenchyma was scattered with nodular formations varying in size from a grain of rice to a hazel nut and deep blood red in colour. The more superficial tended to form flat or polypoid excrescences, mostly sessile and c vered almost everywhere with unbroken serous membranes.

- Heart: Somewhat Increased in volume, particularly in the transverse direction through dilation of both ventricles. The two flaps of pericardial membrane exhibited widespread adhesions although they could be separated fairly easily. The heart as a whole appeared to be wrapped in a layer of material of haematic appearance (sic) whose thickness varied from 1 mm to 1 cm. At first sight this appeared to be a partially organised intrapericardial haematic coagulum, but closer examination revealed it to be a tissue proliferating in the form of nodules between the visceral pericardium and the subpericardial fat. The nodular appearance could be more clearly seen at the posterior wall of the right atrium which appeared to have been infiltrated by the proliferating neoplasm. No pathological deterioration of any significance apart from marked dilation of the ventricular cavities was found when the heart was opened.
- Liver: The entire surface was scattered with small flat formations the size of pinheads, angiomatous in appearance and located just below the capsule, which became flabby and exuded blood when punctured. The cut surface also exhibited a few formations of similar depth as well as a nodule the size of a pea in the left lobe which presented the same characteristics as the pulmonary nodules
Histological examination of the pulmonary, pericardial and hepatic neoplasms revealed that they were angiosarcomata (Fig 9).
It is difficult to establish for certain on the basis of the above report whether the primary site was the liver, though this would appear to be the most likely in the light of the experimental and epidemiological findings.

Case 2 S E, bora 13 November 1929. Started work In the VC/PVC division of the Montedison plant at Porto Marghera In 1957. Worked on the autoclaves (polymerisation) from 1957 to 1961. Appointed shift foreman In 1961, a position he held until 1972. He would therefore appear to have spent 4 years working on the autoclaves.
Another source gives this period as 6_ years, however (did he perhaps have another temporary stint as autoclave operator?).
Case history before November 1972: Removal of a nasal polyp in 1961; nosebleeds in 1971 and bronchial pneumonia in March 1972.
- 13 November 1972: Severe abdominal pain. Chest X-ray negative.
- 19 November 1972: Admitted to Ospedale di S Dona, Piave, with melaena and acute anaemia.
- 24 November 1972: Transferred to Department of Surgical Pathology, University of Padua with very severe haematemesis and melaena.
Oesophagogastroscopy revealed: "Massive oesophageal varices under considerable tension, some of which exhibited haemorrhagic suffusion".

- 29 November 1972: Abdominal operation. Finding: "a mass the size of a melon with an extremely irregular surface, relatively soft (sarcomatous) situated between the liver, lateral abdominal wall and colon. It seemed merely to be resting against the colon and duodenum and the only confirmed point of attachment was the lower lateral angle of the right lobe of the liver". The neoplasm, a large portion of the right lobe of the liver and a lymph node behind the hepatic-portal vein were removed.
An intraoperative biopsy revealed "a malignant neoplasm with poorly differentiated elements probably epithelial (hepatocellular) in nature".
Result of the histological examination of the material removed during the operation: "Hepatocarcinoma with lymphonodal metastases".
- 15 December 1972: Patient discharged.
- 27 December 1972: Death.
Our re-examination of the histological sections in the light of the experimental data and the initial results of the epidemiological study revealed the presence of a poorly differentiated hepatic angiosarcoma (Figs 10-12). The appearance of many parts of the tumour was such that it could in fact be taken for a parenchymal neoplasm.

Case 3 V 6. Born 14 March 1920. Started to work at the Societa Chimica dell'Aniene, which Is owned by Solvay, In 1945. Worked In the Chemical Section from 1945 to 1947 and the Hydrochloric Acid Section from 1947 to 1953. Transferred to Solvlc Ferrara in 1953, where he was employed as an autoclave cleaner from 1953 to
*
1961 and an autoclave operator from 1962 to 1974. In 1974 he was transferred for health reasons to the Stores where he was responsible for counting empty bags. He continued to work at this until his admission to hospital, which preceded death.
It will be helpful to outline the conditions under which VG worked when employed as an autoclave cleaner from 1953 to 1961. The way in which cleaning was carried out at Solvlc from 1953 to 1967 and the degree of exposure of the operatives were as follows:
- The autoclave was discharged when polymerisation was terminated.
- As soon as the contents had been removed it was filled with' water;
- The water was then discharged;
- Once the autoclave was empty, the cleaners entered (they came in quickly because they found the autoclaves easier to clean while the walls were wet and cool);
- According to information obtained by technical means the VC concentration in the autoclave during cleaning by this method varied from 200 to 2000 ppm;

