Dickson SUBJECT: December Safety Inspection The December safety inspection team consisted of the following Union Carbide personnel: G. L. Dickson (Leader) D. A. Lepiano (Recording Secretary) J. M. Moyer G. J. Spencer (Absent due to illness) M. J. Young Bill Leehan (Hourly representative) During this inspection tour emphasis was placed on the general safety program, within this department, with particular attention given to housekeeping, signs, and fire extinguishers. The overall condition of the areas inspected was con sidered extremely good, reflecting the quality of the previous inspections and the corrective actions which followed. All of the fire extinguishers examined were inspected within the past five months. Housekeeping was generally rated as good,with some exceptions which have been noted in the following. All safety and exit signs were in working order with the exception of two (also noted). The major criticism found relates to the large number of offices which contain "temporary" electrical extension cords and telephone leads. In many cases, these are strung over radiators and along floors constituting potential tripping hazards and possible insulation failure. A .'8 03 3 Building 94 2nd Floor Coffee Room - Reconmend three-prong plug for automatic coffee maker and double outlet box with switch on one outlet. (Presently only able to turn off tea kettle by pulling plug.) Room 280 - Telephone wire on floor a tripping hazard. Room 285B - Electrical outlet by desk does not work. Room 276 - Loose ceiling tiles. Room 284 - Leaking radiator. Room 205 - Poor housekeeping. Floor cluttered with unused equipment. 1st Floor Room 123 - Electrical extension cord draped over radiator. Room 180 - Needs additional electrical outlet near desk. Rooms 172 & 174 - Loose phone cords need protective covering. Hallway outside Room 186 - Ceiling tile should be put back in proper position. Basement - Safety station light opposite Room 52 is out. Room 52 Room 54 Buildinq 95 Ceiling tiles need replacing. Phone cord hanging from ceiling. - EXIT light on north end of building is out. - Nitrogen cylinder near vacuum furnace improperly secured. South end of building needs better housekeeping.
