This was an EPA follow-up inspection in response to an initial spill inspection conducted 4/23/2025 (NRC No. 1429257). The inspection was conducted by the authority of the Clean Water Act (CWA) to determine compliance with Environmental Protection Agency (EPA) regulations. There was no other federal, state, or local agency representative present at the time of the inspection. FACILITY DESCRIPTION EPA conducted an inspection at the Calgary Energy, LLC, Tidal Osage 1 Lease, where the Tidal Osage 1 well 1-2 released oil to the surface from a subsurface casing leak. Section II - OBSERVATIONS I observed the release point for the spill which was the Tidal Osage 1 well 1-2. The well was temporarily abandoned and was not in production at the time of the inspection. A catchpit had been installed at the location. I observed crude oil on the surface of the catchpit. See photos No. 1 and No 2. For additional GPS location information and photo metadata, please see Appendix 1 (photo log) attached to this report. I observed an additional catchpit at the spill location which did not contain crude oil. See photo No. 3. Section III - AREAS OF CONCERN The primary area of concern was the crude oil located in the catchpit. No progress was made to prevent recurrence of the spill event. More than 45 days had passed since the spill inspection on April 23, 2025. A temporary solution was applied to the well in 2021 but failed to prevent recurrence of the purging event. A permanent solution to the failed well casing should be achieved to prevent additional recurrence of the spill event. Section IV - FOLLOW UP EPA inspector conducted a follow-up phone call with Calgary Energy, LLC representative, Blaine Haag, at the end of the inspection. Mr. Haag stated he was submitting an Osage 139 form to the BIA Osage Agency to begin the process of repairing or plugging the well. 2 Calgary Energy, LLC / Tidal Osage 1 Lease CWA Inspection Date 06/09/2025 EPA inspector will conduct follow-up calls, field meetings and inspections to ensure the impacted area is returned to "pre spill" condition. EPA inspector will continue follow-up with Calgary Energy, LLC to observe the permanent solution to the recurrent purging problem.