- The cleaning tine per autoclave varied with the type of polymerisation. This factor is of minor importance however as the job of autoclave cleaner presupposes that the cleaner spends time in several autoclaves.
The patient's pathological case history from 1965 to 1974 records: sacrolumbar arthrosis with episodes of scapulohumeral arthralgia, chronic emphysematous bronchitis (he smoked about 20 cigarettes a day), periods of depression alternating with periods of ready excitability, insomnia, poor digestion in the later stages, a feeling of heaviness, sometimes acute pain in the hepatic region and an increase in the volume of the liver which extended 3 fingers' width beyond the costal arch.
- 23 June 1974 to 7 December 1974: Admission to Cardiology Unit of the Arcispedale* S Anna, Ferrara due to myocardial infarction, followed by regular check-ups.
- 3 April 1975: Illness which resulted in admission to the Arcispedale S Anna, Ferrara for 1 day only.
- 16-24 April 1975: Absent from work for health reasons.
- 5 June 1975: Chest X-ray by a private radiologist. According to our information "no pleuro-parenchymal lesions were found".
- 9 June 1975: Following haemoptysis and a serious deterioration in his general condition the patient was sent to the TB Clinic, Ferrara for tests. A radioscopic examination was carried out together with a chest X-ray, which revealed around 10 nodules in the mediobasal area. The verdict, given verbally, was said to he "suspected pulmonary metastases".

- 12 June 1975: (Final) admission to the Infectious Diseases Unit of the Arclspedale S Anna, Ferrara (File No 18255) due to haematemesis (according to the case history, relatives said that similar but less severe episodes had occurred on other occasions in previous years* How often they occurred and how long they had been doing so was not disclosed)* Examination revealed pronounced hepatomegaly, with the liver extending 3 fingers' width beyond the costal arch, though not sclerotic (this confirms the earlier findings of the doctor in charge of the case who had been treating the patient for liver dysfunction for some time but without correlating it with VG's occupational case history); the patient also presented severe anaemia and signs of cardlocirculatory collapse.
The following tests were carried out: two chest X-rays leading to the diagnosis of pulmonary fibrosclerosls (this diagnosis conflicted with the conclusions of the radiological checks carried out earlier at the Ferrara TB Clinic as reported above where the verdict was "suspected pulmonary metastases"), radiography of the complete digestive tract (negative) and colecystography (again negative).
The patient's condition .was too precarious to permit further tests such as oesophagoBcopy, hepatic scintigraphy or a possible laparoscopy. Intensive transfusion therapy was carried out.
- 10 July 1975: Death due to haemoperltoneum and peripheral circulatory collapse. Clinical diagnosis of the cause of death was "severe anaemia with haematemesis and melaena due to suspected neoplasia of the digestive organs preceded by myocardial infarction".

The findings of the autopsy performed at the Department of Pathological Anatomy at the Arclspedale.S Anna, Ferrara were as follows:
- Cranial Cavity: Nothing on the soft pericranial tissues. Configuration of the encephalon normal, slight evidence of arteriosclerosis In the vessels of the brain. Moderate cerebral oedema observed on section.
- Thoracic Cavity: The pleural cavities, which were free from adhesions and effusions, contained a few ml of a fluid resembling a transudate In appearance- The lungs were of normal volume. The surfaces of the pleurae were smooth, glossy and pallid. Section revealed widespread emphysema and the presence of small areas of haemorrhagic appearance about 1 cm In diameter and with Indistinct boundaries. Minute fragments taken from these areas and subjected to docimasla sank.
- Pericardial Cavity: Contained a few ml of a lemon-coloured fluid. Fat was observed on the eplcardium. Incision revealed no damage to the valves of the heart but the myocardium exhibited a circumscribed area of whitish scar tissue In the anteroseptal region; the coronary vessels were open-mouthed and tortuous and the lntlmal surfaces exhibited small llpldic spots.
- Abdominal Cavity: The peritoneal cavity, which was free from adhesions, contained about 3 1 of haematic material. The liver (2,300 gr) was yellowish in colour and mlcronodular in appearance; the capsule of the right lobe appeared to be tom and coagulated necrotic haemorrhagic material was emerging. Incision revealed a vast necrotic haemorrhagic area (about 10 cm in diameter) with a capsule and clearly distinguished from the remaining parenchyma which was yellowish in c lour with stiffening of the stroma.

The spleen (300 gr) had a normal capsule and appeared fluid* when cut. The kidneys (130 gr each) could be readily decapsulated, had a uniform surface and showed signs of moderate pathosls after section. The suprarenal glands exhibited a shrunken cortex and abnormally softened medulla* No pathological findings were obtained in respect of the stomach, the duodenum and the rest of the intestinal tract. The stomach contained residues of greenish-brown food.
- Aorta: Calcific llpidic spots throughout its length.
- Vertebrae: Absence of metastases.
- Anatomical diagnosis: Lipohyalincalcific, aortic and cerebral arteriosclerosis, also coronary arteriosclerosis (myocardiosderosis) Massive haemorrhagic lntrahepatic neoplasia with perforation of the liver capsule and considerable haemoperitoneum. Numerous small bilateral haemorrhagic areas on the lungs.
- Cause of Death: Haemoperitoneum and hepatic neoplasia. Histological tests were carried out on tissues from the liver only. They revealed the presence of an almost completely undifferentiated or poorly differentiated hepatic angiosarcoma within a restricted area that was still clearly differentiated (Tigs 13-16) (the haemorrhagic areas on the lungs may well be localised metastases). Areas of dysplasia In the hepatic parenchymal cells were observed against the surrounding angiomatous tissue (Fig 17).

Case 4 F B. Bom 20 7 1937. Worked at ANIC Ravenna from April 1959 to December 1980. He was employed on monomer production for the first 8 months. From then onwards he worked exclusively in the suspension PVC production plant where he carried out various functions for periods that cannot be specified precisely; he worked as an autoclave operator and console operator (the control panels were in the same shop as the reactors) and was also employed on discharging the autoclaves, filtering the slurry and drying the product. He was team leader from 1963 onwards and shift foreman from 1975 onwards, still working in the same department.
- Middle of December 1980: Complained of pain in the left hypochondrium and left side with nausea and the vomiting of food. Over the next few days this spread to the right hypochondrium and did not respond to analgesic antispasmodic therapy.

- 29 December 1980: Admitted to the Ospedale Civile, Faenza where laparoscopy and a liver biopsy were performed. Histological examination of the resulting tissues revealed the presence of a moderately differentiated hepatic angiosarcoma (Fig 18).
- February 1981: Transferred to the Clinics del Lavoro of the University of
Milan. A scan revealed a mass extensively involving
the left lobe of the
liver.
- 2 April 1981: Death due to haemoperltoneum.

Autopsy Findings
- Abdominal Cavity: The peritoneal cavity contained a large amount of haematic material. A haemorrhagic mass involving the left lobe of the liver was also present* Two small haemorrhagic nodules were found on the omentum.
Histological examination confirmed the biopsy diagnosis of moderately differentiated hepatic angiosarcoma*
The type of work carried out by the patient, the latency period and the period that elapsed between the first symptoms and death are presented in Table 6 for all four cases.
CONCLUSIONS
The 4 Italian cases of hepatic angiosarcoma attributable to VCM plus the 86 recorded in other countries make an international total of 90*
The latency period in the individual cases (apart from Case 1) is of average length and the clinical history and autopsy findings on the angiosarcomata are similar to those described in other countries (Table 6).

When examined under the microscope the angiosarcomata were found to he widespread, multi-centred and of various gradings (down to the forms most difficult to differentiate : angioblastomas). Exactly the same findings were obtained In the VCM-induced cases of occupational angiosarcoma outside Italy, in human angiosarcomata of both known and unknown ("spontaneous") origin and the experimental angiosarcomata induced by VCM and vinyl bromide.
The association between angiosarcoma and dysplasia of the hepatic tissue observed in Case 3 is consistent with the experimental observation of associated hepatic angiosarcomata and hepatocarclnomas in rats treated with VCM at the neonatal stage and a case of associated angiosarcoma and hepatocarclnoma in a Canadian VC/PVC worker reported by Delorme in 1978. The possibility that such an association may exist is apparent from the natural history of angiosarcomata reconstructed from observations on experimental and clinical material (Table 7).

Finally, our data confirm the rapid development of the disease and the fact that the only possible method of checking it is by carrying out the appropriate envir nmental protective measures.
SUMMARY
After a brief survey of the basic data on vinyl chloride (VCM) carcinogenesis a report is given on the first 4 cases of hepatic angiosarcoma observed in Italy In workers exposed to the monomer.
The first two cases had already been published (Maltoni, 1974).
Three of the cases (Cases 2, 3 and 4) concerned workers in polymerisation plants. The other cases (Case 1) involved an operative employed in a PVC extrusion shop.
Except for this latter case, the latency periods of the angiosarcomata described in this paper are similar to those of the non-Italian cases reported in the international literature.
These four Italian cases bring the total number of cases of hepatic angiosarcoma in workers exposed to vinyl chloride to 90.
